Therapeutic Potential of Exosomes in Bone Infection: Mechanisms of Action, Engineering Strategies, and Translational Applications.

Bone infections, including chronic osteomyelitis, implant-associated infection, and infected bone defects, remain among the most refractory challenges in orthopedics, as they involve persistent microbial retention together with defective bone regeneration. Their pathogenesis extends beyond bacterial colonization and reflects a complex interplay among pathogen persistence, dysregulated host immunity, sustained inflammation, impaired bone remodeling, vascular insufficiency, and compromised tissue repair. Exosomes have emerged as promising candidates for bone infection management because of their…

Anatomical distribution, the incidence of malignancy and diagnostic workup in the pathological lesions of the clavicle: a review of 410 cases.

BACKGROUND: The clavicle poses a diagnostic dilemma of the pathological lesions due to the wide range of pathologies seen at this site. This … ntation rate (ESR) and C-reactive protein (CRP) were not sensitive as a diagnostic tool in cases of osteomyelitis confirmed by histology. Magnetic resonance imaging (MRI) was noted to have high sensitivity and specificity for identifying the nature of a lesion and diagnosis. CONCLUSION: We have identified age as a positive predictor of a malignant cause in pat …

Comparison of antibacterial and antibiofilm activity of bioactive glass compounds S53P4 and 45S5.

BACKGROUND: Bone loss and deformation due to damage caused by injury or recurrent invasive infections presents a major clinical obstacle. While bone substitute biomaterials promote osseous tissue regeneration, their application in sites complicated by microbial infections such as osteomyelitis, is limited. Bioactive glass biomaterials (Bioglass) have been shown to have efficient mechanisms of repairing the integrity of bone, while inhibiting growth of a range of bacterial strains. There are several commercially available bioactive glass compounds, each with a unique chemical composition. One c…

Rate of concomitant cellulitis and osteoarticular infections in a pediatric population.

Concern for infection is a common presentation in pediatric emergency departments. Clinical signs of cellulitis in pediatric patients often … ction at a single tertiary care children's hospital. Data were analyzed to assess the prevalence of osteomyelitis concomitant with cellulitis in our sample population. Concomitant cellulitis and osteoarticular infection were present in 11% of all cases (53/482). Of the concomitant infections, 92% percent (49/53) were present in distal locations (Group 1) and 8% …

[A man in his fifties with pain in the head, neck and ear canal].

BACKGROUND: A man in his fifties, originally from a Middle Eastern country, presented with left-sided otalgia and neck pain which worsened o … pathogen that was found in a biopsy from his cervical spine. He was diagnosed with cervical mycotic osteomyelitis, probably secondary to a chronic external otitis. Long term antimycotic therapy and three neurosurgical operations were required to treat the patient.

New Concepts in the Management of Charcot Neuroarthropathy in Diabetes.

Charcot Neuroarthropathy (CN) is an uncommon, debilitating and often underdiagnosed complication of chronic diabetes mellitus though, it can … ent further deterioration and development of an ulcer. In individuals with a concomitant ulcer with osteomyelitis, soft tissue infection and severe deformity, where conservative measures fall short, surgical intervention becomes the only choice of treatment. Early diagnosis and proper management at an early stage can help prevent the occurrence of CN and amputa …

Treatment of odontogenic abscesses: comparison of primary and secondary removal of the odontogenic focus and antibiotic therapy.

PURPOSE: To examine the time of removal of the odontogenic focus, antibiotic therapy and risk factors in odontogenic abscesses. PATIENTS: F … rm (1.4%) complications. The long-term complications included a recurrence in two cases (1%) and an osteomyelitis in one case (0.5%). A logistic regression analysis identified no significant risk factor in relation to these complications. CONCLUSION: The study shows that a primary removal of the odontogenic focus may have advantages over a secondary removal: ( …

Can necrotic bone be objectively identified in chronic fracture related infections? - First clinical experience with an intraoperative fluorescence imaging technique.

INTRODUCTION: Adequate debridement of necrotic bone is of paramount importance for eradication of infection in chronic osteomyelitis. Currently, no tools are available to detect the exact amount of necrotic bone in order to optimize surgical resection. The aim of the present study was to evaluate the feasibility of an intraoperative illumination method (VELscope®) and the correlation between intraoperative and pathohistological findings in surgically treated chronic fracture related infection patients. METHODS: Ten consecutive patients with chronic fracture related infections of the lower ext…

Periodontal and Dental Considerations in Florid Cemento-Osseous Dysplasia: Clinical and Radiographic Analysis of 11 Cases.

PURPOSE: Florid cemento-osseous dysplasia (COD) is a well-described condition that occurs predominantly in black women. The lesion is usually asymptomatic, but in severe cases, symptoms can occur in association with focal expansion due to infection. The safety of dental procedures that involve areas with florid COD has been little discussed. Therefore, the present study aims to emphasise the importance of nonsurgical periodontal treatment in patients with periodontal diseases and COD. MATERIALS AND METHODS: Four thousand five hundred panoramic radiographs from the image bank of the School of

[Complications of sinusitis].

The complications of sinusitis are essentially secondary to ethmoidal and frontal sinusitis, occurring in patients weakened, particularly in case of immunodeficiency or anatomical defects. The gravity is due to the risk of spreading infection in intracranial tissues and orbital cavity. The diagnosis is always to discuss any symptomatology resistant to treatment and the appearance of orbital or neurological signs. The scanner and MRI contribute greatly to the orientation and diagnostic evaluation of the extent of the lesions. Treatment is based on prolonged antibiotic therapy and surgery and re

Maxillectomy for Cementifying Osseous Dysplasia of the Maxilla: A Case Report.

OBJECTIVES: Osseous dysplasias are the most common subtype of fibro-osseous lesions of the maxillofacial bones. They are benign and often present as incidental asymptomatic lesions. Diagnosis can be made with clinical and radiographic examination. CASE REPORT: This article reports the case of a 47-year-old man with a huge cementifying osseous dysplasia of the maxilla that presented with secondary infection after teeth extraction and repair of oroantral fistula. A subtotal maxillectomy had to be done after conservative treatment failed to resolve the infection. DISCUSSION/CONCLUSION: Biopsy,

[Lameness in children: diagnostic process].

A complete examination including the lower limb, the spine as well as a neurological exam must be done for every limp in children to eliminate an infectious process in particular. The complementary exams are guided by this clinical examination but standard assessment necessitate at least plain X-rays with an AP view of the pelvis and a lateral view of the hips, a blood count with the ESR and the CRP to eliminate rapidly a tumoral or an infectious process which requires quickly an appropriate treatment. In particular cases, this work-up must be repeated in association with other complementary e
