BACKGROUND/AIMS: The efficacy and safety of rifaximin in comparison with lactitol in the treatment of acute hepatic encephalopathy was assessed in a prospective randomized, double-blind, double-dummy, controlled trial.
METHODS: A total of 103 patients with grade I-III acute hepatic encephalopathy were randomized to receive rifaximin (50 patients, 1200 mg/day) or lactitol (53 patients, 60 g/day) for 5-10 days. Changes in the portal-systemic encephalopathy (PSE) index on entry and at the end of the study were used to evaluate the efficacy of the two therapies.

Oligosaccharide alditols were released from bovine submaxillary mucin by alkaline borohydride treatment. Two-hundred Wistar rats were allocated to 4 groups. Aldose reductase (AR), an enzyme which converts glucose to sorbitol, has been implicated in the pathogenesis of diabetic cataracts and retinopathy. The normal physiological role of this enzyme in ocular tissue, however, remains unclear. We investigated the influence of inositol triphosphate (IP3), trifluoperazine (TFP), and perhexiline on the calcium sensitivity of freeze-dried frog semitendinosus muscle fibres. Further, the effect of IP3 on calcium release from the sarcoplasmic reticulum (SR) of frog semitendinosus fibres skinned by saponin was studied. IP3 decreased the calcium sensitivity of freeze-dried frog skeletal muscle fibres and failed to induce a calcium release from SR of saponin-skinned fibres. Freeze-dried frog skeletal muscle fibres were strongly sensitized for calcium by TFP and perhexiline.

When incubated in high galactose media, fibroblasts from individuals with the severe (Mediterranean) variety of glucose-6-phosphate dehydrogenase (G6PD) deficiency accumulate significantly less galactitol than do fibroblasts from matched control subjects. Guinea pigs fed an ascorbic acid-deficient diet develop scurvy because of the absence of the enzyme L-gulonolactone oxidase. The spread of lung cancer to bone is a devastating complication often linked to resistance against immunotherapy, but the reasons for this connection are poorly understood. Here we show that the transcription factor YBX1 acts as a central regulator driving both bone metastasis and the formation of an immunosuppressive environment in non-small cell lung cancer (NSCLC). New-onset diabetes in the elderly or acute deterioration of glycaemic control in already known diabetics is sometimes linked with suspicion of pancreatic malignancy. We report the case of an 81-year-old woman with long-standing, type 2 diabetes, well-controlled on gliclazide, who developed worsening glycaemic control over four months. A contrast-enhanced CT scan performed for an unrelated indication four months earlier was reported as normal. Fermentation of coffee beverages with a symbiotic culture of bacteria and yeast (SCOBY) is an emerging alternative to traditional tea-based kombucha. This study examined the effects of SCOBY fermentation on the chemical composition and bioactivity of infusions prepared from green and roasted beans of Coffea arabica and Coffea canephora.
