OBJECTIVE: A second and a separate traumatic spinal cord injury, which results in neurological deterioration, is very rare. In this report we describe a patient who became tetraplegic after sustaining a second spinal trauma.
CASE REPORT: A 27-year-old female had a C(7)-T(1) dislocation after a motor vehicle accident. She was neurologically intact and she had undergone a posterior fusion between C(6)-T(2). She made a complete recovery. Eight months after her initial trauma, she sustained a second motor vehicle accident causing a C(5) burst fracture.

We describe two cases of successful management by native valve salvage of an uncommon tricuspid valve regurgitation following blunt chest trauma. The two patients were diagnosed 13 years and six years, respectively, after the trauma. In both cases, tricuspid valvular insufficiency was caused by anterior leaflet prolapse due to chordal and papillary muscle rupture associated with annular dilatation. A 33-year-old male was involved in a single motor vehicle accident and was referred to our hospital 135 minutes after injury. He had a sign of cardiac tamponade and immediately pericardial window was performed at the emergency department, but hypotension went worse. Any thoracic trauma causes more or less severe hypoxia, due to pain, pleural effusion, mechanical ventilation disorders and pulmonary contusion. These four factors lead to bronchial congestion, which in turn aggravates hypoxia. More or less rapidly a vicious circle of respiratory failure is created. It can be broken only by early treatment of the cause. Without such treatment, respiratory failure quickly becomes autonomic and treatment of the cause is no longer sufficient.

Blunt traumatic rupture of the left ventricle of the heart is rarely diagnosed preoperatively and is usually fatal, with only a few survivors reported in the literature. This report describes a case of a 54-year-old woman who survived a left ventricular rupture from a motor vehicle accident. Her cardiac injury was not suspected because she was not hypotensive and had no signs of pericardial tamponade. She developed circulatory arrest 2 hours after her injury, during induction of anesthesia.

Tension pneumothorax in a large man was inadequately drained by needle thoracocentesis with a 4.5 cm cannula. Unsuccessful needle thoracocentesis of a clinical tension pneumothorax in a large patient should be followed immediately by chest drain insertion, without local anaesthetic, as dictated by clinical urgency. If the clinical situation is still not improved other diagnoses should be considered.

The clinical material summarized is derived from the observation of 150 patients. The accent is put on the faults in the X-ray interpretation of the pneumothorax and on the mistaken tactics in the treatment of the pneumothorax, which led to four lethal cases. The attention is drawn to the importance of the prompt physical study of traumatic patients, which gives the opportunity for prompt diagnosis and urgent adequate treatment. The indications have been precised relating pleurocentesis, pleural drain and when lacking aerostasis for thoracotomy.

Radiation-induced cardiotoxicity is a factor not to be neglected, especially after mediastinal irradiation in Hodgkin's disease and Non-Hodgkin-Lymphoma. Caused by primary and secondary radiation lesions, cardiopathy may develop up to several years after treatment. Clinically, acute pericarditis is prevalent. To reduce cardiac long-term side effects, adequate radiation technique is crucial. Diagnosis and therapy in radiation-induced cardiopathy are presented.

Vertebral column allografts, with their intervertebral discs, were implanted into thoracic spine defects (T7-T9) in 11 dogs in an attempt to re-establish spinal stability and preserve spinal biomechanics. We report a 31-year-old male patient who sustained blunt trauma to the chest, abdomen and left knee resulting in a liver laceration, left patella fracture and aortic regurgitation. Hepatorrhaphy, open reduction of the fractured patella and resuspension of the prolapsed aortic valve secondary to the aortic root intimal tear were performed subsequently. Prompt diagnosis and aggressive intervention of such injuries can lead to successful repair of potentially fatal cardiac trauma.

Torn pectoralis major muscles are either treated acutely by orthopedic surgeons or, if neglected, result in deformities of the pectoralis muscle. These deformities are visible, with pectoralis muscle asymmetry, especially on muscle contraction. Bodybuilders prone to this injury accentuate the asymmetry by contracting the pectoralis muscles together. Late repair is not usually possible, and cosmetic improvement relies on removing the bulging, unattached muscles or camouflaging the underlying exposed ribs with a customized implant. Sixteen patients with traumatic flail chest underwent surgical stabilization using procedures introduced by Rehbein, Judet and Vecsei. Compared with sixteen patients treated by internal stabilization with intermittent positive pressure respiration, there are some advantages: time of artificial respiration is shortened, early mobilization is possible, nursing is easier, the costs are lower. The procedure is indicated in those patients, in whom long-term artificial respiration is not necessary for other reasons.
