Biomechanical analysis of the longitudinal ligament of upper cervical spine in maintaining atlantoaxial stability.

STUDY DESIGN: In vitro human cadaveric biomechanical study. OBJECTIVES: To investigate the roles of transverse atlantal ligament (TAL) and longitudinal ligament (LL) of the upper cervical spine (UCS) in maintaining atlantoaxial stability. SETTING: China. METHODS: Six intact UCS specimens were harvested and embedded in polymethylmethacrylate. Three-dimensional movements including flexion, extension, right and left lateral bending, and axial rotation, as well as the C1-C2 displacement in flexion (atlantodental interval, ADI), were tested on specimens with the following state sequentially: (1)…

Thoracic osteotomy for Gorham-Stout disease of the spine: a case report and literature review.

INTRODUCTION: Gorham-Stout syndrome is an aggressive, non-heritable skeletal disease characterized by osteolysis following minor trauma. The primary involvement of the spine is less common (10 %) and has been described in only about 20 cases; there is no consensus about the best way to treat this condition. PURPOSE OF THE STUDY: To report a case of Gorham-Stout syndrome involving the thoracic spine and to review the literature to suggest a post-operative treatment to prevent osteolysis. CASE REPORT: A thirty-year-old female patient was admitted to the unit in March 2013 for a pathologic T4 f…

Complications with the use of bone morphogenetic protein 2 (BMP-2) in spine surgery.

BACKGROUND CONTEXT: Recombinant human bone morphogenetic protein 2 (rhBMP-2) is a very potent osteogenic growth factor that has been used successfully in various spine fusions, obviating the need for autologous iliac crest bone graft harvest and therefore avoiding the associated morbidities. PURPOSE: In the past few years, a tremendous increase in rhBMP-2 usage was noted, and concerns regarding costs, benefits, and safety issues were raised by many. The goal of this work was to provide a comprehensive review of the adverse events and complications associated with use of rhBMP-2. STUDY DESIGN…

Factors that correlate with the decision to delay extubation after multilevel prone spine surgery.

BACKGROUND: Multilevel spinal decompressions and fusions often require long anesthetic and operative times, which may result in airway edema and prolonged postoperative intubation. Delayed extubation can lead to bronchopulmonary infections and other complications. This study analyzed which factors correlated with the decision to delay extubation after multilevel spine surgery. METHODS: We reviewed the records of 289 patients who underwent multilevel spine surgery lasting ≥8 hours in the prone position from 2006 to 2012. Variables hypothesized to affect the decision of the anesthesiologist to …

Indications for spine surgery: validation of an administrative coding algorithm to classify degenerative diagnoses.

STUDY DESIGN: Retrospective analysis of Medicare claims linked to a multicenter clinical trial. OBJECTIVE: The Spine Patient Outcomes Research Trial (SPORT) provided a unique opportunity to examine the validity of a claims-based algorithm for grouping patients by surgical indication. SPORT enrolled patients for lumbar disc herniation, spinal stenosis, and degenerative spondylolisthesis. We compared the surgical indication derived from Medicare claims with that provided by SPORT surgeons, the "gold standard." SUMMARY OF BACKGROUND DATA: Administrative data are frequently used to report proced…

Malignant triton tumor of the cervical spine: report of one case and review of the literature.

Malignant triton tumor (MTT) is a highly aggressive malignant neoplasm, classified as a variant of malignant peripheral nerve sheath tumor with rhabdomyoblastic differentiation. MTT is rarely reported in children, and its true prevalence may be underestimated. We herein report such a case in an 8-year-old boy who presented with a mass over the trapezius muscle. He was previously diagnosed with neurofibromatosis in the same area. Four years later, a follow-up magnetic resonance imaging revealed an intradural tumor recurrence at the level of C1-C7. An immunohistochemical test result was positive

Evaluation of distracting pain and clinical judgment in cervical spine clearance of trauma patients.

BACKGROUND: The concept of distracting pain (DP) is a controversial subjective confounder that often impedes the efficient and timely clearance of the cervical spine (C-spine). This study attempted to define DP more objectively and assess its true potential to mask the presence of C-spine injury. It also evaluated reliability and safety of clinical judgment in discounting the significance of pain peripheral to the neck (PP). METHODS: This prospective study included patients with a Glasgow Coma Score ≥14 at a level I trauma center presenting in a C-spine collar. Demographics, mechanism of inju…

The risks of epidural and transforaminal steroid injections in the Spine: Commentary and a comprehensive review of the literature.

BACKGROUND: Multiple type of spinal injections, whether epidural/translaminar or transforaminal, facet injections, are offered to patients with/without surgical spinal lesions by pain management specialists (radiologists, physiatrists, and anesthesiologists). Although not approved by the Food and Drug Administration (FDA), injections are being performed with an increased frequency (160%), are typically short-acting and ineffective over the longer-term, while exposing patients to major risks/complications. METHODS: For many patients with spinal pain alone and no surgical lesions, the "success"
