Fifteen Years of Non-Hodgkin Lymphoma in an Indonesian National Referral Hospital: Epidemiologic Trends and Diagnostic Challenges.

PURPOSE: The global burden of lymphoma is substantial because of the increase in its incidence in recent decades. However, disease characteristics vary across different geographical locations. Numerous immunohistochemistry markers and molecular studies are essential to determine lymphoma diagnosis and prognosis. This poses significant challenges in developing countries with limited health care resources. This large-scale study assesses the frequency of non-Hodgkin lymphoma (NHL) in Indonesia over the past 15 years, analyses its clinicopathologic features, and predicts future trends. METHODS:

An immunohistochemical study of double-expressor lymphomas and its correlation with cell of origin.

BACKGROUND: Diffuse large B cell Lymphomas (DLBCL) that co express C MYC and BCL 2 are known as 'double expressor lymphomas' and are believed to have a worse prognosis than other diffuse large B cell lymphomas. This was a study to assess the frequency of double expressor lymphomas in our cohort of DLBCL. AIMS AND OBJECTIVES: The aims of this study were to assess the frequency of double expression of C MYC and BCL 2 in cases of DLBCL and to correlate the same with clinicopathological parameters including cell of origin, namely germinal centre type versus non germinal centre type. Chemotherapy combinations for B-cell lymphoma and chemo-free approach in elderly patients: an update on best practice.

INTRODUCTION: Elderly patients represent a consistent portion of new diagnoses of B cell Non-Hodgkin Lymphoma (B-NHL). The treatment approach in this setting can be challenging for clinicians due to treatment toxicities and patients' comorbidities to deal with. Immunochemotherapy still represents the main option in the front-line setting for diffuse large B cell lymphoma (DLBCL), follicular lymphoma (FL), and mantle cell lymphoma (MCL), with different options to choose depending on patient characteristics. In the last decade, a number of new drugs and combinations have been investigated, demon

Secondary hematological malignancies after breast cancer chemotherapy.

According to several reports, the 10 year incidence of secondary acute myelogenous leukemia (AML) or myelodysplastic syndrome (MDS) after systemic chemotherapy is approximately 1.5%. The cumulative risk increases by 0.25--1% for the first 8 years after treatment. We have reported only 6 cases of hematological malignancies (0.3%) after breast cancer chemotherapy in our institute. We detected 2 cases of secondary AML and 1 case of MDS, 19, 52 and 12 months, respectively, after systemic chemotherapy for breast cancer. Published data on the occurrence of secondary hematological malignancies other

Acquired immune deficiency syndrome-related intravascular large B-cell lymphoma primarily arising from lymph nodes: a case report.

BACKGROUND: Intravascular large B-cell lymphoma (IVLBCL) is a rare subtype of diffuse large B-cell lymphoma, with nodal involvement being particularly uncommon. Due to its atypical clinical presentation, timely and accurate diagnosis is often challenging. Positron emission tomography-computed tomography (PET-CT) and pathological biopsy can assist in the diagnostic process. CASE DESCRIPTION: A 52-year-old woman with acquired immune deficiency syndrome (AIDS) and a one-year history of chronic hepatitis B was admitted to the Infection Department of Chongqing Public Medical Center in September 20

The present status of therapy for patients with aggressive non-Hodgkin's lymphoma.

The aggressive non-Hodgkin's lymphomas include some of the malignancies most frequently cured with chemotherapy. However, not all patients are cured, and the best treatment approach remains uncertain. The most common aggressive non-Hodgkin's lymphomas are diffuse large-cell lymphoma and immunoblastic lymphoma. Most recent studies suggest no useful difference between these two groups. When these lymphomas are localized at presentation they are highly curable. Earlier studies showed that radiotherapy alone had a high relapse rate. Chemotherapy alone has been found to have an excellent cure rate,

[Problems in biopsy differential diagnosis in lymphomas of the small and large intestines].

Besides the problems inherent in endoscopically obtained tissue and the low incidence of intestinal lymphomas, the major difficulties reside in the distinction to reactive processes and in the differential diagnosis among several lymphoma entities. Knowledge of the microanatomical and biological properties of the intestinal MALT, supplemented by sufficient clinical information, are important prerequisites for the diagnostic work-up which has to include immunohistochemical studies. Whereas the diagnosis of aggressive B-cell lymphomas (diffuse large B-cell lymphoma, Burkitt's lymphoma) is usuall

Loss of Fas (CD95/APO-1) regulatory function is an important step in early MALT-type lymphoma development.

Fas (CD95, APO-1) mutations were found in autoimmune diseases and some lymphomas, suggesting impairment of Fas-mediated cell death signaling that may cause tumor development. Because mucosa-associated lymphoid tissue (MALT)-type lymphoma B cells recognize autoantigens and proliferate in response to antigen and T cell-mediated signals, it is suggestive that autoreactive B cell lymphoma precursor cells may have escaped the Fas-mediated checkpoint that normally operates in healthy individuals. Using different biochemical, molecular, and functional approaches, we analyzed the Fas signaling in mali

Targeting RAB7 in human B lymphoma by a small molecule inhibitor arrests tumor cell growth.

RAB7, encoded by RAB7A in humans and Rab7 in mice, is a small GTPase that catalyzes endosome maturation. It mediates NF-κB activation through the assembly of intracellular membrane signalosomes in stimulated normal B cells and plays a B cell-intrinsic role in the antibody response in mice. Here we show RAB7A transcripts are expressed in primary diffuse large B-cell lymphomas (DLBCLs), and that RAB7 protein expression is heightened in activated human tonsil B cells as well as in DLBCL and Burkitt lymphoma cell lines. Treating these cell lines with CID1067700, a selective small-molecule RAB7 inh

Imaging of Pulmonary Malignant Lymphoproliferative Disorders.

Malignant pulmonary lymphomas are a heterogeneous group of malignancies characterized by the clonal proliferation of lymphocytes within the lung. They are classified as primary, arising in the lung without extrathoracic involvement (mostly mucosa-associated lymphoid tissue lymphoma), or secondary, resulting from the spread of systemic lymphoma, often diffuse large B-cell or Hodgkin lymphoma. Imaging findings correlate with histopathologic features, showing single or multiple nodules/masses, lobar or segmental consolidations, reticular patterns, or disseminated small nodules. Although a definit

A Great Mimicker in Thoracic Spine: Spinal Double Expressor Lymphoma.

Diffuse large B-cell lymphoma (DLBCL) is one of the most common causes of non-Hodgkin's lymphoma (NHL). Some of these DLBCLs can have genetic mutations as well as protein overexpression. The genes involved are MYC, BCL-2 and BCL-6. These are very aggressive and do not respond well to standard chemotherapy regiment. Lymphomas usually show classic signs and symptoms but rarely can present with little or no symptoms or even mimic other disease processes. Here we will present a case where a spinal lymphoma mimicked a hematoma and the patient developed signs and symptoms only after mechanical fall

Isolated Pituitary Stalk Relapse of Primary Penile Lymphoma.

A 76-year-old Japanese man was admitted to our hospital because of the occurrence of multiple nodules on the shaft of his penis. He was diagnosed with diffuse large B cell lymphoma (DLBCL). His lymphoma was located only in his penis. He received immunochemotherapy and involved-field radiotherapy, and achieved complete response (CR). About two years later, he complained of a poor appetite. Magnetic resonance imaging showed a mass lesion in the pituitary stalk. Biopsy of this mass revealed the recurrence of DLBCL. He received whole-brain radiotherapy, and achieved CR. This is the first case of a

The clinical impact of staging bone marrow examination on treatment decisions and prognostic assessment of lymphoma patients.

This study investigates the value of performing a staging bone marrow in patients with diffuse large B-cell lymphoma (DLBCL), follicular lymphoma (FL) and classical hodgkin lymphoma (CHL). The results of 3112 staging bone marrow examinations were assessed for impact on prognostic assessment and critical treatment decisions. The detection of marrow involvement altered the disease-specific prognostic index for 4·3% of DLBCL, 6·2% of FL and 0·6% of CHL but marrow involvement in DLBCL was an independent prognostic factor. Knowing the marrow status potentially changed treatment in 92 patients, dete

A case of Mikulicz's disease complicated by malignant lymphoma: a postmortem histopathological finding.

A 66-year-old Japanese man with an 11-year history of Mikulicz's disease (MD) received continuous corticosteroid administration. At age 58, a left renal pelvic mass was identified and diagnosed as an IgG4-related inflammatory pseudotumor. The patient underwent an operation to remove the tumor. Subsequently, he contracted repeated pulmonary infections and eventually died of severe gastrointestinal bleeding. Autopsy revealed systemic lymph node swelling and infiltration in some organs, and diffuse large B-cell lymphoma (DLBCL) was diagnosed. These findings suggest that an IgG4-related disease ca

Successful treatment of primary malignant lymphoma of the penis by organ-preserving rituximab-containing chemotherapy.

Primary penile lymphoma is extremely rare. Here we report the case of a 67-year-old man with the chief complaints of difficulty in urination and priapism, who was eventually diagnosed with primary malignant lymphoma of the penis. Pathological examination of excision biopsy of the left inguinal lymph node revealed malignant CD20+ diffuse large B-cell lymphoma. We treated this patient with a systemic rituximab-chemotherapy regimen and obtained good results, in terms of both functional and cosmetic outcomes. Soluble interleukin-2 receptor was a useful tumor marker for evaluating the therapeutic e

Clinical features and prognostic relevance of ovarian involvement in non-Hodgkin's lymphoma: A Consortium for Improving Survival of Lymphoma (CISL) report.

Fourteen patients had primary ovarian lymphoma, while eighteen patients had secondary ovarian involvement. There was no significant difference in survival rates between primary and secondary involvement with diffuse large B-cell lymphoma (DLBCL), the most common subtype. The 5-year overall survivals of primary and secondary involvement with DLBCL were 70.0% and 59.3%, respectively. The localized bilateral ovarian involvement showed poorer survival compared to stage III/IV patients with secondary ovarian involvement. Treatment outcomes of secondary ovarian involvement in non-Hodgkin's lymphoma

Malignant lymphoma associated with rheumatoid arthritis, developing shortly after initiation of oral methotrexate.

We report a patient with rheumatoid arthritis (RA) who developed malignant lymphoma of the diffuse large B-cell type in the right submandibular region shortly after initiation of oral methotrexate (MTX). Despite cessation of MTX, the lymphadenopathy did not regress, and only reached complete remission after 3 courses of CHOP therapy followed by irradiation. In this patient highly active RA itself was considered to be the main cause of malignant lymphoma, and MTX might have contributed to the development by modifying the immune system. When RA is highly active, MTX should be used carefully beca
