The transmastoid partial labyrinthectomy approach to medial skull base lesions.

INTRODUCTION: It has long been thought that surgical disruption of the membranous labyrinth invariably results in sensorineural hearing loss and balance dysfunction. Recent evidence suggests that the inner ear can withstand such manipulation without loss of function. The technique of transmastoid partial labyrinthectomy has recently been described as a means of providing access to lesions of the medial skull base by removing part of the labyrinth and at the same time attempting to preserve hearing and vestibular function of the lateral semicircular canal (LSCC) and otolithic organs. PROCEDURE

The widened retrolabyrinthe approach: a new concept in acoustic neuroma surgery.

For many years, the retrolabyrinthine approach has been limited to functional surgery of the cerebellopontine angle (CPA). As a result of the increased surgical exposure, particularly the opening of the internal auditory meatus (IAM), the widened retrolabyrinthine technique permits tumor excision from both the CPA and the IAM, regardless of the histological nature of the tumor. The authors have treated 60 acoustic neuromas of varying sizes via this approach (6% intrameatal tumors; 30% > 25 mm in diameter). The postoperative mortality rate was 0%. The risk of fistula formation was 3.3%, and 3.3

Evaluating 3D semicircular canal function by perception of rotation.

OBJECTIVE: We aimed to develop a clinical test of 3D canal dysfunction by perceptual matching of motion stimuli as an alternative to vestibular ocular reflex assessment. STUDY DESIGN: The study was a comparison with age-matched controls. SETTING: The study was performed in a clinical neurophysiology laboratory. PATIENTS: Ten patients with acute unilateral vestibular nerve section and 9 patients in the chronic stage of recovery, 2 acute-stage and 2 chronic-stage patients with posterior canal plugging, and 35 healthy individuals were studied. INTERVENTIONS: Seated on a motorized rotating cha

Mechanisms of auditory impairment during acoustic neuroma surgery.

Hearing loss during removal of acoustic neuroma (AN) may be due to labyrinthine and/or neural and/or vascular damage. Surgical maneuvers relating to perioperative and postoperative hearing may give rise to mechanisms of auditory impairment. Recording action potentials from the intracranial portion of the cochlear nerve (CN) has proven particularly useful for identifying the mechanisms of iatrogenic auditory injury. In this paper intraoperative and postoperative auditory impairments are investigated in relation to surgical steps in a group of 47 subjects with AN (size ranging from 5 to 25 mm) u

Neurotransmitters of the olivocochlear lateral efferent system: with an emphasis on dopamine.

The olivocochlear lateral efferent system (OLES) of the adult mammalian cochlea uses variety of neuroactive substances, such as acetyl choline, GABA, dopamine (DA), enkephalins, dynorphins and CGRP. These neuroactive substances have been located within the efferent, small and dense matrix, fibers and terminals of the inner spiral and tunnel bundles. However, some of these neuroactive substances have also been found outside the OLES. Acetyl choline and CGRP, for instance, appear within the olivocochlear medial efferent fibers, and DA and CGRP may also be present in the perivascular innervation.

Endolymphatic hydrops associated with otosclerosis.

Endolymphatic hydrops (EH) associated with otosclerosis has been noted for many years. However, the causal relationship of these two entities remains controversial. Having reviewed the records of patients with otosclerosis describing fluctuant hearing loss and vertiginous symptoms, the authors found the EH may coexist with otosclerosis preoperatively; they may be two separate diseases that exist coincidentally; or EH may be caused by the otosclerotic process. Secondly, EH may occur with a fistula after surgery. Occurring after stapedectomy, EH may be caused by fistulization of the bony labyrin

[Evoked otoacoustic emissions in noise-induced hearing loss].

The Authors evaluated patterns of click-evoked otoacoustic emissions (EOE) in a group of 184 patients exposed to noise in their working environment. The aim of the study was to detect changes in EOE patterns in this group compared to the EOE of healthy subjects. The clinical history of each subject was recorded and otoscopy, pure tone audiogram and impedance tests (tympanometry, stapedial reflex threshold) were carried out in all patients. Noise-induced hearing loss found in 97 patients, unilateral sensorineural hearing loss in 17, other hearing disorders in 24 and normal auditory threshold in

Petrous apex cholesteatoma: diagnostic and treatment dilemmas.

The diagnosis and treatment of petrous apex cholesteatoma is a difficult surgical challenge. This study is a review of 14 cases of cholesteatoma involving the petrous apex. These cholesteatomas originated as a congenital primary lesion or secondary to an acquired lesion. The cases were evaluated according to the clinical features, the intraoperative findings, the radiological findings, and the surgical approaches. In this series, 83% of the patients presented with hearing loss and 50% presented with facial nerve weakness or paralysis (House grade II to VI). Intraoperative and radiological feat

Stability of the head: studies in normal subjects and in patients with labyrinthine disease, head tremor, and dystonia.

The dynamics of postural control of the head were investigated in normal human subjects and patients with neurological disease. The technique adopted was to measure the head movements provoked by passive, unpredictable oscillations of the trunk in the frequency range 0-6-7 Hz when subjects were required (a) to try to stabilise their head "in space" and (b) to try to make their heads move "en bloc" with the trunk. Head movement responses were characterised by the gain and phase with respect to trunk movement (transfer function) and degree of linear relationship between head and trunk (coherence

Cochlear endolymphatic shunt for Meniere's disease.

We have now performed the cochlear endolymphatic shunt operation for Meniere's disease on 102 ears and have follow-up data on ninety cases. In this fourth report of results of the procedure, we show that after follow-up ranging from one month to six years (average 22.3 months), definitive vertigo was relieved in 72% of the cases and that hearing at the time of most recent audiogram (average 12.6 months postoperatively) was worse (AAO criteria) in 45% of the cases.

[Labyrinthine and extralabyrinthine mechanisms of the development of motion sickness in weightlessness].

This paper describes an important problem of aerospace medicine, i. e. space motion sickness. The review surveys relevant Soviet and foreign publications. On their basis, including their own data, the authors discuss possible mechanisms of the sickness symptom-complex in the weightless state. The authors put forth their own ideas concerning current hypotheses of space motion sickness and discuss potential applicability of the theoretical concepts with respect to the prevention of this adverse symptom-complex. The authors describe selected outlines in the study of mechanisms of space motion sic

The ultrastructure of the endolymphatic sac in man.

The present investigation describes the ultrastructure of the human endolymphatic sac in surgical biopsy specimens from 14 patients with acoustic neuroma and Ménière's disease. The intent of the study is to illustrate the normal appearing human endolymphatic sac by transmission electronmicroscopy (TEM) and to detail histopathologic alterations which occur in patients with acoustic neuroma and Ménière's disease. Normal endolymphatic sac histology is elucidated and provides a basis for discussing pathologic changes seen in disease states. The endolymphatic sac undergoes similar histopathologic c

[Human visual functions during mild vestibular exposures].

Human experiments have shown that after mild stimulation of semi-circular receptors, which induces no motion sickness symptoms, the time of recovery of the central vision acuity shortens and after stimulation of otolith receptors tends to increase. Thresholds of color discrimination as applied to the red, green and violet colours as well as the course of back adaptation after mild stimulation of otolith receptors remain unchanged. The time of appearance of the Purkinje phenomenon after stimulation of receptors of semi-circular canals often remains unaltered and sometimes increases. It is assum
