Distinguishing severe sleep apnea from habitual snoring using a neck-wearable piezoelectric sensor and deep learning: A pilot study.

This study explores the development of a deep learning model using a neck-wearable piezoelectric sensor to accurately distinguish severe sleep apnea syndrome (SAS) from habitual snoring, addressing the underdiagnosis of SAS in adults. From 2018 to 2020, 60 adult habitual snorers underwent polysomnography while wearing a neck piezoelectric sensor that recorded snoring vibrations (70-250 Hz) and carotid artery pulsations (0.01-1.5 Hz). The initial dataset comprised 1167 silence, 1304 snoring, and 399 noise samples from 20 participants. Using a hybrid deep learning model comprising a one-dimensio…

Novel digital markers of sleep dynamics: causal inference approach revealing age and gender phenotypes in obstructive sleep apnea.

Despite evidence that sleep-disorders alter sleep-stage dynamics, only a limited amount of these parameters are included and interpreted in clinical practice, mainly due to unintuitive methodologies or lacking normative values. Leveraging the matrix of sleep-stage transition proportions, we propose (i) a general framework to quantify sleep-dynamics, (ii) several novel markers of their alterations, and (iii) demonstrate our approach using obstructive sleep apnea (OSA), one of the most prevalent sleep-disorder and a significant risk factor. Using causal inference techniques, we address confoundi

Positive airway pressure settings do not predict outcomes of maxillomandibular advancement surgery in the treatment of obstructive sleep apnea.

PURPOSE: The purpose of this study was to determine if the therapeutic continuous positive airway pressure (CPAP) setting identified during pre-surgical CPAP titration polysomnogram (PSG) is predictive of surgical success after MMA in adults with OSA. METHODS: This multi-institutional retrospective study evaluated adults treated for OSA with MMA between 2000 and 2020 at Michigan Medicine and Massachusetts General Hospital. Patients included were those with a diagnosis of moderate-severe OSA (AHI > 15), a pre-operative CPAP titration PSG, and available pre-operative and postoperative PSGs. The

Development of a Machine Learning Model for Screening Sleep Apnea in Heart Failure Patients Using Sleep Sensor Data.

Sleep apnea (SA) is a prevalent disorder among individuals with heart failure (HF), often leading to complications. Early identification is essential for timely interventions and better outcomes. This study explores the feasibility of developing a screening tool for SA in patients with HF using data from the Future Patient Telerehabilitation program. A random forest classifier was used to develop a predictive model, achieving a promising receiver operating characteristic area under the curve (ROC-AUC) of 0.85, suggesting that the random forest classifier has the potential as a SA screening too

Gender Disparities in Patients with Atrial Fibrillation and Sleep Apnea.

INTRODUCTION: Atrial fibrillation (AF) and obstructive sleep apnea syndrome (OSAS) manifest differently in men and women, which may influence treatment decisions and adherence. The aim of our study was to investigate clinical, polysomnographic, and biological characteristics of AF-OSAS patients and to compare gender differences in this specific population. METHODS: In a prospective single center study, the authors analyzed a population of consecutive patients with AF (paroxysmal or persistent). Anthropometric measurements, clinical and polysomnographic parameters were collected and evaluated.

Obstructive sleep apnea is ralated to metabolic dysfunction associated steatotic liver disease in type 2 diabetes mellitus.

Obstructive sleep apnea (OSA) may be associated with the pathogenesis and severity of metabolic dysfunction-associated steatotic liver disease (MASLD). We aimed to determine the relationship between OSA and MASLD in type 2 diabetes mellitus (T2DM) patients. OSA was diagnosed via polysomnography, and liver ultrasound was used to measure the severity of MASLD in hospitalized patients with T2DM. The severity of OSA was categorized as non, mild, moderate, and severe according to the apnea-hypopnea index (AHI). Moreover, the oxygen desaturation index (ODI) and the lowest oxygen saturation (LSaO2) d…

Improving Obstructive Sleep Apnoea Mitigates Dampened Heart Rate Responses to Respiratory Events in Children With Down Syndrome.

Children with Down syndrome (DS) have a dampened heart rate (HR) response at obstructive respiratory event termination compared with typically developing children. Whether improving obstructive sleep apnoea (OSA) severity improves the HR response to both obstructive and central events remains unknown. Twenty-four children (3-19 years at baseline) were included. Children were grouped into improved (decrease in obstructive apnoea-hypopnoea index to ≤ 50% of baseline; n = 12; seven treated between studies) and unimproved (n = 12; two treated between studies) 2 years following baseline study.
