[Effect of Negative Pressure Wound Therapy on Infectious Complications in Grade III Open Fractures].

UNLABELLED: PURPOSE OF THE STUDY Grade III open fractures are associated with infectious complications in 25-66% of injuries. Negative pressure wound therapy (NPWT) applied to an injured soft tissue coverage provides an impermeable barrier between the injured structures and the external environment, in addition to early secretion draining and a positive effect on the site of application. All this also prevents secondary bacterial contamination. The objective of the study was to compare the results of treatment methods in view of infectious complications in patients with soft tissue injury in g

Surgery versus non-surgical splint treatment for proximal phalanx shaft finger fractures in adults: the POINT Randomised Controlled Trial.

BACKGROUND: Proximal phalanx finger shaft fractures are common and can impair hand function. There is controversy, but no high-quality evidence, on how they are best treated. We compared the clinical and cost-effectiveness of surgery versus non-surgical splint treatment. OBJECTIVE: The primary objective was to compare hand function following surgical fixation with hand function following non-surgical splint treatment using the Hand Health Profile of the Patient Evaluation Measure at 6 months post randomisation. DESIGN: Pragmatic multicentre, parallel superiority randomised (1 : 1) trial. SE

[Total elbow replacement in patients with rheumatoid arthritis].

PURPOSE OF THE STUDY: Total elbow arthroplasty is associated with a higher occurrence of complications than is usual for large-joint replacements. Two kinds of prostheses, unconstrained and semi-constrained, are currently used and each has its supporters or opponents. In this study the results of the two techniques used in our patients are evaluated and compared. MATERIAL: Two groups of elbows in patients with rheumatoid arthritis were evaluated. One comprised 58 elbows treated by Souter-Strathclyde total elbow arthroplasty (S-S group). The mean age of the patients at the time of surgery was

[Eleven-Year Experience with Total Ankle Arthroplasty].

UNLABELLED: PURPOSE OF THE STUDY Total joint replacement is one of the options in surgical treatment of advanced ankle arthritis. It allows the ankle to remain mobile but, unfortunately, it does not provide the same longevity as total knee or hip replacements. Therefore, decisions concerning the kind of treatment are very individual and depend on the clinical status and opinion of each patient. MATERIAL AND METHODS A total of 132 total ankle replacements were carried out in the period from 2004 to 2015. The prostheses used included the Ankle Evolutive System (AES) in 52 patients, Mobility Tota

Intraarticular Matrix Metalloproteinases and Aggrecan Degradation Are Elevated After Articular Fracture.

BACKGROUND: Posttraumatic osteoarthritis (OA) is a variant of OA that can develop after articular injury. Although the mechanism(s) of posttraumatic OA are uncertain, the presence and impact of postinjury proteolytic enzymes on articular cartilage remain unknown. To our knowledge, there are no studies that evaluate the presence of matrix metalloproteinases (MMPs) or aggrecan degradation after articular fracture. QUESTIONS/PURPOSES: (1) Are MMP concentrations and aggrecan degradation elevated after intraarticular fracture? (2) Are MMP concentrations and aggrecan degradation greater in high-ene

Outcomes of Surgical Treatment for Masada Type lVa Forearm Deformity in Children with Multiple Hereditary Exostosis: A Case Series.

INTRODUCTION: Multiple hereditary exostosis (MHE) is a rare autosomal dominant skeletal disorder widely known as multiple osteochondromatosis that frequently affects the growing ends of long bones. It causes multiple progressive deformities, especially in the wrist, elbow, knee and ankle. The spectrum of features in the forearm includes deformities due to growth arrest of the distal ulnar physis, an increase in the negative ulnar variance, excess radial bowing, an increased radial articular angle, with or without radial head dislocation and restricted pronosupination. Masada's classification a

The Role of Hip Arthrodiastasis in Management of Late-Onset Legg-Calvé-Perthes Disease: A Prospective Case Series.

BACKGROUND: Late-onset Legg-Calvé-Perthes disease (LCPD), defined as onset after the age of 8 years or younger, is associated with a poor prognosis and limited remodeling potential. The role of hip arthrodiastasis in this age group remains controversial. METHODS: This prospective case series enrolled patients consecutively and collected data at predefined follow-up intervals. It included 24 patients with late-onset LCPD treated with articulated hip arthrodiastasis using a hinged Ilizarov external fixator combined with percutaneous adductor tenotomy. This reflects a deliberate institutional sh

Total hip replacement after gunshot hip injuries: a severity-based clinical framework-a single-centre experience.

PURPOSE: Gunshot hip injuries present major reconstructive challenges due to contamination, retained projectiles, soft-tissue compromise, and variable bone loss. Total hip replacement (THR) in this setting carries increased risks of infection, instability, and technical failure. This study evaluated the clinical and radiological outcomes of THR following gunshot hip trauma using a structured, severity-based subclassification to inform reconstructive strategy. METHODS: Thirty patients with gunshot hip injuries underwent surgical debridement followed by THR, performed as single-stage or staged

Predictive value of a preoperative computed tomography-derived perifracture soft-tissue envelope index for fracture-related infection after open reduction and internal fixation of tibial plateau fractures.

BACKGROUND: Fracture-related infection (FRI) is a major complication after open reduction and internal fixation (ORIF) of tibial plateau fractures. This study evaluated whether a preoperative computed tomography (CT)-derived soft-tissue envelope index (STEI) was associated with 12-month FRI and whether it provided incremental value for preoperative risk stratification. METHODS: This retrospective single-center cohort study included 347 adults with unilateral closed tibial plateau fractures who underwent ORIF between January 2020 and December 2024. The STEI was defined as the mean ratio of sof

Correction method for moderate and severe degrees of hallux valgus associated with transfer metatarsalgia.

BACKGROUND: Hallux valgus (HV) is a common foot deformity that manifests with increasing age, especially in women. The associated foot pain causes impaired gait and decreases quality of life. Moderate and severe HV is a deformity that is characterized by the involvement of lesser rays and requires complex surgical treatment. In this study, we attempted to develop a procedure for this condition. AIM: To analyse the treatment results of patients who underwent simultaneous surgical correction of all parts of a static forefoot deformity. METHODS: We conducted a prospective clinical trial between

Severe Trauma Patients Requiring Undelayable Combined Cranial and Extra-Cranial Surgery: A Proof-of-Concept Monocentric Study.

INTRODUCTION: To date, there is no evidence concerning the emergency surgical management of severe trauma patients (STP) with severe traumatic brain injury (STBI) presenting a life-threatening intracranial hematoma and a concomitant extra-cranial noncompressible active bleeding. Current guidelines recommend stopping the extra-cranial bleeding first. Nevertheless, the long-term outcome of STP with STBI mainly depends from intracranial lesions. Thus, we propose a combined damage-control surgical strategy aiming to reduce the time spent with intracranial hypertension and to hasten the admission i

Distraction osteogenesis at the proximal third of the ulna for the treatment of Masada type I/IIb deformities in children with hereditary multiple exostoses: a retrospective review of twenty cases.

BACKGROUND: Ulna distraction by monolateral external fixator (MEFix) is a good option for the treatment of Masada type I and IIb deformities in children with hereditary multiple exostoses (HMEs). However, there is no consensus regarding where to perform ulnar osteotomy. Our hypothesis is that osteotomy at the proximal third of the ulna and progressive distraction with MEFix can simultaneously correct elbow and wrist deformities in patients with HME. METHODS: We retrospectively reviewed patients with HME who underwent ulna distraction osteogenesis from June 2014 to March 2019. The carrying ang
