Aviation dentistry: current concepts and practice.

BACKGROUND: With the growing number of air passengers, flight attendants, leisure pilots as well as military and airline pilots, dentists may increasingly encounter flight-related oral conditions requiring treatment. Moreover, dentists should prevent the creation of in-flight hazards when treating aircrew members. The aim of this article is to introduce the concepts of aviation (aerospace) medicine and dentistry. METHODS: Data were gathered to cover the following issues: head and facial barotraumas (barotrauma-related headache, external otitic barotrauma, barosinusitis and barotitis-media), d

Surgical removal of an oral pyogenic granuloma and subsequent root coverage with a pedicle graft.

Pyogenic granuloma (PG) is a lesion characterized by non-neoplastic proliferation of endothelial cells, occurring in gingival tissue and representing an excessive reaction of the connective tissue to stimuli or injuries. The purpose of this report was to describe the treatment of an oral pyogenic granuloma, with emphasis on clinical, histopathological, and radiographic aspects. The surgical therapy comprised lesion excision followed by pedicle graft to cover the exposed root surface. The patient's pyogenic granuloma has been under control for a year, and recurrence has not been observed. The p

Facial nerve paralysis after impacted lower third molar surgery: a literature review and case report.

Facial nerve paralysis (FNP) is the most common cranial nerve disorders and it results in a characteristic facial distortion that is determined in part by the nerves branches involved. With multiples etiologies, these included trauma, tumor formation, idiopathic conditions, cerebral infarct, pseudobulbar palsy and viruses. FNP during dental treatment is very rare and can be associated with the injection of local anesthetic, prolonged attempt to remove a mandibular third molar and subsequent infection. We report a case of a 21 years-old black woman who developed a Bell's palsy after an impacted

Metaphyseal dysplasia of Braun-Tinschert type: report of a Japanese girl.

We report on a 7-year-old Japanese girl with metaphyseal dysplasia (MD) of Braun-Tinschert type, a recently recognized, autosomal dominant sclerosing bone dysplasia. All individuals with the disorder from four families in the literature originated from a small town in Bohemia or its vicinity. The occurrence of the disorder in a Japanese girl indicates that it is not restricted to Germans. The radiographic hallmarks of the disorder include metaphyseal undermodeling (Erlenmeyer-flask deformity); osteosclerosis of the chondroosseous junctions, metaphyseal cortices, and epiphyseal margins; and exo

A scanning electron microscopy study of idiopathic external tooth resorption in the cat.

BACKGROUND: Multiple idiopathic root resorption (MIRR) is a rare condition in man characterized by cervical resorption leading to significant tooth loss. A similar condition, feline osteoclastic resorptive lesions (FORL), affects up to 70% of domestic cats and thus provides a valuable model for investigating the etiopathogenesis of MIRR. The aim of the present study was to establish changes in the surface microanatomy of the tooth in late stage FORL and to identify whether its location has a surface bias. METHODS: Scanning electron microscopy (SEM) was used to analyze the surface features of

Hypodontia in an adolescent with the HbSC genotype: a case report.

The haemoglobin SC (HbSC) genotype is a variant form of haemoglobin disorder that often presents less frequent clinical symptoms of sickle cell disease (SCD) than the homozygous state (HbSS). Various dental abnormalities, such as enamel hypoplasia, hypomineralization, midline diastema and delayed eruption of the teeth, have been associated with SCD. This report presents the case of an adolescent patient with HbSC who had hypodontia, transposition of a permanent canine, retained primary teeth, multiple carious lesions and poor oral hygiene complicated by sickle cell crisis. This report focuses

Eruption cyst formation associated with cyclosporin A.

BACKGROUND: Cyclosporin A (CyA) is a potent immunomodulatory agent with a wide range of applications. Despite its therapeutic value, multiple adverse effects of CyA have been identified. This case report describes eruption cyst formation as a possible adverse effect of CyA administration during tooth eruption in a boy treated with CyA as a consequence of a cardiac transplantation. The clinical diagnosis of eruption cyst was confirmed by histopathological examination. TREATMENT: The periodontal treatment consisted of supragingival and subgingival scaling, followed by surgical removal of the ti

Prodromal herpes zoster--a diagnostic challenge in endodontics.

This case report highlights the diagnostic challenge that herpes zoster represents if pain develops in the prodromal stage. A 58-year-old male presented with pain in the left maxilla. The symptoms had lasted for 7 months. The first premolar had been extracted soon after the onset, and the second premolar root filled shortly thereafter. Symptoms were experienced as sudden pain attacks lasting for several hours and analgesics gave some pain relief. Clinical examination showed that the second premolar was tender to percussion. No sinus tract or swelling were present. Radiographic examination show

A lesion of endodontic origin misdiagnosed as a globulomaxillary cyst.

This report presents a case of a lesion originating from a pulpless central incisor misdiagnosed as a globulomaxillary cyst. The initial diagnosis of globulomaxillary cyst was made solely from radiographs by observing an almond-shaped radiolucency between the lateral incisor and cuspid. No pulp testing was performed before surgical removal of the lesion. The final diagnosis of an endodontic lesion caused solely by a pulpless central incisor was complicated by the initial lack of a pulpal response from the lateral incisor next to the surgical site. By delaying the start of endodontic treatment,

Clinical and morphological phenotype of HMSN 1A mosaicism.

Clinical, neurophysiological and morphological studies on a patient with mosaicism of the 17p11.2 duplication were performed in detail for the first time. Since duplication occurs during paternal meiosis, a somatic reversion is suggested, leading to mosaicism. The proportion of nuclei with duplication varied markedly between 49% in blood cells and 74% in tissue from the sural nerve. Clinically, mild symptoms of a motor and sensory neuropathy were present. However, neurophysiological changes and findings in the sural nerve biopsy were consistent with a typical hereditary motor and sensory neuro

[Oral complications following a herpes zoster infection of trigeminal nerve].

A case of herpes zoster involving the ophthalmic and maxillary divisions of the trigeminal nerve is reported. It presented as a oral herpes zoster infection with prodromal odontalgia and progressed to spontaneous exfoliation and devitalization of teeth and osteonecrosis of the maxilla. The literature is reviewed and the pathophysiology of tooth exfoliation, tooth devitalization and osteonecrosis by V-Z viruses are discussed in addition to the management of herpes zoster and post-zoster complications.

Treatment of an osseous lesion associated with a severe palato-radicular groove: a case report.

This is a case report that describes the management of a severe periodontal defect associated with a palato-radicular (palato-gingival) groove affecting the maxillary right lateral incisor of a 50 year-old male. The patient presented with pain, gingival swelling, and a 10 mm periodontal pocket on the distopalatal aspect of the right maxillary lateral incisor. The defect was initially treated by scaling and root planing. Several days later a flap was elevated, the osseous defect was debrided, and odontoplasty was performed to eliminate the groove. The root surface was treated with citric acid f

Analysis of the DNA duplication 17p11.2 in Charcot-Marie-Tooth neuropathy type 1 pedigrees: additional evidence for a third autosomal CMT1 locus.

We have restudied two clinically typical Charcot-Marie-Tooth neuropathy type 1 (CMT1; also known as hereditary motor and sensory neuropathy 1) pedigrees that were previously reported to be unlinked to the regions of proximal chromosome 1q and chromosome 17p by multipoint linkage analyses. In these two pedigrees, there is no evidence for linkage to additional DNA markers that flank and span the CMT1A locus on chromosome 17p11.2, and a duplication associated with CMT1A is not present in these pedigrees. These findings confirm that the CMT1 locus in these two pedigrees does not map to chromosome

Evaluation of biocompatible osteoconductive polymer activity and biocompatibility for minor oral surgery in animals and humans.

The speed of recovery with and biocompatibility of biocompatible osteoconductive polymer used for minor oral surgery were investigated in animals and humans. Histological and radiological follow-up of graft sites and experimental mandibular bone loss was performed in 20 rabbits. Evaluation of X-rays showed no notable difference between the treated and control animals; however, biopsies 60 days after surgery showed that those animals treated with biocompatible osteoconductive polymer did not have any interposition of connective tissue between the polymer and the new bony tissue whereas the cont

Type I dentin dysplasia: report of two cases.

Dentin dysplasia is a rare developmental disturbance of dentin affecting approximately 1:100,000 people. It has been classified as an autosomal dominant disease. Two distinct forms of dentin dysplasia have been described. As more cases of dentin dysplasia were reported these categories seemed inadequate; subclassification of type I dentin dysplasia were proposed based on root length and the presence or not of a pulpal remnant. This paper presents two cases demonstrating the classic features of type I dentin dysplasia in the mixed and permanent dentitions and discusses the suggested subclassifi

[Persistent fever of dental origin].

Based on 5 cases of unexplained prolonged fever, the authors stress the need to systematically look for a dental focus of infection. They discuss the difficulties in determining the site of the probable causal focus and stress the uncertain pathogenic relationship between the dental focus of infection and the fever. The extraction of infected teeth leads to a cure, however, the functional disadvantages of multiple extractions need to be carefully taken into account.

[Early orthodontic treatment].

Several preventive procedures are recommended for the interception of early malocclusion. Therefore some important characteristics of normal dental development are mentioned. This preventive approach is focused around selected topics as forced bite, midline diastema and retarded eruption. This influence of functional disturbances on dentofacial morphology is discussed. An overview of possible therapy by the general dentist is presented for problems, aroused by thumb- and fingersucking, agenesis of upper lateral and mandibular second premolars as well as early loss of deciduous molars. Finally,
