Radial forearm flap plus Flexor Carpi Radialis tendon in Achilles tendon reconstruction: Surgical technique, functional results, and gait analysis.

BACKGROUND: Wound dehiscence, infection, and necrosis of tendon and overlying skin are severe complications after open repairs of Achilles tendon. A simultaneous reconstruction should be provided in a single stage operation. We evaluated the outcomes of one of the possible options: the radial forearm free flap with Flexor Carpi Radialis (FCR) tendon. METHODS: Between 2006 and 2014, six patients affected by infection and necrosis after Achilles tendon open repair underwent multi-tissutal reconstruction by a composite radial forearm free flap including a vascularized FCR tendon. The mean skin a

Tendon transfer or tendon graft for ruptured finger extensor tendons in rheumatoid hands.

We evaluated the clinical outcome of tendon reconstruction using tendon graft or tendon transfer and the parameters related to clinical outcome in 51 wrists of 46 patients with rheumatoid arthritis with finger extensor tendon ruptures. At a mean follow-up of 5.6 years, the mean metacarpophalangeal (MP) joint extension lag was 8 degrees (range, 0-45) and the mean visual analogue satisfaction scale was 74 (range, 10-100). Clinical outcome did not differ significantly between tendon grafting and tendon transfer. The MP joint extension lag correlated with the patient's satisfaction score, but the

Potential for Posterior Interosseus Nerve Entrapment Suggests Exploration Is a Viable Option Following Distal Biceps Repair: A Case Report.

CASE: A 34-year-old man developed weakness in finger extension following distal biceps repair, suggestive of posterior interosseus nerve (PIN) palsy. Surgical nerve exploration demonstrated direct PIN entrapment beneath the cortical button. Removal of the button resolved the patient's postoperative palsy. CONCLUSION: Precise placement of the cortical button is of utmost importance during repair of the distal biceps. Specifically, seating centered on the face of the bicipital tuberosity in line with the long axis of the radius will reduce the risk of PIN compression. If a PIN palsy should occu

Pulmonary embolism following bilateral quadriceps tendon repair with unilateral tourniquet use: A case report.

CASE: An active healthy 68-year-old male sustained a bilateral quadriceps tendon rupture while running. He underwent a simultaneous bilateral quadriceps tendon repair in a dual-surgeon approach. The right quadriceps tendon was repaired with a tourniquet, while the left quadriceps tendon tear was repaired without one. Postoperatively, the patient developed deep vein thrombosis in the right leg, which led to a bilateral pulmonary embolism. This case illustrates a potential complication of tourniquet use in patients undergoing quadriceps tendon repairs. CONCLUSION: The use of tourniquets in orth

Editorial Commentary: Isolated Arthroscopic-Assisted Posterior Latissimus Dorsi Transfer May Not Be Sufficient Treatment for Posterosuperior Irreparable Rotator Cuff Tears.

Posterosuperior irreparable rotator cuff tears (PSIRCTs) are challenging to treat with both conservative and surgical approaches. Patients with PSIRCTs often have a high burden of suffering with regard to pain level and functional impairments. Different surgical treatment options are available for PSIRCTs depending on age, functional requirements, and grade of osteoarthritis: joint-preserving procedures such as arthroscopic debridement, partial repair with or without augmentation, superior capsular reconstruction, and subacromial balloon spacer for young and active patients and joint-replacing

The effect of sterilization and storage on the viscoelastic properties of human tendon allografts - Continued: Storage for 0 to 4 months.

The role of donor-derived tendons, also known as allografts, in anterior cruciate ligament replacement surgeries is steadily increasing. Before surgery, temporary storage and, in most cases, sterilization are essential. It is, thus, crucial to determine how these procedures alter the grafts' biomechanical properties. The purpose of this research was to analyze the effect of different sterilization methods (native, frozen, frozen + 21 kGy gamma irradiation, frozen + 21 kGy electron beam irradiation) and storage durations (0 to 4 months) on the deformation and creep of two tendon types (tibialis

Tendon Transfers, Balloon Spacers, and Bursal Acromial Reconstruction for Massive Rotator Cuff Tears.

Several options exist for the management of irreparable rotator cuff tears without advanced arthritic changes. Tendon transfer poses technical challenges and has limited but promising outcomes data. Newer procedures such as balloon spacers and bursal acromial reconstruction are currently being investigated as a reproducible solution to this challenging problem. Ultimately the decision to continue with conservative measures, use one of the aforementioned techniques, or proceed with reverse shoulder arthroplasty remains a decision to be made in the context of patient's unique demands and provide

Editorial Commentary: The Long Head of the Biceps Tendon Is Useful for Shoulder Reconstruction Including Glenohumeral Stabilization: From Biceps Killers to Biceps Users.

Historically, it was believed that the shoulder long head of the biceps tendon (LHBT) was a pain generator and had to be routinely sacrificed. Recently, it has become apparent that the LHBT is useful as an autograft for various types of surgical reconstruction, including superior capsular reconstruction for irreparable rotator cuff tears, and augmentation for poor soft-tissue quality during rotator cuff repair or shoulder arthroplasty. In cases of shoulder instability, the biceps can reinforce the capsule or reconstruct a missing labrum for glenohumeral stabilization. Dynamic anterior stabiliz

Arthroscopic Intra-Articular Repair of the Torn Subscapularis Tendon with Single Anterior Portal, Single Suture Anchor, and X-Shaped Fixation Technique.

Subscapular tendon plays an important role in shoulder joint function. With the advance of magnetic resonance imaging technology and the popularization of arthroscopic shoulder surgery, subscapularis tears have been increasingly detected. However, reduction and fixation of subscapular tendon tears appears to be technically challenging. This study aims to describe an arthroscopic intra-articular X-shaped fixation technique: a procedure of subscapularis tendon repair performed with the aid of a suture passer using only a single anterior portal and a single suture anchor. By incorporating the adv

Restoration of elbow extension and construction of a "key grip" in people with tetraplegia. Comparative study of a surgical plan in one or two stages.

AIM: We report the comparative results of functional upper extremity surgery plans in patients with tetraplegia, including restoration of elbow extension and construction of a key grip in a "one-stage" or "two-stage" operation. MATERIALS AND METHODS: A series of 36 patients with tetraplegia, with a total of 45 operated upper limbs, was analyzed retrospectively with a mean follow-up of 23 months (13-39 months). We evaluated the analytical strength of elbow extension using the MRC (Medical Research Council) score, the strength of the key grip (kg/F) as well as the autonomy of the patients using

Short graft anterior cruciate ligament reconstruction: Current concepts.

Short graft anterior cruciate igament reconstruction is increasing in popularity for performing a primary ACLR. The short graft coupled with the all-inside technique using closed sockets and suspensory fixation at both femoral and tibial ends are its defining features. The outcomes of this technique have been comparable to well established transportal ACLR techniques. It has the benefits of preserved hamstring strength and less pain attributed to transtibial drilling. However, there is a learning curve involved and will require time before mastery of the technique. Furthermore, in combined ost

Intratendinous Strain Variations of the Supraspinatus Tendon Depending on Repair Technique: A Biomechanical Analysis Regarding the Cause of Medial Cuff Failure.

BACKGROUND: Double-row (DR) and transosseous-equivalent (TOE) techniques for rotator cuff repair offer more stability and promote better tendon healing compared with single-row (SR) repairs and are preferred by many surgeons. However, they can lead to more disastrous retear patterns with failure at the medial anchor row or the musculotendinous junction. The biomechanics of medial cuff failure have not been thoroughly investigated thus far. PURPOSE: To investigate the intratendinous strain distribution within the supraspinatus tendon depending on repair technique. STUDY DESIGN: Controlled lab

Biomechanical bases for tendon transfers at the shoulder.

Paralysis of the muscles around the shoulder is a debilitating condition that continues to be a very challenging problem. It leads to an inability to position one's hand in space. This greatly compromises the function of the upper limb and can lead to chronic shoulder pain due to inferior glenohumeral subluxation. Management of these complex problems has two main objectives: (i) stabilize the glenohumeral joint to decrease pain related to inferior glenohumeral subluxation; (ii) restore active range of motion in external rotation, abduction, and internal rotation. All the shoulder muscles contr

Biceps Rerouting for Semirigid Large-to-Massive Rotator Cuff Tears.

PURPOSE: To compare clinical and radiographic results of arthroscopic rotator cuff repair (ARCR) with biceps rerouting (BR) and those of conventional repair for semirigid, large-to-massive rotator cuff tear. METHODS: We prospectively collected data of 111 patients who underwent either ARCR + BR (n = 59, group 1) or only ARCR (n = 52, group 2) for semirigid, large-to-massive rotator cuff tear between January 2016 and December 2018. We comparatively analyzed both groups with respect to preoperative factors including concomitant lesions of the long head of the biceps tendon (LHBT). Univariate lo

Cape-like Anomalous Confluence of the Long Head of the Biceps and Rotator Cuff in the Presence of a SLAP Tear: A Case Report.

CASE: A 25-year-old woman sustained an acute tear of her superior anterior-posterior labrum with arthroscopic identification of an anomalous origin of the long head of the biceps arising exclusively from the supraspinatus tendon with no attachment to the supraglenoid tubercle or superior labrum. CONCLUSION: This is the first report of a symptomatic superior labrum anterior-posterior tear in a patient with a rare anatomic confluence of the long head of the biceps to the supraspinatus. We recommend addressing the labral pathology with utilization of a standard technique because satisfactory res

Surgical repair for abductor lesion after revision total hip arthroplasty: a systematic review.

BACKGROUND: Abductor mechanism lesions represent a severe complication after revision total hip arthroplasty (rTHA). The resulting abductor insufficiency can cause limping, pain, instability, and a higher rate of failure, thus requiring further revision surgery. In case of severe degeneration and retraction of the abductor mechanism, several different surgical treatments are needed. AIM: To systematically review all studies reporting on surgical treatment of severe abductor lesion after rTHA, focusing on surgical techniques, and clinical and functional results. METHOD: Scientific databases w

An arthroscopic technique for full-thickness rotator cuff repair by transposition of the long head of biceps.

Arthroscopic rotator cuff (RC) repair is now considered as an effective treatment for patients with symptomatic rotator cuff tears. We reported a new method for repairing a full-thickness RC tear by using the double-row technique with transposition of the long head of biceps (LHB). The novelty of this technique is using the long head of the biceps as an augmentation. The indication of this technique consists of two aspects including LHB lesions and RC tears. Three patients were enrolled. An ideal reconstruction of the anatomic footprint of the tendon and stabilization of glenohumeral joint was

Surgical treatment of the adductor longus muscle's distal tendon total rupture in a soccer player.

Only a few cases of adductor longus tendon ruptures have been reported in the literature and - there are no clear criteria for conservative or surgical treatment. A case of traumatic rupture of the right distal adductor longus tendon is presented in an elite soccer player, which was surgically repaired. The condition was managed conservatively primarily. However, after 2 months, a palpable mass remained on the medial side of the thigh, and the patient had pain after moderate everyday load and insufficient strength of the right leg during physical exercise. It was decided to explore ruptured te
