Late emergence of myasthenia gravis complicating envenoming by Viperaammodytes.

INTRODUCTION: Vipera ammodytes, is the most important medically venomous snake in North Macedonia and one of the most dangerous species in Europe. CASE PRESENTATION: A 41-year-old construction worker sustained a bite to the distal phalanx of his right thumb while lifting a brick. The injury was followed by local swelling and visual disturbances. Initial management at a regional medical center included administration of specific antivenom, tetanus prophylaxis, methylprednisolone, chloropyramine, prophylactic anticoagulation, and antibiotic therapy, resulting in transient clinical improvement.

Quantum dots in periodontology: emerging promise and translational challenges.

Periodontitis is a chronic inflammatory disease driven by microbial dysbiosis, resulting in irreversible destruction of the periodontal ligament and alveolar bone. Conventional therapies, including mechanical debridement and local drug delivery, frequently fail to achieve adequate outcomes in advanced disease due to poor biofilm penetration, limited site-specificity, and the inability to modulate the host immune microenvironment. Quantum Dots (QDs) are semiconductor nanocrystals measuring 1-10 nm, which possess unique size-dependent photoluminescence, high photostability, broad excitation prof

Emerging insights into the physical health burden of personality disorders.

PURPOSE OF REVIEW: This review summarizes recent evidence on the physical health burden associated with personality disorders (PDs), focusing on physical comorbidity and multimorbidity, physical symptoms, and premature mortality. RECENT FINDINGS: Accumulating evidence indicates that individuals with PDs experience markedly elevated rates of chronic physical illness, multimorbidity, and premature mortality. Large-scale cohort studies demonstrate increased risks of cardiovascular disease, myocardial infarction, stroke, and all-cause mortality emerging early in life. Recent investigations furthe

How Hazards Turn Into Disasters: Perspectives of Emergency Responders.

Natural hazards like floods, storms, or earthquakes turn into disasters if they hit vulnerable communities and societies. In policy and academia, this understanding has led to a surge of models and risk reduction policies that aim to reduce vulnerability and strengthen resilience. However, it remains unclear which vulnerabilities are the most important, and what stakeholders in different contexts prioritize. To address this gap, this article identifies critical exposure, vulnerability, and coping capacity factors, elicits their priority among emergency responders from different contexts, and a

Characteristics of Pediatric Visits to the Emergency Department With Dermatological Complaints.

OBJECTIVES: Dermatological diseases constitute 4% to 12% of all emergency department (ED) visits and may contribute to high health care costs. However, little is known about pediatric visits. The aim of this study was to determine characteristics, photo app utilization, and diagnostic concordance at follow-up of pediatric visits to the ED with dermatological complaints. METHODS: This was a retrospective study conducted in the ED at an academic medical center. Patients younger than or equal to 18 years of age who presented with a dermatologic concern between May 2022 and April 2023 were includ

MASLD: Established Knowledge and Emerging Directions - Guideline Updates.

Metabolic dysfunction-associated steatotic liver disease (MASLD) has become a major global health challenge, largely driven by the increasing prevalence of obesity and type 2 diabetes mellitus. The disease spectrum ranges from isolated steatosis to metabolic dysfunction-associated steatohepatitis (MASH) with progressive fibrosis. Advanced fibrosis substantially increases the risk of cirrhosis, hepatocellular carcinoma, and cardiovascular complications, thereby contributing significantly to morbidity and mortality.Although lifestyle modification remains the cornerstone of therapy, pharmacologic

Established and emerging roles of lysosomal dysfunction in cardiac aging.

Cardiac aging is a central biological process underlying most cardiovascular diseases. Lysosomes, once regarded as terminal degradative compartments, are now recognized as dynamic metabolic and signaling hubs whose dysfunction has profound consequences for the aging heart. Human lysosomal storage disorders provide compelling evidence that isolated lysosomal defects are sufficient to cause early cardiomyopathy, underscoring the myocardium's exceptional dependence on sustained lysosomal competence. In physiological aging, impaired autophagy is the most apparent manifestation of lysosomal decline

Oropouche virus infection: an emerging arboviral threat in Asia.

BACKGROUND: Oropouche virus (OROV), a neglected arbovirus of the Orthobunyavirus genus within the Peribunyaviridae family, has historically been confined to South and Central America. Recent epidemiological changes, including geographic expansion, increasing incidence, exported travel-associated cases, and reports of neurological and congenital complications, have raised concern regarding its potential for wider dissemination. Although no confirmed cases have been reported in Asia to date, future emergence remains biologically and ecologically plausible due to interconnected virological, ecolo

Respiratory mucosal vaccines for emerging viruses: promise and challenges.

Systemically administered vaccines were instrumental in reducing severe disease, hospitalizations, and deaths during the SARS-CoV-2 pandemic. However, they often failed to consistently elicit robust immunity at mucosal sites. This minireview examines a central role for respiratory mucosal immunity in protection against emerging viral pathogens and highlights key takeaways from the SARS-CoV-2 pandemic. Evidence from SARS-CoV-2 and influenza studies demonstrates that mucosal vaccination uniquely induces secretory IgA, as well as resident memory B and T cells within the upper and lower airways, t

Emerging insights into the presentation, pathophysiology, and management of eosinophilic esophagitis.

Eosinophilic esophagitis (EoE) is a chronic, immune-mediated disease of the esophagus with substantial clinical manifestations including dysphagia, odynophagia, food impaction and failure to thrive in children. New evidence is emerging regarding phenotypic and symptom variations in diverse EoE populations. EoE pathogenesis involves impaired epithelial barrier function, antigen-driven Type 2 inflammation, and eosinophilic infiltration, yet eosinophil and mast cell depletion does not improve patient symptoms. Recent research has unveiled new potential mechanisms of epithelial dysfunction, includ

Glaucoma and Autoimmunity: Immunopathogenic Mechanisms and Emerging Immunomodulatory Therapies.

Glaucoma is a chronic progressive optic neuropathy and one of the leading causes of irreversible blindness worldwide. Although elevated intraocular pressure remains the most important modifiable risk factor, increasing evidence suggests that immune dysregulation and autoimmune responses also contribute substantially to disease onset and progression. Clinical studies across different glaucoma subtypes have identified subtype-dependent immune abnormalities, including altered serum autoantibody profiles, dysregulated cytokine and chemokine expression, and changes in peripheral immune cell subsets

Emerging pharmacological targets in migraine therapy: CGRP and beyond.

Migraine is a prevalent, complex, and disabling neurological disorder. Available treatments consist of repurposed drugs, with medication switching and treatment failure a common occurrence. Pathophysiological models have shifted from migraine being a primarily vascular disorder to a complex network disease encompassing cortical, hypothalamic, thalamic, brainstem, trigeminovascular, and meningeal structures. These areas are interconnected by cascades of multiple signalling transmitters and hormones. This review explores how these molecules may offer novel pharmacological targets for efficacious

A rare and severe case of toxic megacolon: Radiological emergency.

Toxic megacolon is a rare and life-threatening complication of ulcerative colitis, representing a medical and surgical emergency with a poor prognosis. We report the case of a 31-year-old man with no significant past medical history, admitted for a 15-day history of bloody mucoid diarrhea associated with general health deterioration. Endoscopic and histopathological findings were consistent with a severe inaugural flare of ulcerative colitis. Despite optimal medical management, the clinical course was marked by the abrupt onset of septic shock. Abdominal imaging revealed acute colonic dilatati

Emergent multiscale organisation of neural dynamics fragments in anaesthesia.

Conscious experience depends on the coordinated activity of neural processes that span multiple scales: from synapses to whole-brain dynamics. A recently introduced measure, dynamical independence (DI), identifies, characterises, and quantifies these multi-scale relationships using an information-theoretic dimensionality reduction approach. Here, we use DI to examine changes in the emergent dynamical organisation in the human brain under three pharmacologically distinct anaesthetic interventions (propofol, xenon, ketamine). Applied to source-reconstructed electroencephalography (EEG), our anal

The Emerging Role and Clinical Utility of Transnasal Transesophageal Echocardiography.

Transesophageal echocardiography (TEE) is a cornerstone of cardiovascular diagnostics. A major advantage of TEE over transthoracic echo (TTE) is superior resolution of posterior structures such as the mitral valve and the left atrial appendage (LAA). TEE is invasive and requires the use of moderate and sometimes deeper levels of sedation. Anesthesia accounts for most TEE-related risks including hemodynamic (hypotension, arrythmia) and respiratory compromise. Risks of anesthesia are amplified by the high prevalence of co-morbidities in patients undergoing TEE. Anesthesia also requires specializ
