INTRODUCTION: Patellofemoral instability (PFI) is a common knee injury in the pediatric population, and medial patellofemoral ligament (MPFL) reconstruction is common surgery to minimize risk of chronic PFI. Medial patellotibial ligament (MPTL) reconstruction may be subsequently performed, providing more lateral stability to reduce rotational instability and tilt. Recent literature has described MPTL reconstruction in adults and nonpathologic pediatric patients; however, little literature has discussed this procedure in pediatric patients with underlying abnormal patellofemoral morphology.

BACKGROUND: Whether early surgery reduces mortality in revision total hip arthroplasty (THA) for periprosthetic proximal femur fractures (PPFF) remains unclear. The aim of this study was to evaluate the association between time to surgery and short- and long-term outcomes following PPFF.
METHODS: Using a large national database, adult patients undergoing revision THA for PPFF were identified. Cohorts were defined by time from diagnosis to surgery: two days versus greater than two days. Propensity score matching (1:1) was performed on demographics and comorbidities, yielding 2,098 patients per

In this work, aptamer AD1-modified poly(lactic-co-glycolic acid)-poly(ethylene glycol)-fluconazole nanoparticles (AD1-PLGA-PEG-FLUCZ) were prepared, enabling the antifungal drug FLUCZ to specifically target the biofilm of Candida albicans, prolonging its action time on the biofilm, and enhancing its killing effect. The analytical results demonstrated that the AD1-PLGA-PEG-FLUCZ nanoparticulate system achieved a FLUCZ loading capacity of 5.89 ± 0.07% with an encapsulation efficiency of 84.51 ± 0.18%. Physicochemical characterization revealed a uniform particle size distribution (390 nm) and sta

INTRODUCTION: To improve surgical quality and safety, health systems must prioritise equitable care for surgical patients. Racialised patients experience worse postoperative outcomes when compared with non-racialised surgical patients in settler colonial nation-states. Identifying preventable adverse outcomes for equity-deserving patient populations is an important starting point to begin to address these gaps in care. To derive literature-based estimates of the outcome gap for racialised surgical patients, we will systematically review and meta-analyse rates of adverse postoperative events as

AIMS: In an increasingly frail population, simultaneous "fix and replace" surgery (fixation of the acetabulum to accommodate a press fit cup and total hip arthroplasty (THA)) is a novel alternative to open reduction and internal fixation (ORIF) alone in the management of acetabular fractures. We aimed to determine whether patients managed with "fix and replace" have comparable survivorship to those undergoing staged THA following previous open reduction and internal fixation for acetabular fracture.
METHODS: All Patients with acetabular fractures surgically managed within our Tertiary centre

BACKGROUND: Intramedullary nail breakage is an uncommon complication in patients with femoral intertrochanteric fractures treated with proximal femoral nail antirotation. Salvage surgery for nail breakages associated with delayed union or nonunion is challenging, particularly when breakage occurs within an acceptable reduction range, complicating implant selection. This study evaluated outcomes in patients with proximal femoral nail antirotation breakage, acceptable reduction, and fixation treated with the long proximal femoral nail antirotation change with distal screw dynamization and latera

BACKGROUND: The medial pivot total knee arthroplasty (MP-TKA) was developed to better replicate the kinematics of the healthy knee. Although outcomes of MP-TKA have been investigated, previous studies involved primarily varus preoperative deformities. The objective of this study was to determine patient outcomes using the Knee Injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS JR) following MP-TKA in relation to the type (varus/neutral/valgus) and severity (moderate/severe) of preoperative coronal deformity.
METHODS: This was a single-center retrospective cohort study of 311

INTRODUCTION: Patient-specific instruments (PSIs) have been introduced to enhance the accuracy of medial opening wedge high tibial osteotomy (MOWHTO). This study aimed to evaluate the accuracy of a newly developed PSI and its impact on postoperative clinical outcomes.
METHODS: Forty patients with varus alignment who underwent MOWHTO using the newly developed PSI were retrospectively analyzed for accuracy of correction. Radiographic evaluations, including hip-knee-ankle angle (HKA) and medial proximal tibial angle (MPTA) were performed using long-leg standing radiographs preoperatively and at

Introduction A warm-up is often performed to prevent injury and prepare for optimal performance. Nonetheless, research on its impact on performance, particularly in para-sports, remains limited. We hypothesized that the use of an assistive device during warm-up would enable wheelchair basketball players to perform full-body movements efficiently and effectively, contributing to enhanced wheelchair mobility. Therefore, this feasibility study aimed to assess the safety of warm-up exercise with an assistive device and the changes in wheelchair mobility performance before and after warm-up in able

The validity of the Agency for Healthcare Research and Quality's Patient Safety Indicators (PSIs) has been established in the USA and Canada. However, these indicators are also used for hospital benchmarking and cross-country comparisons in other nations with different health-care settings and coding systems as well as missing present on admission (POA) flags in the administrative data. This study sought to comprehensively assess and compare the validity of 16 PSIs in Switzerland, where they have not been previously applied. We performed a medical record review using administrative and electro

AIMS: Low-energy distal radius fractures (DRFs) are the most common upper arm fractures correlated with bone fragility. Vitamin D deficiency is an important risk factor associated with DRFs. However, the relationship between DRF severity and vitamin D deficiency is not elucidated. Therefore, this study aimed to identify the correlation between DRF severity and serum 25-hydroxyvitamin-D level, which is an indicator of vitamin D deficiency.
METHODS: This multicentre retrospective observational study enrolled 122 female patients aged over 45 years with DRFs with extension deformity. DRF severity

BACKGROUND: Surgical treatment of complex fractures of the distal femur in the elderly is controversial. Osteoporosis and pre-existent osteoarthritis are common comorbidities in the elderly which add to the need for early walking and rapid restoration of function and also pose significant obstacles to achievement of satisfactory results with standard fixation techniques. Recently, several authors have suggested that primary arthroplasty could be a viable alternative option to standard fixation techniques in selected patients with complex distal femur fractures.
AIM: To present our experience

INTRODUCTION: The frequency of intraoperative periprosthetic occult fracture of the acetabulum (IPOA) in primary total hip arthroplasty (THA) is relatively high. However, the effect of this fracture on clinical outcome and survival of primary THA remains unknown. This study aimed to determine the impact of IPOA on the outcomes of primary THA during a minimum follow-up of 5 years.
MATERIALS AND METHODS: This retrospective case-control study compared the outcomes of primary THA with or without IPOA during a minimum 5-year follow-up. Thirty-eight patients who underwent primary THA with IPOA (O g

Tranexamic acid, a synthetic derivative of the amino acid lysine, is an antifibrinolytic agent that acts by binding to plasminogen and blocking the interaction of plasmin(ogen) with fibrin, thereby preventing dissolution of the fibrin clot. Tranexamic acid (Transamin®) is indicated in Japan for use in certain conditions with abnormal bleeding or bleeding tendencies in which local or systemic hyperfibrinolysis is considered to be involved. This article reviews the efficacy and tolerability of tranexamic acid in conditions amenable to antifibrinolytic therapy and briefly overviews the pharmacolo
