Hip fractures in the geriatric population are associated with high morbidity and mortality rate. Early surgical fixation is of major importance, as it is one of the factors that contribute to patient optimization. However, these patients usually present a high burden of comorbidities, including aortic stenosis that could affect their treatment. Despite major advances in anesthetic and surgical techniques, severe aortic stenosis remains an independent, important risk factor for patients undergoing anesthesia for noncardiac surgery. In these patients, general and/or neuraxial anesthesia should b

Equine odontoclastic tooth resorption and hypercementosis (EOTRH) is a recently described painful and progressive condition of unknown etiology that occurs in middle-aged and geriatric equines. It predominantly affects the permanent incisor and canine teeth and, less commonly, the premolar and molar dentition. EOTRH was first reported in peer-reviewed literature in 2008, with subsequent publications of case reports, histological studies and retrospective case series. There have been few significant research developments related to this disease. The existing studies have primarily involved sing

A 62-year-old lady is referred to a geriatric dentist because of sudden increased caries activity. She is suffering from Parkinson's disease. Saliva loss, or drooling, had such a negative impact on her quality of life that, she had been treated with Botox injections into her glandula parotis on both sides on referral from the neurologist until about a year ago. Hhyposalivation as a side effect of her medication in combination with the Botox injections likely caused increased caries activity. The risks of therapies to reduce salivary flow should be made known to doctors and dentists. Saliva test

The defining feature of a surgeon's professional practice is clinical decision making coupled with surgical execution. Surgeons' decisions are continually being scrutinized, and clinicians are trained to consult and interpret the peer-reviewed literature in formulating a treatment plan. An important decision in the surgical episode of care is which implant(s) will be used, and surgeons should feel comfortable defending their implant choices. Understanding the finer details of a bench study is important in formulating a clinical opinion of a biomechanical investigation of surgical implants. The

The demographic situation in recent decades has been characterized by a progressive increase in the proportion of older age groups in both developed countries and the Russian Federation. The main contribution to morbidity and mortality of people over 60 years of age is made by diseases of the circulatory system. Early development and progression of cardiovascular diseases contributes to kidney dysfunction, often developing against the background of morphological changes occurring with age in the kidneys, called "senile kidney". Reduction of glomerular filtration rate in elderly patients is not

As a result of the aging population dementia is a growing challenge, especially in healthcare. Nevertheless, cognitive disorders are often not systematically evaluated, especially during hospital stays for other reasons; however, cognitive impairment is associated with a number of geriatric syndromes, including falls, delirium, dysphagia and lack of adherence to treatment plans. This article considers the current state of diagnosis and treatment of dementia. Non-pharmacological therapeutic approaches as well as current and future pharmacological treatment options are discussed. The drugs of ch

The majority of blood cancers occur in the elderly. This fact conspires with an aging population in many countries to make rigorous assessment for frailty increasingly important for hematologic oncologists. In this review, we first define frailty and its relevance for patients with hematologic malignancy. Next, we review current data regarding the effect of domains of frailty on outcomes for blood cancers including myelodysplastic syndromes, acute leukemia, non-Hodgkin lymphomas such as chronic lymphocytic leukemia, and multiple myeloma. Finally, after presenting assessment and treatment optio

Follicular lymphoma is essentially a disease of the elderly, and the aging of the population in developed countries will increase patient numbers in coming years. Significant achievements have been made for treatment, but better understanding of the disease and major progress in biology now facilitate the development of many new drugs, which may have improved toxicity profiles making them appropriate for treatment of older adults. However, the increasing number of treatment possibilities, can also increase the toxicity risks, and unexpected toxicities specific to older adults may be encountere

Cigarette smoking is the foremost health risk issue affecting individuals of all age groups globally. It specifically influences the geriatric population as a result of chronic exposure to toxins. Its role in various systemic and oral diseases including cancer, premalignant lesions, periodontitis, tooth loss, dental caries and implant failures is well established. Smoking causes immuno-inflammatory imbalances resulting in increased oxidative stress in the body. The latter hastens the immunosenescence and inflammaging process, which increases the susceptibility to infections. Thus, implementati

Percutaneous coronary intervention (PCI) in an anomalous right coronary artery (RCA) can be technically difficult because selective cannulation of the vessel may not be easy. We thereby present two cases with unstable angina pectoris of anomalous originated RCA. The PCI were successfully performed in two patients with a special guiding wire manipulating skill which we called "gone with the flow" combined with balloon anchoring technology, providing excellent angiographic visualization and sound guide support for stent delivery throughout the procedure without severe cardiovascular adverse effe

Epilepsy, being one of the most common neurological disorders, has raised concerns in developed and developing countries. Due to its heterogeneous pathophysiology and different cellular, molecular and genetic mechanisms, it is becoming a threat worldwide. Despite wide availability of antiepileptic drugs (AEDs), epilepsy still remains a major cause of morbidity and mortality among the pediatric and geriatric populations. Commonly available AED therapies include sodium and calcium channel blockers, GABA agonists, and potassium channel activists. Along with beneficial effects, AEDs have been know

Among geriatric syndromes, fall and fall-related fractures are one of the leading causes of the elderly's need for long-term care. Hip fractures are the typical cases. The underlying diseases of locomotive syndrome, such as sarcopenia, musculoskeletal ambulation disorder symptom complex, and osteoporosis, are closely associated with fall and fall-related fractures. From the stand point of frailty, sarcopenia, which is considered the major cause of aging-associated declines in function and reserve across multiple physiologic systems, plays a role in fall and fall-related fractures. As the elderly population in our society significantly increases, the incidence of displaced femoral neck fractures will increase proportionally. Three surgical procedures are available to treat such fractures: internal fixation, hemiarthroplasty (unipolar or bipolar), and total hip arthroplasty. Long-term costs and efficacy of these three procedures vary, primarily due to postoperative complications. Thus, it is imperative that all surgeons conduct a proper preoperative evaluation of each patient before choosing the optimal treatment plan. Internal fixation has been shown to be more beneficia

The effect of delay of surgery on the geriatric hip fractures has been a subject of interest in the past two decades. While the elderly patients will not tolerate long periods of immobilization, it is still unclear how soon these surgeries need to be performed. A review of existing literature was performed to examine the effect of timing of surgery on the different outcome parameters of these patients. Although there is conflicting evidence that early surgery would improve mortality, there is widespread evidence in the literature that other outcomes including morbidity, the incidence of pressu

Screening for impaired cognition is very important in geriatric and palliative medicine. There are several validated screening tools for delirium; the Confusion Assessment Method, The Memorial Delirium Assessment Scale, and the Bedside Confusion Scale. The Clock Drawing Test, which was initially described in 1963, has also been used to assess cognition and is validated in Alzheimer's disease and in elderly hospice patients with a negative predictive value greater than 0.95. The Clock Drawing Test is quickly and easily performed by patients and can be kept in the patient's medical record for fu

When you take care of elderly, you need to evaluate cognitive functions due to the frequency of dementia. However, before this evaluation, you must be aware of difficulties that you will encounter. You have to determine general characteristics of elderly patients and use geriatric reasoning in order to list pathologies that can interfere with cognitive functions. Thereafter, you need to precise frontiers between normal cognitive functions and dementia. When dementia is confirmed, CT scan or MRI appears very useful to exclude a cause of reversible dementia or to precise the nature of dementia.

OBJECT: The authors assessed the safety of balloon kyphoplasty in the reduction and repair of osteopenic vertebral compression fractures and report functional outcomes (back pain and activity levels) in the first 96 patients (with 133 fractures) at their institution. Additionally they provide radiographic outcomes in the first 26 patients (41 fractures) treated and followed for 1 year.
METHODS: The authors conducted a retrospective chart review of functional outcomes and evaluated radiographs obtained at 1 week, 1 month, 3 months, 6 months, and 1 year postoperatively.
CONCLUSIONS: Balloon ky

We hypothesized that an intensive personal educational program would reduce the number of hospital admissions due to acute exacerbations of chronic airflow obstruction in elderly patients. The educational program was designed to improve self-management of asthma in these patients. We compared the number of patients admitted for treatment of acute asthma attacks at Tokyo Metropolitan Geriatric Hospital from April 1987 through March 1989 (before the program began) with the number admitted from April 1991 through March 1993 (after the program began). Fewer patients were admitted in the latter per

Ninety-seven nursing home residents were divided into 2 groups according to whether they had had an episode of stroke or not. Each group was classified into 3 subgroups: prolonged bed rest, walking with aids or walking by themselves. They were investigated for both the carotid blood flow volume and the modified Stockton geriatric rating scale (SGRS). In chronic stroke residents, the total blood flow volume of prolonged bed rest residents was lower than that who can walk by themselves. In residents without stroke, the total blood flow volume of prolonged bed rest residents was lower than the ot
