Factors associated with extravasation severity in Thai patients receiving noncytotoxic intravenous infusions: An observational study.

BACKGROUND: Extravasation is a serious complication of peripheral intravenous (PIV) therapy. It has been linked to mechanical disruption of a PIV line due to inappropriate site selection or securement, and to the pH, concentration, and cytotoxic properties of the infused solution or drug. However, evidence on extravasation in patients receiving noncytotoxic drugs remains limited. Understanding the causes and risk factors in this population is essential for prevention, early detection, and the development of interventions. OBJECTIVE: To examine demographic and clinical factors associated with

Posaconazole enteral tube administration in adults: pharmacokinetic target attainment of oral suspension compared with crushed delayed-release tablet.

BACKGROUND: Posaconazole immediate release (IR) suspension is the standard product for enteral feeding tube (EFT) administration but has erratic pharmacokinetic behaviour, often resulting in subtherapeutic concentrations. Limited evidence suggests that posaconazole oral delayed-release (DR) tablets may be crushed. METHODS: This was a retrospective cohort study of hospitalized adults who received EFT administration of posaconazole IR suspension or crushed DR tablets between 2018 and 2024. Patients were eligible if they had at least one serum drug concentration drawn ≥5 days after posaconazole

Burning Questions: Oral Ulceration Secondary to Improper Bisphosphonate Administration in Advanced Dementia.

Background: Oral bisphosphonates are first-line osteoporosis therapy but can cause mucosal chemical injury when tablets are retained rather than swallowed correctly. This risk is heightened in cognitively impaired older adults with dysphagia or difficulty complying with medication administration guidelines. Case Presentation: A 73-year-old female nursing home resident with advanced vascular dementia presented with 24 h of oral pain, reduced intake, lethargy and new facial swelling. She had received monthly ibandronic acid 150 mg for two years, with documented difficulty swallowing tablets but

Efficacy of prophylactic pegfilgrastim compared with therapeutic filgrastim for preventing febrile neutropenia during chemotherapy for musculoskeletal tumors: A single-center retrospective study.

BACKGROUND: Febrile neutropenia (FN) is a serious adverse event of chemotherapy that can lead to treatment delays, dose reductions, and life-threatening infections. Prophylactic pegylated granulocyte colony-stimulating factor (peg G-CSF; pegfilgrastim) is recommended to reduce FN risk; however, evidence in patients with musculoskeletal tumors receiving diverse chemotherapy regimens remains limited. METHODS: We retrospectively reviewed 20 patients (104 chemotherapy administrations) with musculoskeletal tumors treated at a single institution between May 2014 and April 2017. Each patient had rec

Improving Antibiotic Administration Time in At-Risk Pediatric Populations: A Quality Improvement Project.

INTRODUCTION: Bacterial sepsis is a life-threatening concern for pediatric patients with a history of sickle cell disease, oncological disease, and/or neutropenia who present with fever. Prompt antibiotic administration is critical. However, studies show that many hospitals fail to meet the 60-minute benchmark for high-risk patients, despite its known benefits. METHODS: This quality improvement initiative aimed to ensure that at least 80% of the target population received antibiotics within 60 minutes of emergency department registration. The Plan-Do-Study-Act model was used, and data were co

A Markov Chain methodology for care pathway mapping using health insurance data, a study case on pediatric TBI.

BACKGROUND: Care pathways are increasingly used in healthcare systems globally to guide patient care and improve outcomes. These pathways offer a structured approach to managing patient care processes, potentially reducing costs and enhancing efficiency. However, the dynamic and complex nature of healthcare presents challenges in analyzing and improving these pathways, particularly due to the unique and varied patient journeys. This study focuses on the use of administrative data to map care pathways for pediatric Traumatic Brain Injury (TBI) patients, a population significantly impacted by hi

Assessment of appetite using health-care-professional-reported SNAQ.

BACKGROUND: Appetite loss is a major concern in various clinical contexts but the self-assessment of appetite might be compromised in some populations such as in individuals with dementia. The use of surrogate responders might be useful but no study so far has investigated whether hetero-administrated questionnaires might be valid tools to assess appetite. METHODS: Forty men and women aged 66-97 years, without dementia, were recruited from the Medical care and Rehabilitation Unit of the Toulouse University Hospital, France. Within the same day, patients' appetite was assessed using the SNAQ,

Anti-Fibrotic Effect of Tacrolimus on Preventing Adhesion after Achilles Tendon Surgery in a Murine Model.

BACKGROUND: Sickle cell disease (SCD) prevalence is predicted to rise dramatically in the upcoming years. Although several medications have received Food and Drug Administration (FDA) approval in recent years, low- and middle-income countries (LMICs) still struggle to access these medications due to their remarkably high prices. Crizanlizumab, owing to its clinical and economic privileges, appears to be the most suitable option for addition to pharmacotherapy guidelines. However, no study has yet investigated its cost-effectiveness in Iran's healthcare system. METHODS: This cost-effectiveness
