Peroneus brevis muscle flap for a 20-year-old neuroischemic diabetic ulcer induced by traumatic injury: a case report detailing the efficacy of advanced combined treatment modalities for chronic wound closure.

INTRODUCTION: Peroneus brevis flaps provide a viable option to achieve soft tissue coverage in hard-to-heal lower extremity wounds, specifically those to the lateral ankle and hindfoot. CASE REPORT: The authors present a unique case of a patient with a 20-year-old wound dehiscence complicated by osteomyelitis. The wound was a complication from a lateral extensile incision utilized during prior calcaneal open reduction and internal fixation. Due to many factors, including multiple comorbidities, the patient could not obtain complete healing despite IV antibiotics, vascular optimization, local

Synthetic grafts for anterior cruciate ligament reconstructive surgery.

The quest for a good and durable substitute to anterior cruciate ligament (ACL) is driving scientists to explore new promising areas of research. Autologous and allogenic ligament reconstruction bring satisfactory results in managing ACL surgery although their use is associated with significant drawbacks. To overcome the limitations of biologic grafts, many artificial devices have been developed and implanted as a substitute to the native ACL over the past decades. Although many synthetic grafts used in the past have been withdrawn from the market due to early mechanical failures ultimately le

Free superficial circumflex iliac artery perforator flow-through flap transfer for reconstruction after excision of arteriovenous malformations of the hand: A case report.

The management of arteriovenous malformations (AVMs) of the hand remains challenging. When radical excision results in large defects of both soft tissue and vessels, flow-through flap transfer is useful; however, flow-through flap options for hand and digit reconstructions are limited. Herein, we describe the use of a superficial circumflex iliac artery perforator (SCIP) flow-through flap after excision of an AVM of the hand. A 44-year-old female patient with an AVM of the hand required simultaneous reconstruction of soft tissue, vascular, and bone defects after radical excision of vascular le

Preputial Shortening Reconstruction Surgery in a Dog with a Micropenis and Prepuce-to-Penis Size Disparity.

A 1 yr old castrated male shih tzu was referred for recurrent urinary tract infections (UTI), prostatitis, and urine dribbling that was not responsive to medical management. Physical examination and computed tomography scan revealed a micropenis with a disproportionately high prepuce-to-penis ratio. Preputial shortening with a hexagonal, full thickness preputial resection followed by preputial anastomosis was performed. The dog recovered from surgery with no complications. Urine dribbling persisted in the short-term postoperative period, but the patient achieved significant clinical improvemen

Right-sided luxation of the heart caused by blunt traumatic pericardial rupture.

Cardiac luxation is a rare condition in cases of blunt thoracic trauma, yet it is quite fatal. We present a case of a 28-year-old man, admitted to the emergency room after a motorcycle accident in a hemodynamically unstable condition and radiographic presentation of multiple rib fractures, bilateral pneumothorax, pneumomediastinum, and significant dislocation of the heart to the right. After performing emergency bilateral tube thoracostomy and achieving hemodynamic stability, a CT scan was performed and the patient was diagnosed with pericardial rupture with right-sided luxation of the heart.

Neo-glans reconstruction with dartos flaps covered with buccal mucosal graft after total glans amputation during circumcision: novel technique.

Penile glans amputation is a rare and catastrophic complication of circumcision. Reconstruction of the penile glans was indicated following amputation. Our report discusses a novel technique for reconfiguration of the amputated penile glans of a 5-year-old male admitted 6 months following a complicated circumcision. The parents complained of severe meatal stenosis and penile disfigurement. The penis was 3 cm long. Complete penile degloving was performed. The distal part of the remaining penis was prepared by removing fibrous tissue. Dartos flaps, which had been placed on the dorsal side by the

Accuracy of Orbital Shape Reconstruction-Comparative Analysis of Errors in Implant Shape Versus Implant Positioning: A Cadaveric Study.

INTRODUCTION: Orbital blowout fractures are commonly reconstructed with implants shaped to repair orbital cavity defects, restore ocular position and projection, and correct diplopia. Orbital implant shaping has traditionally been performed manually by surgeons, with more recent use of computer-assisted design (CAD). Accuracy of implant placement is also key to reconstruction. This study compares the placement accuracy of orbital implants, testing the hypothesis that CAD-shaped implants indexed to patient anatomy will better restore orbit geometry compared with manually shaped implants and man

[Reconstruction of orbital exenteration defects: about 20 cases].

The face is social support. Eyes allow the individual to interact with the environment and others. Orbital exenteration defects are the results of extreme disfiguring surgery, leading to functional, aesthetic and psychological sequelae. Reconstruction is essential and must meet several objectives: filling the cavity and closing any communications with the adjacent structures, obtaining rapid healing, allowing for local monitoring and finally enabling patients to reintegrate into society and achieve a satisfactory quality of life. We here present our experience in filling and reconstructing the

A Modified Technique to Harvest Integrated Zygomatic Arch-Temporal Bone Flap: Clinical Experience.

BACKGROUND: In skull base surgery, zygomaticectomy is an effective method to increase surgical exposure and reduce brain retraction. However, the traditional zygomaticectomy methods are complicated and more invasive. OBJECTIVE: To improve the procedure of zygomaticectomy, we introduced a modified technique to harvest integrated zygomatic arch-temporal bone flap. SUBJECTS AND METHODS: A modified technique to section the zygomatic arch integrated with the temporal bone flap was described in the present work. This technique was applied in eight skull base lesion patients. The improved surgical

Outcomes of plastic surgical reconstruction in extremity and truncal soft tissue sarcoma: Results from the US Sarcoma Collaborative.

BACKGROUND: This study aimed to define how utilization of plastic surgical reconstruction (PSR) affects perioperative outcomes, locoregional recurrence-free survival (LRRFS), and overall survival (OS) after radical resection of extremity and truncal soft tissue sarcoma (ETSTS). The secondary aim was to determine factors associated with PSR. METHODS: Patients who underwent resection of ETSTS between 2000 and 2016 were identified from a multi-institutional database. PSR was defined as complex primary closure requiring a plastic surgeon, skin graft, or tissue-flap reconstruction. Outcomes includ

Treatment of Adenoid Cystic Carcinoma of the Palate and Reconstruction of Defect With Nasoseptal Flap: A Clinical Case and Review of Literature.

Adenoid cystic carcinoma (ACC) is a malignant tumor arising from the salivary glands. While surgery is the mainstay of treatment for ACC of the palate, adjuvant radiotherapy and/or chemotherapy should be considered in high-risk cases. Oronasal fistula, a complication of palatal surgery, may cause speech disturbance and food regurgitation; the nasoseptal flap is a potential option to repair this defect as it is readily available and reliable. Here, we present a case of locally advanced ACC of the palate in a patient who underwent endoscopic-assisted transoral tumor excision with nasoseptal flap

Simulated slide tracheoplasty for congenital tracheal stenosis using three-dimensional printed models.

PURPOSE: The slide tracheoplasty (STP) is the standard treatment for severe congenital tracheal stenosis (CTS). Understanding the features of the tracheal stenosis in each case and choosing an appropriate incision design are very important for successfully executing the procedure. The present study aimed to evaluate the advantages of three-dimensional (3D) printed models of the trachea for improving CTS. METHODS: Three-D tracheal models were created using computed tomography (CT) data from ten patients undergoing STP for CTS. Simulated surgery was performed using the hollow models after reinf

Tensor fascia lata flow-through modification of the anterolateral thigh flap for the repair of through-and-through defects of the face.

The treatment of tumour recurrence or severe surgical complications in the head and neck region is often very complicated and limited. In particular, there is no suitable reconstruction strategy for patients whose necks are poor in blood vessels and whose anterolateral thigh (ALT) donor areas concurrently have only one available perforating branch. Therefore, a 'flow-through' technique was applied to fabricate chimeric ALT flaps for such patients. An ipsilateral tensor fascia lata flap was anastomosed to the distal end of the descending branch of the lateral circumflex femoral artery. Six pati
