Growth and puberty in a fostered kindred.

A kindred of four children of different ages was at the same time adopted and transferred from a very poor to a fairly good environment. A rapid catch-up growth was observed in all four during the first 2 years after adoption. Beyond the 2nd year, physical development varied according to age, sex, and the rhythm of sexual maturation. Puberty was not advanced. Final height depended mainly on the height at the onset of puberty. The homogeneity of this familial story can contribute to a better understanding of the age-related interactions between the nutritional and environmental conditions and t

Biochemical indices and neuromuscular function tests in rural Gambian schoolchildren given a riboflavin, or multivitamin plus iron, supplement.

Ninety preselected children, aged between 8 and 14 years, living in two rural West African (Gambian) villages, were randomly divided into three groups, matched for age and sex. One group received a placebo (lactose) tablet, one received riboflavin (5 mg) on 5 d every week, which was sufficient to correct an endemic riboflavin deficiency, and one received a multivitamin supplement (Protovit; Hoffmann La Roche), on 5 d every week, together with FeSO4 (200 mg) once weekly, and the supplements were given for 1 year. Neuromuscular tests, including arm tremor and manipulative skills, were performed

The impact of schistosomiasis on human nutrition.

This article reviews studies that examine the negative effects of urinary and intestinal schistosomiasis on the following nutritional parameters in humans: urinary and faecal blood and iron loss, anaemia and haemoglobin levels, proteinuria, child growth and adult protein-energy status, physical fitness, physical activity, appetite and symptomatology. The conclusions reached are (1) that community-level treatment and control of schistosomiasis in areas where the infection, protein-energy malnutrition, and anaemia are common are to be encouraged and are likely to improve child growth, appetite,

Diarrheal disease as a cause of malnutrition.

Although long associated with infectious diseases, malnutrition is recognized as a major effect of specific infections, especially those of the gastrointestinal tract. Synergistic exacerbation of infections and nutritional deficiency commonly begin with weaning, where the impact of repeated infections and possible monocyte mediator release may have an even greater effect on malnutrition of young children than that of deficient diets in many areas. Reviewed here are the detailed host alterations seen with specific enteric infections that lead to malnutrition. These include mucosal dysfunction,

Maternal practices and risk factors for dehydration from diarrhoea in young children: a case-control study in central Thailand slums.

To determine factors related to dehydration from diarrhoea, we conducted a hospital-based, case-control study in children aged 24 months or younger who had acute watery diarrhoea and attended Chonburi Regional Hospital in central Thailand during November 1988 through May 1989. The study compared 48 cases who had moderate or severe dehydration with 48 controls who had no dehydration. Both cases and controls belonged to low socioeconomic families and were living in urban slum areas. They had adequate health care facilities and access to ORS packets. Overall, 56% of the mothers used ORS solution

Shwachman syndrome: a case report.

Cutaneous involvement is frequent in Shwachman syndrome, and includes various degrees of dry skin, and eczematous and ichthyosiform lesions. A 12-year-old boy with Shwachman syndrome had cutaneous involvement characterized by dry skin, perioral dermatitis, and follicular keratosis. Polymorphonuclear motility was decreased. A nutrition work-up showed a decrease in liposoluble vitamins, and suggested a causative link with the skin lesions.

Effect of undernutrition on the chemical composition and the activity of alkaline phosphatase in soluble and particulate fractions of the newborn rat calvarium and femur. I: Effect of gestational undernutrition in the rat.

We studied the effect of maternal protein deficiency on the development of the rat calvarium and femur. The results show that maternal protein deficiency during gestation leads to fetal growth retardation, and that the calvarium is more affected than the femur. A significant decrease in the activity of alkaline phosphatase was found in the soluble fraction in both the calvarium and the femur. However, this was not true in the case of the particulate fraction in which a significantly increased activity was found. This increase was not associated, however, with a concomitant increase in calcium

The epidemiology of pyoderma in Jamaican children.

Preschool and school-aged children from a small area of rural Jamaica were examined in an attempt to assess the epidemiologic factors predisposing certain persons to pyoderma. Twenty-three children were included in the affected group; their disease consisted of secondarily infected insect bites or infestations as well as primary pyoderma. The control group was composed of twenty-three children who had no evidence of infected skin disease. Comparison of the two groups revealed a significant difference in the socioeconomic status of the control and study groups. There was no statistically signif

Growth failure in infants with bronchopulmonary dysplasia: nutrition and elevated resting metabolic expenditure.

The mechanisms underlying growth failure in infants with bronchopulmonary dysplasia are poorly understood. Thirteen infants with bronchopulmonary dysplasia at 6 months of corrected age and 12 full-term healthy control infants matched for age or size were studied. Resting oxygen consumption was measured during natural sleep, and an estimation of the resting metabolic expenditure by indirect calorimetry was performed. Growth parameters were measured, and a nutritional profile including dietary intake, stool analysis, and serum albumin, cholesterol, glucose, and prealbumin was obtained. Seven of

Malnutrition and the heart.

Earlier concepts that the heart is spared in malnutrition have been shown to be incorrect. Inadequate intake of protein and energy results in proportional loss of skeletal and myocardial muscle. As myocardial mass decreases, so does the ability to generate cardiac output; however, various compensatory factors come into play. Nutritional supplementation for malnourished patients reverses the compensatory factors and may increase the short-term potential for heart failure. Severe cardiac debility results in poor nutrition, which may in turn produce unsuspected but clinically significant myocardi

Perinatal anatomic development of alveolar type II cells in rats.

We studied the perinatal development of rat alveolar type II cells. The volume density (Vv) of lamellar bodies increased twofold from gestational day 21.5 to postnatal day 1 and 1.6-fold between age 1 and 60 days. The Vv of glycogen areas fell sharply between late gestation and postnatal day 1; this decrease was greater in pups delivered from dexamethasone- compared with diluent-treated dams. In utero administration of glucagon to fetuses on gestational day 21.5 caused a decline in the Vv of glycogen areas within 4 h. Pups from dams underfed in late gestation had a higher Vv of glycogen areas

Nutritional nystagmus in infants.

Two infants presented with acquired infantile nystagmus. On examination, they manifested peculiar facies, growth and developmental delay, failure to thrive, and a Wernicke's type of nystagmus. Suspicion of a B vitamin deficiency resulted in correct diagnosis of dietary deficiency and/or enzymatic defect. In both instances, dietary treatment with B vitamins resulted in resolution of the nystagmus and improved growth and development of both individuals. Physicians, especially pediatricians and ophthalmologists, should be alert to the possibility of a Wernicke's type nystagmus in such infants, si

Changing concepts in treatment of severe symptomatic hyponatremia. Rapid correction and possible relation to central pontine myelinolysis.

Severe symptomatic hyponatremia (serum sodium level below 120 meq/liter) is often a life-threatening emergency that can result in permanent neurologic damage or death if left untreated. Early recognition and rapid correction to mildly hyponatremic levels by the administration of hypertonic saline are important in order to reduce the potential mortality and morbidity. If the serum sodium level is more than 105 meq/liter, it can be corrected to a value of 125 to 130 meq/liter. However, if the serum sodium level is less than 105 meq/liter, it may be safe to raise the value by only 20 meq/liter. C

Quantitation of apo-, mono-, and diferric transferrin by polyacrylamide gradient gel electrophoresis in patients with disorders of iron metabolism.

We have developed a polyacrylamide gradient gel electrophoretic method to quantitate apo-, mono-, and diferric transferrin based upon differences in their molecular size. Purified transferrin saturated to different extents (3% to 98%) with iron showed proportions of the three forms as predicted from an approximately random distribution of iron between the two metal-binding sites. The iron distributions in sera of 14 normal individuals similarly correlated with the predicted values. In contrast, 22 of 43 patients with diseases associated with abnormalities in iron or transferrin metabolism had

Immunofluorescent antigluten antibody test. Titer and profile of gluten antibodies in celiac disease.

Circulating antibodies to gluten fractions have been detected in most patients with celiac disease during gluten ingestion. The various detection techniques, however, are rather complex and inadequate for routine clinical use. Recently, a new indirect immunofluorescent method, named the antigluten antibody (AGA) test, has been developed. To establish the reliability of the test we compared six groups of patients: (1) 15 patients with biopsy-proved celiac disease of whom 13 had positive test results (the other two had already been receiving a gluten-free diet for two to three weeks); (2) 13 mal
