Cavovarus foot realignment to treat anteromedial ankle arthrosis.

BACKGROUND: Adult patients with cavovarus feet were seen with symptomatic anteromedial ankle arthrosis and, frequently, lateral hindfoot instability. Static and dynamic realignment was performed to redistribute joint contact pressures and restore stability. METHODS: Thirteen patients with fixed cavovarus feet (6 neurogenic, 7 idiopathic; 6 with hindfoot instability, 7 without) and mild to moderate anteromedial ankle arthrosis were treated by osteotomies and tendon transfers but no lateral ligament reconstruction. Anteromedial cheilectomy of the ankle was added to increase dorsiflexion and all

Bilateral os subtibiale and talocalcaneal coalitions in a college soccer player: a case report.

An os subtibiale is an accessory bone separated from the distal medial tibia proper. Subtalar tarsal coalition is a failure of joint formation between the talus and calcaneus during hindfoot maturation. The patient in this case report has large bilateral os subtibiale and subtalar coalitions, which were undiagnosed throughout his soccer career until recently when he began having anteriorlateral ankle pain. After failing conservative treatment the patient underwent ankle arthroscopy, which revealed a fully separated, large articular portion of the medial malleolus. The hypertrophic synovium and

Acardiac twin fetus with severe hydrops fetalis and bilateral talipes varus deformity.

Twin reversed-arterial-perfusion syndrome (TRAPS) is a rare complication of monochorionic twin pregnancies. TRAPS is characterized by the hemodynamic dependence of a "recipient" twin from a "pump" twin. The "recipient" twin exhibits lethal abnormalities, such as acardia and acephaly. Circulatory failure of the normal twin derives from the existence of arterio-arterial and veno-venous anastomoses within the placenta that allow retrograde perfusion of the acardiac twin by blood coming from the normal twin. Acardiac twinning is the most extreme manifestation of the twin-twin transfusion syndrome.

A retrospective review of the weil metatarsal osteotomy for lesser metatarsal deformities: an intermediate follow-up analysis.

The purpose of this retrospective clinical study was to determine the clinical results of the Weil metatarsal osteotomy. There were 51 patients (89 procedures), consisting of 10 males and 41 females, with a mean 53.1 +/- 11.9 years. Patients were evaluated for subjective improvement on a scale from 0 to 10, and asked if they would repeat the procedure. They were evaluated functionally using the American Orthopedic Foot and Ankle Society's lesser metatarsal rating scale, and assessed for toe purchase and range of motion. Thirty-nine point two percent of patients reported a complete resolution o

Marfan syndrome: orthopedic and genetic review.

Marfan syndrome is an autosomal dominant disorder of connective tissue that affects the cardiac, eye, and skeletal systems. More than 135 mutations have been identified in the fibrillin-1 gene, localized on chromosome 15q21.1 [corrected] and responsible for the clinical manifestations of Marfan syndrome. The major orthopedic manifestations of Marfan syndrome include scoliosis, chest wall deformity, dural ectasia, joint hypermobility, and acetabular protrusion. In addition, decreased bone mineral density has been reported in patients with Marfan syndrome. This review summarizes recent developme

Twins and their mildly affected mother with Weaver syndrome.

A pair of twins, a brother and sister, with the complete form of Weaver syndrome (overgrowth, macrocephaly, facial, skeletal, nail and feet anomalies) and their mildly affected mother are reported, suggesting autosomal dominant inheritance. They all have plantar and palmar hyperhydrosis and twins also have nail dysplasia, symptoms which have not yet been described in this syndrome.

A previously undescribed autosomal recessive multiple congenital anomalies/mental retardation (MCA/MR) syndrome with fronto-nasal dysostosis, cleft lip/palate, limb hypoplasia, and postaxial poly-syndactyly: acro-fronto-facio-nasal dysostosis syndrome.

We describe two sibs born to a consanguineous couple. Among other clinical findings both have mental retardation, short stature, facial and skeletal abnormalities characterized by hypertelorism, broad notched nasal tip, cleft lip/palate, campto-brachy-poly-syndactyly, fibular hypoplasia, and marked anomalies of foot structures. Facial signs of the reported patients resemble those present in the fronto-nasal "dysplasia" syndrome; however, the whole clinical picture in the present patients suggests a true MCA/MR syndrome, most likely inherited as an autosomal recessive trait. Clinical and geneti

[The fetal hydantoin syndrome (author's transl)].

Two siblings have various congenital malformations due to the teratogenic effect of anticonvulsant drugs. Their mother has a 14-year history of continuous anticonvulsant medication. The malformations noted in the older girl are less and in the 8 years younger sister more extensively developed as well as in the fetal hydantoin syndrome. The causes of different severity of the fetal hydantoin syndrome in two sisters are discussed.

Intraoperative neuromonitoring: lessons learned from 32 case events in 2095 spine cases.

STUDY TYPE: Restrospective chart review Introduction:  Intraoperative neuromonitoring is becoming the standard of care for many more spinal surgeries, especially with deformity correction and instrumentation. We reviewed our institution's neuromonitored spine cases over the past 4 years to see the immediate intraoperative and postoperative clinical findings when an intraoperative neuromonitoring event was noted. OBJECTIVE: The main question addressed in this review is how multimodality intraoperative neuromonitoring has affected our ability to avoid potential neurological injury during spine

Long-term Outcomes of Talonavicular Arthrodesis for the Treatment of Planovalgus Foot in Children With Cerebral Palsy.

BACKGROUND: Planovalgus foot (PVF) in cerebral palsy (CP) tends toward progression and rigidity in adolescence, especially in patients with greater functional impairment. Deformity at the talonavicular joint justifies the use of talonavicular arthrodesis as a corrective surgical technique. This study aims to assess patient or caregiver functional satisfaction and radiographic outcomes of talonavicular arthrodesis for PVF in CP patients with assisted ambulation in the long-term. METHODS: Retrospective comparative study of level III and level IV pediatric CP patients who underwent talonavicular

Proximal phalanx and flexor digitorum longus tendon biomechanics in flexor to extensor tendon transfer.

BACKGROUND: The flexor to extensor transfer of the flexor digitorum longus (FDL) tendon has been a relatively common operative procedure for the treatment of a flexible hammer toe deformity and chronic metatarsophalangeal (MTP) joint dislocation. A possible complication of using the tunnel technique rather than the tendon splitting technique is iatrogenic fracture through the drilled tunnel site. The purpose of this investigation was to study the FDL tendon and proximal phalanx dimensions in the area of the transfer procedure in order to improve preoperative planning and minimize postoperative

Evaluation of calcaneal inclusion angle in the diagnosis of pes planus with pretrained deep learning networks: An observational study.

Pes planus is a common postural deformity related to the medial longitudinal arch of the foot. Radiographic examinations are important for reproducibility and objectivity; the most commonly used methods are the calcaneal inclusion angle and Mery angle. However, there may be variations in radiographic measurements due to human error and inexperience. In this study, a deep learning (DL)-based solution is proposed to solve this problem. Lateral radiographs of the right and left foot of 289 patients were taken and saved. The study population is a homogeneous group in terms of age and gender, and d
