Management of Hypertension and Associated Comorbidities: An Expert Consensus Statement from India.

Hypertension is a leading global health concern that significantly contributes to cardiovascular (CV) and renal diseases. In India, its prevalence is rising, often coexisting with comorbidities such as type 2 diabetes mellitus (T2DM), chronic kidney disease (CKD), coronary artery disease (CAD), and metabolic syndrome (MetS). Effective hypertension management in these populations is challenging due to variations in blood pressure (BP) targets, the need for combination therapy, and the complexity of treating associated conditions such as albuminuria, nephropathy, CKD, CAD, and acute coronary syn

Managing Lymphedema Induced by Lymphatic Filariasis: Implementing and Improving Care at the Individual and Programmatic Levels.

Providing and improving the care of patients suffering from lymphedema remains an essential goal for the clinical management of populations affected by lymphatic filariasis. Although the Essential Package of Care (EPC) recommended by the WHO leads to important positive benefits for many of these lymphedema patients, it is important to continue to address the challenges that remain both in quantifying these effects and in ensuring optimal care. This report, based on the authors' scientific and field experience, focuses on the impact and significance of lymphedema, its clinical presentation, cur

Cancer pain during an epidemic and a pandemic.

PURPOSE OF REVIEW: As our global population ages, cancer has become more prevalent. Thankfully, oncologic treatments are highly effective, leading to significantly improved rates of long-term survival. However, many of these therapies are associated with persistent pain syndromes. Clinicians caring for people with cancer must understand how the influence of the current epidemic of opioid misuse and the coronavirus disease 2019 (COVID-19) pandemic have complicated cancer pain management. Creative solutions can emerge from this knowledge. RECENT FINDINGS: Persistent pain due to cancer and its t

Iron status of blood donors.

PURPOSE OF REVIEW: This review examines recent research on the prevalence and importance of iron deficiency in blood donors, and on efforts to mitigate it. RECENT FINDINGS: Premenopausal females, teenagers, and high-frequency donors are at the highest risk for donation-induced iron deficiency, in both high-resource and low-resource settings. The physiology relating iron stores to hemoglobin levels and low hemoglobin deferral is well elucidated in blood donor populations, yet the clinical effects attributable to iron loss in the absence of anemia are challenging to identify. Expanded adoption

A comprehensive approach to therapy of haematological malignancies in older patients.

Haematological malignancies are a heterogeneous group of diseases with diverse incidence. In Europe, the median age at diagnosis across all disease entities is 69 years. Incidence generally increases with age, reaching a maximum at 75-99 years, with the notable exceptions of Hodgkin lymphoma and acute lymphocytic leukaemia. Overall survival for patients aged 75 years and older with haematological malignancies is generally poor, particularly for acute leukaemias. Understanding the heterogeneity in outcomes for haematological malignancies, treatment challenges, and management of frailty and como

Nutritional guidelines for the management of insulin resistance.

Obesity and its related co-morbidities, namely type 2 diabetes (T2D), pose a significant global public health problem. Insulin resistance (IR) in muscle and liver is the core pathophysiologic defect that underlies obesity preceding and predicting the onset of T2D in susceptible humans. There is a broad population with IR that has no indication for prescription of medications, who still need medical consultation and specific advice in this respect. This prevalent need can be achieved by appropriate diet, exercise, and other behavioral therapies for lifestyle interventions. Despite a well-recogn

Central and peripheral arterial diseases in chronic kidney disease: conclusions from a Kidney Disease: Improving Global Outcomes (KDIGO) Controversies Conference.

Chronic kidney disease (CKD) affects about 10% of all populations worldwide, with about 2 million people requiring dialysis. Although patients with CKD are at high risk of cardiovascular disease and events, they are often underrepresented or excluded in clinical trials, leading to important knowledge gaps about how to treat these patients. KDIGO (Kidney Disease: Improving Global Outcomes) convened the fourth clinical Controversies Conference on the heart, kidney and vasculature in Dublin, Ireland, in February 2020, entitled Central and Peripheral Arterial Diseases in Chronic Kidney Disease. Thyroid hormone therapy in congenital hypothyroidism and pediatric hypothyroidism.

The evaluation and management of hypothyroidism in children are similar to adults, but there are important differences based on the dependence on normal thyroid function for neurocognitive and physical development. In the pediatric population, hypothyroidism is frequently categorized as congenital or acquired hypothyroidism, depending on the age of presentation and the underlying etiology. The evaluation and management of children and adolescents with hypothyroidism are determined by the etiology as well as by the age at diagnosis, severity of the hypothyroidism, and the response to thyroid ho

Substance abuse treatment in persons with HIV/AIDS: challenges in managing triple diagnosis.

Clinical management of HIV must account for the "triple diagnosis" of HIV, psychiatric diagnosis, and substance use disorders and requires integrated treatment services that focus beyond just mitigation of substance use and psychiatric and medical symptoms but also address other health behaviors. Because clinical management of HIV/AIDS has shifted significantly with the advent of highly active antiretroviral therapies (HAART) in the mid 1990s, a literature review focusing on literature published since 2000, and using relevant key words was conducted using a wide range of literature search data

Influence of the acquired immunodeficiency syndrome on resource availability in critical care medicine in the coming decade.

Information presented regards the consumption of resources in the management of critically ill patients with the acquired immunodeficiency syndrome (AIDS). Predictions are made about future increases in critical care services for patients with this condition. While increased need will primarily be related to an increased number of patients with AIDS, an expanded need for critical care services is also likely to result from changes in the populations at risk and in the presentation of the syndrome. In particular, the relationship between the AIDS epidemic and tuberculosis is described. The bala

Identification of serum metabolites associated with the EAT-Lancet planetary health diet, and their mediating role in weight status and weight changes in adults.

PURPOSE: Better adherence to a healthy and sustainable diet has been associated with a lower body weight, but the underlying metabolic mechanisms remain unclear. We examined whether serum metabolites mediate this association. METHODS: We used data from three Finnish population-based health examination studies (n = 9225, 54% of women, mean age of 54 years). Diet quality was assessed with a dietary score based on the Planetary Health Diet (PHD) adapted to Finnish food culture. Weight, height, waist circumference (WC), and body mass index were measured at baseline and follow-up, and their percen

Neuro-nutraceutical role of Indian spices, spice mixes and their bio-actives: An update.

Neurodegenerative disorders (NDD), one of the major health care challenges, are exponentially increasing as the elderly segment of the population continues growing in size worldwide. Currently, the role of nutraceuticals, in general and particularly for brain health, has generated immense research interest. Ancient, traditionally used culinary spices have been recognized for their potential health benefits. Abundant evidence clearly suggests that spices do impart considerable biological activity in the central nervous system (CNS). Owing to the multi-mechanistic actions of spices/bioactives, s

Recognition, treatment, and control of hypertension in the Danish population-based Lolland-Falster Health Study.

In the Danish population-based Lolland-Falster Health Study (LOFUS), conducted in a socioeconomically disadvantaged area with marked regional health inequalities, we recently identified a hypertension prevalence of 45%. A more detailed understanding of recognition, control, and treatment of hypertension in this setting may help improve interventions to reduce hypertension-related cardiovascular disease. This was a cross-sectional study including participants with hypertension identified in LOFUS using data from clinical examinations and questionnaires. Recognition was assessed by a registered
