The case is presented of a twice-failed tubal ligation in a woman with six previous children. The first failure followed laparoscopic tubal occlusion by silastic falope rings, while the second failure was subsequent to tubal ligation and division via minilaparotomy. No similar reports have been reported in the East African Literature. Possible reasons for failure are discussed.

OBJECTIVE: To compare pregnancy rates after three surgical procedures for ectopic pregnancy (EP) over a 9-year period for normal and infertility patients.
DESIGN: In a retrospective analysis, we examined crude pregnancy rates and life-table analysis of cumulative pregnancy rates. A proportional hazard regression model was used to examine relative risk of type of surgery and fertility rates.
PATIENTS, PARTICIPANTS: One hundred twenty-six cases of EPs were reviewed at the Cleveland Clinic Foundation, a tertiary institution.

A prospective study of 17 cases of microsurgical reversal of tubal sterilization by a combined approach of laparoscopy and minilaparotomy was performed. Primary carcinoma of the duodenum is an uncommon tumor of the gastrointestinal tract. We reviewed the histories of ten patients seen between 1976 and 1986. Some of the patients with tumors in the second part of the duodenum presented with clinically evident jaundice. Reported here is a case of primary adenosquamous carcinoma of the common bile duct. This unusual variant of the ordinary adenocarcinoma has not, to the best of our knowledge, been previously reported as occurring in the extrahepatic bile ducts. A tumor of notorious prognostic significance, it represents an exceedingly rare and peculiar biological phenomenon of malignant metaplastic change.

Forty-seven patients with an abdominal aortic aneurysm (AAA) and recent onset of abdominal or back pain were evaluated by emergency computed tomography (CT) to identify those patients with a confined rupture, and unstable aneurysm, nonaneurysmal cause of pain and a stable AAA. CT suggested that 25 per cent of these had a confined rupture and should undergo emergency surgical treatment. Rupture was confirmed at operation in one-half of these instances. Records of twenty-six patients who presented at the University College Hospital, Ibadan with 'lost' IUDs between 1 July 1980 and 30 April 1982 were analysed. During the 22-month study period there were 3476 IUD insertions. Routine evaluation showed that eighteen IUDs (69.2%) were intrauterine whilst eight IUDs (30.8%) were extrauterine. Seventeen (65.4%) intrauterine devices were recovered by routine D and C. The treatment of biliary atresia by variations of the original Kasai hepatoportoenterostomy has shown early success with good bile flow and the elimination of jaundice in 50% to 70% of cases in many series. Long-term follow-up in many of these patients shows continued problems with ascending cholangitis and progressive liver disease leading to death. Three methods of tubal sterilisation, viz. laparoscopy, culdoscopy and minilaparotomy, were evaluated in a trial involving 300 voluntary acceptors at the University Hospital in Medan, Indonesia. Two cases of splenic complications occurring during chronic pancreatitis are reported. One patient with a splenic pseudocyst and the other with a ruptured spleen. From the two cases and from a review of the world literature, clinical symptoms, pathogenicity and diagnosis of this condition are described. Prophylactic splenectomy during surgery for chronic pancreatitis is suggested.

Between July, 1974 and February, 1979, 109 adolescent girls, ranging in age from 10 1/2 to 19 yr. with unexplained chronic pelvic pain, underwent diagnostic laparoscopy. Endometriosis was the most common finding occurring in 49 patients (45%), followed by postoperative adhesions in 17 patients (16%) and congenital abnormalities of the uterus in 10 patients (9%). Other important causes were chronic pelvic inflammatory disease with peritubal and periovarian adhesions in 9%, chronic hemoperitoneum in 5%, functional ovarian cysts in 5%, and uterine serositis in 2%. Fifty-eight children were studied for the presence of Meckel's diverticulum by scintigraphy with 99mTc-pertechnetate. Meckel's diverticulum was found at surgery in 6 of 6 patients with positive radionuclide studies and in 2 of 48 patients with negative studies. Four studies could not be interpreted because of widespread intestinal activity. Using the criteria described in this paper, false-positive readings did not occur.
