Beyond "Biomedical-Plus": Reclaiming the biopsychosocial model in musculoskeletal physiotherapy.

BACKGROUND: The biopsychosocial (BPS) model is widely endorsed as best practice in musculoskeletal (MSK) physiotherapy. However, its implementation often remains constrained by biomedical assumptions, with psychosocial approaches primarily used to improve adherence, reduce symptoms, or restore function. As a result, contemporary BPS practice risks becoming "biomedical-plus" care rather than a genuinely integrated approach. MAIN MESSAGE: We argue that physiotherapists can move toward a more authentic application of the BPS model through three interconnected foundational conditions - the 3Cs: p

Proteomic alterations in cerebrospinal fluid of spinal muscular atrophy patients undergoing nusinersen therapy: a systematic review and meta-analysis of potential biomarkers of treatment response.

This systematic review and meta-analysis synthesizes evidence on cerebrospinal fluid (CSF) proteomic alterations in patients with spinal muscular atrophy (SMA) treated with nusinersen. Across 21 studies, consistent post-treatment decreases were observed in neurofilament light chain (NfL), phosphorylated neurofilament heavy chain (pNF-H), and tau protein, which correlated with motor improvement, particularly in younger patients. Synaptic stress markers (e.g., α-synuclein, DJ-1) and metabolic proteins (e.g., apolipoprotein A1, transthyretin) demonstrated dynamic responses, suggesting systemic ef

Thermographic Behaviour of the Orbicularis Oris Muscle Under Different Provocative Tests.

BACKGROUND: Infrared thermography is an objective method for investigating muscle functioning, enabling inferences about physiology and therapeutics. This noninvasive and non-ionising imaging diagnostic method allows real-time visualisation of the vascular and musculoskeletal systems through skin microcirculation dynamics. It transforms information about the human body's thermal radiation, captured via infrared radiation, into an analysable image. OBJECTIVE: To describe the thermographic behaviour of the orbicularis oris muscle during sustained contraction and chewing tasks. METHODS: The stu…

Impact of sensory organization tasks on prefrontal cortex activity in older women: a comparative fNIRS study of osteoarthritis and healthy aging.

INTRODUCTION: Osteoarthritis (OA), a prevalent musculoskeletal condition, is associated with an increased risk of falls. Maintaining posture relies on visual, vestibular, and proprioceptive inputs, but these systems can be compromised due to aging or disease, heightening fall risk. Such impairments may result from neuromuscular decline and reduced cognitive or visuospatial processing abilities. This study aimed to investigate prefrontal cortical (PFC) activation patterns during clinical sensory organization tasks (SOT) using functional near-infrared spectroscopy (fNIRS) in older women with OA

Obesity: pathophysiology and therapeutic interventions.

Over the past few decades, obesity has transitioned from a localized health concern to a pressing global public health crisis affecting over 650 million adults globally, as documented by WHO epidemiological surveys. As a chronic metabolic disorder characterized by pathological adipose tissue expansion, chronic inflammation, and neuroendocrine dysregulation that disrupts systemic homeostasis and impairs physiological functions, obesity is rarely an isolated condition; rather, it is frequently complicated by severe comorbidities that collectively elevate mortality risks. Despite advances in nutr

Treatment of Pathologic Proximal Femur Fractures Using the Improvised Megaprosthesis: Combination of the Hip Prosthesis and Intramedullary Nail.

INTRODUCTION: The proximal femur is the most common long bone affected by metastatic disease. Pathologic fractures in this area are frequent, secondary to weight-bearing and deforming forces. Long-stem endoprosthetic replacement is often used to replace and bypass segments affected by metastases. However, implant cost remains prohibitive for patients in low-resource settings. An improvised megaprosthesis using a hip implant combined with Kuntscher nail provides an economic option. MATERIAL AND METHODS: This is a case series of three patients diagnosed with pathologic fracture of the hip secon

Association between arthritis and urinary incontinence in US adults: A cross-sectional study based on NHANES 2011-2023.

Although musculoskeletal conditions and lower urinary tract symptoms frequently coexist, the independent link between arthritis and urinary incontinence (UI) has received little empirical attention. We therefore quantified the strength of this association in a cross-sectional, nationally representative series of U.S. adults. We pooled 5 consecutive cycles of the National Health and Nutrition Examination Survey (2011-2023). Logistic regression was used to model UI (stress, urge, mixed) as the outcome and self-reported arthritis (osteoarthritis, rheumatoid arthritis or other types of arthritis)

Displaced patellar stress fracture revealing primary hyperparathyroidism: a case report.

BACKGROUND: Stress fractures are fatigue-induced injuries that typically affect weight-bearing bones of the lower limb. While often associated with mechanical overuse, atypical sites of fracture-such as the patella-should raise suspicion for underlying systemic pathology. Primary hyperparathyroidism (PHPT), characterised by autonomous parathyroid hormone overproduction, can lead to cortical bone resorption and fragility, predisposing patients to such fractures. CASE DESCRIPTION: A 45-year-old physically active woman presented to her general practitioner with non-specific right knee pain and n

Frequency of dysphagia among patients submitted to a rheumatology department: a cross-sectional analysis based on the EAT-10 questionnaire.

Symptoms related to difficulty in swallowing and bolus transfer from the oral towards the gastric cavity have been collectively termed "dysphagia". The etiology, gravity and complications of dysphagia warrant a rigorous, often multidisciplinary diagnostic evaluation and individualized management. The present study aimed to evaluate the prevalence of dysphagia among patients with rheumatic and musculoskeletal diseases (RMDs) and identify associated risk factors. This cross-sectional study included 340 consecutive patients diagnosed with RMDs from the Department of Rheumatology and Clinical Immu

Sequential Folliculitis, Paraspinal Myositis, and Septic Arthritis Due to Hematogenous Staphylococcus aureus Infection in a Patient Receiving Adalimumab Therapy.

Skin infections are a well-recognized source of bacteremia and can serve as foci for hematogenous spread leading to sepsis, septic arthritis, or osteomyelitis. Immunosuppressive therapies, particularly tumor necrosis factor alpha (TNF-α) inhibitors such as adalimumab, predispose patients to severe infections and are associated with increased infectious morbidity and mortality. These agents impair innate immune signaling and may attenuate the clinical manifestations of infection. Specifically, TNF-α blockade disrupts downstream interleukin-1β (IL-1β) and interleukin-6 (IL-6) pathways that media
