Closing the gap between decision and delivery--amniotic fluid embolism with severe cardiopulmonary and haemostatic complications with a good outcome.

Perimortem caesarean section is very rare, mostly resulting in high mortality of mother and/or fetus. We report a case of successful resuscitation of both mother and newborn following maternal cardiac arrest prior to delivery. Postoperative outcome was complicated by severe bleeding and coagulopathy following fibrinolysis and subcapsular hepatic haematoma. We consider a fast reaction time based on a special in-hospital emergency team for immediate caesarean section and an aggressive management of coagulopathy as major factors that led to both patients recovery without neurological sequelae.

Long-term outcome of patients undergoing balloon mitral valvotomy in pregnancy.

The outcome of 36 patients (mean age 25.8 +/- 4.3 years) who underwent balloon mitral valvuloplasty during pregnancy is described in this report. The procedure was successful in 35 patients (97.2%), with no maternal mortality, and all patients subsequently had symptomatic improvement and uneventful deliveries. The children had normal growth and development at a follow up of 2.8 +/- 3.3 years. Thus, in patients with symptomatic severe mitral stenosis, balloon mitral valvuloplasty is feasible, safe, and effective with good long-term outcomes for both the mother and the fetus.

Choledochal cysts diagnosed in pregnancy: a case report and review of treatment options.

An 18-year-old woman at 18 weeks' gestation presented with abdominal pain and jaundice. After extensive evaluation, she was diagnosed as having a choledochal cyst. Ten days after percutaneous drainage, she underwent complete excision and Roux-en-Y hepaticojejunostomy anastomosis. At term, she delivered vaginally without complications. Although choledochal cysts are rare in pregnancy, it is important for the obstetrician to be familiar with the presentation and the best treatment modalities available. Delay in diagnosis and inappropriate treatment can result in maternal morbidity and fetal mort

Neonatal lupus erythematosus.

Neonatal lupus is categorized by typical clinical features and the presence of maternal auto-antibodies. Mothers are uncommonly affected with clinical disease. The major clinical manifestations in neonates are cardiac, dermatological and hepatic, with rare instances of haemolytic anaemia and thrombocytopenia. The major morbidity and mortality result from complete congenital heart block. Dermatological manifestations occur mainly over the face and present with plaques of erythema with central atrophy, a mid-facial erythema, atrophy around the eyes and a telangiectatic variety. The long term out

Management of severe preeclampsia.

Preeclampsia affects two patients, the mother and the fetus. Traditionally, women with severe preeclampsia have been delivered without delay, regardless of fetal considerations. Although delivery is appropriate therapy for the mother, aggressive management with immediate delivery of a fetus remote from term leads to high neonatal mortality and morbidity resulting from prematurity. Recent studies have shown favorable neonatal outcomes after conservative management of severe preeclampsia. Candidates for conservative management should be selected carefully and managed with intensive maternal and

Varicella during pregnancy: the timing of effective treatment.

Maternal varicella infections during pregnancy can be either benign or disastrous for the infant according to the timing of infection during gestation. We report a case of varicella occurring at term that was managed successfully. Appropriate management includes tocolysis, unless contraindicated, and varicella-zoster immune globulin (VZIG) administered to the mother within 72 hours of delivery, or to the neonate when the mother's rash appears within 48 hours after delivery. Treatment can markedly decrease the risk of mortality and severe morbidity in the relatively immunocompromised infant.

Neonatal myotonic dystrophy associated with prenatal ventriculomegaly. A case report.

Myotonic dystrophy is an autosomal dominant neuromuscular disease in which the maternal manifestations may be mild or subclinical. However, the congenital form, transmitted only from affected mothers, is reported to have a neonatal mortality rate as high as 50%. Ultrasonographic features of pregnancies with affected fetuses include hydramnios and decreased fetal movement, breathing motion and swallowing. Several reports have also noted an occasional infant with ventriculomegaly or hydrocephalus. It appears that prenatal ventriculomegaly may also be an occasional ultrasonographic finding herald

An analysis of cause-specific under-5 mortality in Bangladesh using the demographic and health survey 2011 and 2017-2018.

BACKGROUND: As the Sustainable Development Goal 3.2.1 deadline (2030) approaches, rapidly reducing under-5 mortality (U5M) gains more prominence. However, initiatives or interventions that aided Bangladesh in achieving Millennium Development Goal 4 showed varied effectiveness in reducing certain cause-specific U5M. Therefore, this study aimed to examine the predictors of the key cause-specific mortalities. METHODS: This cross-sectional study was conducted using the Bangladesh Demographic and Health Survey 2011 and 2017-18 data. Cause-specific U5M was examined using multilevel multinomial mixe

Neonatal Sequential Organ Failure Assessment as a late-onset sepsis mortality predictor in very low birth weight newborns: a Brazilian cohort study.

Death is a frequent occurrence in late-onset neonatal sepsis (LOS). We aimed to evaluate if the Neonatal Sequential Organ Failure Assessment (nSOFA) is associated with mortality due to LOS in very low birth weight (VLBW) infants. This is a single-center Brazilian cohort study including VLBW infants admitted between 2006 and 2020 who were diagnosed with LOS caused by Staphylococcus aureus, Enterococcus sp or Gram-negative bacteria. The primary outcome was mortality associated with sepsis. Two groups of patients-survivors and non-survivors-were compared regarding descriptive maternal and neonata

Perinatal outcomes of hypertensive disorders in pregnancy at a tertiary hospital in Ghana.

BACKGROUND: Hypertensive disorders in pregnancy remain a major global health issue not only because of the associated high adverse maternal outcomes but there is a close accompaniment of significant perinatal morbidity and mortality especially in Sub-Saharan Africa (SSA). However, the perinatal burden of HDP in Ghana has not been explored. We conducted this study to determine the perinatal outcomes of HDP at a tertiary hospital in Ghana. METHODS: A cross-sectional study conducted between January to February 2013 at Korle Bu Teaching Hospital (KBTH) in Accra, Ghana. Data collection involved ba

Burden and trend of maternal sepsis and other maternal infections in BRICS countries from 1990 to 2021.

BACKGROUND: Maternal septicemia and other maternal infections (MSMIs) continue to pose considerable challenges to public health on a global scale. However, comprehensive data on their impacts and trends are limited, particularly in BRICS countries, which include Brazil, Russian Federation, India, China, and South Africa. This study aims to compare the global epidemiological trends of MSMIs from 1990 to 2021 to MSMIs trends in BRICS countries and examine the relationship of disease burden with age and social development. METHODS: Using data from the Global Burden of Disease database, this stud

Vasa previa: Perinatal outcomes in singleton and multiple pregnancies.

Vasa previa (VP) is a rare and life-threatening condition for the fetus. It is associated with increased perinatal mortality rates. The current study sought to retrospectively analyze the perinatal outcomes of VP in singleton and multiple pregnancies between January 1, 2013 and December 31, 2019 at a tertiary hospital in west China. One hundred and fifty-seven cases of VP were identified, including 131 singletons, 23 twins and 3 triplets. VP in 20 cases was diagnosed at delivery. There were 183 live births. Neonatal mortality was significantly higher in cases with no prenatal diagnosis (9.7% v
