Research priorities for the primordial prevention of acute rheumatic fever and rheumatic heart disease by modifying the social determinants of health.

The social determinants of health (SDH), such as access to income, education, housing and healthcare, strongly shape the occurrence of acute rheumatic fever (ARF) and rheumatic heart disease (RHD) at the household, community and national levels. The SDH are systemic factors that privilege some more than others and result in poverty and inequitable access to resources to support health and well-being. Primordial prevention is the modification of SDH to improve health and reduce the risk of disease acquisition and the subsequent progression to RHD. Modifying these determinants using primordial p

Prospective Surveillance of Primary Healthcare Presentations for Scabies and Bacterial Skin Infections in Fiji, 2018-2019.

Scabies, impetigo, and other skin and soft tissue infections (SSTIs) are highly prevalent in many tropical, low-middle income settings, but information regarding their burden of disease is scarce. We conducted surveillance of presentations of scabies and SSTIs, including impetigo, abscesses, cellulitis, and severe SSTI, to primary health facilities in Fiji. We established a monthly reporting system over the course of 50 weeks (July 2018-June 2019) for scabies and SSTIs at all 42 public primary health facilities in the Northern Division of Fiji (population, ≈131,914). For each case, information

Internet-based guided self-help for glioma patients with depressive symptoms: design of a randomized controlled trial.

BackgroundAmong glioma patients, depression is estimated to be more prevalent than in both the general population and the cancer patient population. This can have negative consequences for both patients and their primary informal caregivers (e.g., a spouse, family member or close friend). At present, there is no evidence from randomized controlled trials for the effectiveness of psychological treatment for depression in glioma patients. Furthermore, the possibility of delivering mental health care through the internet has not yet been explored in this population. Therefore, a randomized contro

Prevalence of stroke/transient ischemic attack among patients with acute coronary syndromes in a real-world setting.

BACKGROUND: Atherothrombosis is a systemic disease that may manifest as acute ischemic events in multiple vascular beds. Patients who have experienced an atherothrombosis-related ischemic event in 1 vascular bed are at risk for developing ischemic events in other vascular beds. Antiplatelet therapy demands an understanding of the balance between arterial thrombosis benefit and adverse event risk. Clinical trials indicate that dual antiplatelet therapy with aspirin and the newer thienopyridines increases the risk of bleeding in patients with acute coronary syndromes (ACS) with prior cerebrovasc

Overview of the Anatomy, Physiology, and Pharmacology of the Autonomic Nervous System.

Comprised of the sympathetic nervous system, parasympathetic nervous system, and enteric nervous system, the autonomic nervous system (ANS) provides the neural control of all parts of the body except for skeletal muscles. The ANS has the major responsibility to ensure that the physiological integrity of cells, tissues, and organs throughout the entire body is maintained (homeostasis) in the face of perturbations exerted by both the external and internal environments. Many commonly prescribed drugs, over-the-counter drugs, toxins, and toxicants function by altering transmission within the ANS.

Psychiatric hospitalizations in the Unified Health System: an observational study on hospitalization rates from 2012 to 2023.

BACKGROUND: Psychiatric care in Brazil is based on the National Mental Health Policy and is aligned with the guidelines of the Brazilian Unified Health System. It is based on the preeminence of care in the extra-hospital context over the hospital context. Hospital admissions should occur solely when extra-hospital resources prove insufficient for the proper management of the mental health condition. METHOD: It refers to a time series investigation of a descriptive, ecological, and observational nature. We used publicly available hospital admissions data from the Brazilian Unified Health Syste

The effects of exposure to solar radiation on human health.

This assessment by the Environmental Effects Assessment Panel (EEAP) of the Montreal Protocol under the United Nations Environment Programme (UNEP) evaluates the effects of ultraviolet (UV) radiation on human health within the context of the Montreal Protocol and its Amendments. We assess work published since our last comprehensive assessment in 2018. Over the last four years gains have been made in knowledge of the links between sun exposure and health outcomes, mechanisms, and estimates of disease burden, including economic impacts. Of particular note, there is new information about the way

Regional differences in determining cardiovascular diseases as the cause of death in Poland: time for change.

BACKGROUND: Data regarding deaths in many countries is a reliable source of information on population health status, due to the legal obligation to register the fact of a death and its cause. Such data is widely used to analyse regional health differences, changes in health over time, and to pursue and monitor the effects of health policies. Therefore, it is extremely important that the data is reliable and comparable across the country. AIM: To analyse death rates from cardiovascular diseases in 2007-2009 among residents of large Polish cities, where medical universities are located, in orde

The Emotional and Mental Wellbeing of Women Who Use Drugs Engaged in Community-Based Overdose Response.

BACKGROUND: Fatal overdose among women in the United States has risen sharply in recent years. Women who use drugs often respond to overdoses within their communities, including administering naloxone and providing peer-based care in contexts shaped by structural vulnerability, stigma, and criminalization. However, the emotional and mental health dimensions of these experiences remain underexplored. METHODS: This qualitative study draws on in-depth, semi-structured interviews with 27 women in Miami-Dade County, Florida (June 2023 to January 2024). Participants were recruited from the communit

Separation and Identification of Native Proteoglycans by Composite Agarose-Polyacrylamide Gel Electrophoresis and Immunoblotting.

Composite agarose-polyacrylamide gel electrophoresis (CAPAGE) in gels of 1.2% w/v polyacrylamide and 0.6% w/v agarose can be used to examine the heterogeneity of full-length native proteoglycan populations and their fragments in crude tissue extracts, and when used in conjunction with immunoblotting and specific antibodies to proteoglycan core protein and glycosaminoglycan, side chain epitopes can provide significant information on the level of proteoglycan polydispersity/heterogeneity and a number of proteoglycan populations present in tissue samples. This can be a technically difficult techn

Health workers' disrespectful and abusive behaviour towards women during labour and delivery: A qualitative study in Durban, South Africa.

BACKGROUND: A high prevalence of disrespectful and abusive behaviour by health workers towards women during labour and delivery has been widely described in health facilities, particularly in Africa, and is a worldwide public health concern. Such behaviours are barriers to care-seeking, and are associated with adverse outcomes for mothers and newborns. This paper reports experiences of disrespectful care among informal working women in three public health facilities in Durban, South Africa. METHODS: A qualitative longitudinal study was conducted among a cohort of informal working women recrui

"If I have money, I cannot allow my baby to breastfeed only …" barriers and facilitators to scale-up of peer counselling for exclusive breastfeeding in Uganda.

BACKGROUND: Early initiation and exclusive breastfeeding for 6 months reduces infant morbidity and mortality and can positively impact on cognitive function. In Uganda, exclusive breastfeeding for 6 months is recommended but many women introduce alternative feeds early. Interventions to scale-up peer support provision for exclusive breastfeeding are limited. We explored the barriers, facilitators and solutions to scaling-up of peer counselling support for exclusive breastfeeding in Uganda. METHODS: A qualitative study was conducted in Mbale District and Kampala City between April and July 201

Cost of health systems strengthening for small and sick newborn care in four sub-Saharan African countries implemented with NEST360: incremental cost analyses.

BACKGROUND: Newborn survival requires high-quality small and sick newborn care (SSNC). Domestic and donor financing for SSNC is low, yet real-world cost data to inform investment are scarce. We analysed primary financial data for SSNC health system improvements nationally in Malawi and subnationally in Kenya, Nigeria, and Tanzania. METHODS: We analysed incremental health system perspective costs using hospital-specific assessments, inpatient data (n=93 942 admissions per year), and government and NEST360 costs. We compared annual hospital-specific costs post-implementation (June, 2022, to Jun

A Patient-Centered Website (Within Reach) to Foster Informed Decision-making About Upper Extremity Vascularized Composite Allotransplantation: Development and Usability Study.

BACKGROUND: Upper extremity (UE) vascularized composite allotransplantation (VCA; hand transplantation) is a reconstructive treatment option for patients with UE loss. Approximately 37 UE VCAs have been performed in the United States to date; thus, little is known about long-term psychosocial outcomes and whether the benefits outweigh the risks. To make an informed treatment decision, patients must understand the procedure, risks, and potential benefits of UE VCA. However, few educational resources are publicly available providing unbiased, comprehensive information about UE VCA. OBJECTIVE: T

SWOT analysis of Mpox surveillance in high-burden districts in Uganda, February-March 2025.

BACKGROUND: From July 2024–February 2025, three-quarters of the districts in Uganda experienced mpox outbreaks, with sustained transmission in urban cities and towns. Following the rise in cases and severe outcomes, there was an urgent need to assess sub-national surveillance capacities to inform improvements in mpox response. This study aimed to assess the strengths, weaknesses, opportunities, and threats to mpox surveillance in Uganda’s high-burden districts and document lessons learned for responding to future outbreaks. METHODS: We conducted a cross-sectional qualitative study during Febru
