Reverse shock index multiplied by Glasgow Coma Scale score (rSIG) is a simple measure with high discriminant ability for mortality risk in trauma patients: an analysis of the Japan Trauma Data Bank.

BACKGROUND: The shock index (SI), defined as heart rate (HR) divided by systolic blood pressure (SBP), is reported to be a more sensitive marker of shock than traditional vital signs alone. In previous literature, use of the reverse shock index (rSI), taken as SBP divided by HR, is recommended instead of SI for hospital triage. Among traumatized patients aged > 55 years, SI multiplied by age (SIA) might provide better prediction of early post-injury mortality. Separately, the Glasgow Coma Scale (GCS) score has been shown to be a very strong predictor. When considering these points together, rS

Scratching Below the Surface: Screening for Posttraumatic Stress Symptoms Following Hospitalization With the Pediatric Trauma Service.

UNLABELLED: Significant progress has occurred medically for children who have experienced traumatic injuries; however, attention to their psychological adjustment has only more recently been a focus in research and clinical practice. These needs do not cease at discharge but, instead, require monitoring to determine whether further assessment and/or intervention are required. Our team, inclusive of the Psychology Service and the Trauma Service, identified 2 established screening measures (based on age) that were completed by patients during their outpatient follow-up visits postdischarge. Shou

Surgical treatment of rib fracture nonunion: A single center experience.

INTRODUCTION: In contrast to the emerging evidence on the operative treatment of flail chest, there is a paucity of literature on the surgical treatment of rib fracture nonunion. The purpose of this study was to describe our standardized approach and report the outcome (e.g. patient satisfaction, pain and complications) after surgical treatment of a rib fracture nonunion. METHODS: A single centre retrospective cohort study was performed at a level 1 trauma centre. Symptomatic rib nonunion was defined as a severe persistent localized pain associated with the nonunion of one or more rib fractur

Glasgow motor scale alone is equivalent to Glasgow Coma Scale at identifying children at risk for serious traumatic brain injury.

BACKGROUND: Glasgow Coma Scale (GCS) is a validated assessment of neurologic state. Assessment of the eye and verbal components is difficult to reliably obtain in children. We hypothesized that an abnormal Glasgow motor scale (GMS) score alone will reliably identify children with serious traumatic brain injury (TBI). METHODS: We reviewed all children with a diagnosis of TBI from 2002 to 2011 at two urban Level I pediatric trauma centers. We used logistic regression to model GCS, GMS, Glasgow verbal scale (GVS), and Glasgow eye scale (GES) for seven outcomes: need for craniotomy, intracranial …

Treatment of hypertrophic distal tibia nonunion and early malunion with callus distraction.

BACKGROUND: Hypertrophic nonunions and early malunions of pilon and distal tibia fractures result in complex, challenging to treat deformities. Callus distraction histiogenesis is an option for their management, allowing for the simultaneous correction of multiplanar deformity and limb length discrepancy. METHODS: A single-surgeon, retrospective case series was performed. Eight patients (6 males and 2 females) who were treated with callus distraction from 1991 to 2011 were reviewed. Six of 8 patients had varus deformities (range, 8-19 degrees) and 2 patients had valgus deformities (both 16 de

Complications after plate fixation and elastic stable intramedullary nailing of dislocated midshaft clavicle fractures: a retrospective comparison.

PURPOSE: The incidence of operative treatment of dislocated midshaft clavicle fractures (DMCF) is rising due to unsatisfactory results after non-operative treatment. Knowledge of complications is important for selection of the surgical technique and preoperative patient counselling. The aim of this study is to compare complications after plate fixation and elastic stable intramedullary nailing (ESIN) with a titanium elastic nail (TEN) for DMCF. METHODS: A retrospective analysis of our surgical database was performed. From January 2005 to January 2010, 90 patients with DMCF were treated with p

Musculoskeletal injuries in terrorist attacks--a comparison between the injuries sustained and those related to motor vehicle accidents, based on a national registry database.

Terror-related injuries are becoming more prevalent. The predominant mechanism of damage is related to blast effects. These include penetrating injury due to material in the explosives and blunt trauma due to objects falling after detonation. However, the more commonly encountered severe trauma in civilian centres is related to motor vehicle accidents. A comparison between the two, although problematic, might enhance the knowledge of orthopaedic traumatologists dealing with these injuries. Thus 1072 in-patients, treated in levels I and II centres in Israel for orthopaedic injuries due to terro

Comparison of the effectiveness of trauma services provided by secondary and tertiary hospitals in Malaysia.

STUDY OBJECTIVE: The trauma services provided by 6 hospitals operating at 2 levels of care (4 secondary or district general hospitals and 2 tertiary care hospitals) in Malaysia are compared in terms of mortality and disability for direct admissions to emergency departments to test the hypothesis that care at a tertiary care hospital is better than at a district general hospital. METHODS: All cases were recruited prospectively for 1 year. The hospitals were purposefully selected as typical for Malaysia. There are 3 primary outcome measures: death, musculoskeletal impairment, and disability at

Bacterial contamination of hospital pagers.

We assessed the bacterial contamination of the pagers of healthcare personnel and the efficacy of disinfection with 70% isopropyl alcohol. Microorganisms were isolated from all pagers; 21% yielded Staphylococcus aureus, of which 14% were methicillin resistant. Cleaning with alcohol reduced the total colony count by an average of 94%. Bacterial load varied by healthcare worker group and service assignment.

A population-based study of outcome after injury to car occupants and to pedestrians.

OBJECTIVE: The literature indicates that trauma center pedestrian mortality rates approximate twice that of injured car occupants. This study was performed to test the hypothesis that outcomes will be similar given similar degrees of injury severity if analyzed from an epidemiologic perspective. Differences are expected in the cause and place of death because of different injury profiles. DESIGN: Prospective population-based study. METHODS: The study group consisted of all adult car occupants and pedestrians sustaining major injury -- Injury Severity Score (ISS) of >15 -- in a defined area o

Psychosocial characteristics of alcohol-involved and nonalcohol-involved seriously injured drivers.

This study compared two groups of alcohol-positive and alcohol-negative, seriously injured, crash-involved drivers on demographics, personality characteristics, driving-related attitudes, prior driving history, lifestyle, substance use, and antecedent driver condition. The study sample was drawn from motor vehicle accident admissions to the Sunnybrook Health Science Centre Regional Trauma Unit. One hundred and six interviews were completed between August 1986 and November 1989, with blood alcohol concentration (BAC) data available for 96 drivers. These data suggest no driving-related attitude

Prehospital Risk Stratification for Early Transfusion in Pediatric Trauma: A National Trauma Data Bank Analysis.

OBJECTIVES: Pediatric criteria remain limited for identifying injured children who may benefit from early blood transfusion for hemorrhagic shock. This study evaluated whether emergency medical services (EMS)-observable variables in a national trauma registry could identify children at risk for early hospital blood transfusion. METHODS: We conducted an observational retrospective cohort study using the National Trauma Data Bank (2017-2020). Among 391,126 records for patients aged 1-15 years, we included children transported by EMS directly from the scene to a participating trauma center after

Prospective blinded interim evaluation of a portable radiofrequency device TES HT to support triage in emergency department patients with mild head trauma or acute neurological symptoms.

BACKGROUND: Emergency Department (ED) evaluation of patients presenting with mild head trauma or acute neurological symptoms is common and often requires rapid exclusion of intracranial pathology. Although computed tomography (CT) is the diagnostic reference standard, its widespread use raises concerns regarding radiation exposure, cost, and ED workflow efficiency. TES HT100 is a portable, non-invasive radiofrequency-based device intended to assist early triage decisions. We report the prespecified interim analysis of a prospective validation study conducted in an ED population. METHODS: Betw

Area Deprivation Index and Social Determinants of Health: Association With Loss to Follow-Up After Facial Trauma Surgery.

BACKGROUND: Postoperative loss to follow-up in facial trauma patients is a persistent challenge potentially linked to adverse outcomes. However, factors associated with loss to follow-up remain poorly characterized. PURPOSE: The purpose of this study was to measure the association between socioeconomic deprivation, as defined by the Area Deprivation Index (ADI), and postoperative loss to follow-up within 90 days of discharge after operative management of facial trauma. STUDY DESIGN, SETTING SAMPLE: This was a retrospective cohort study of all adults undergoing operative treatment for facial

Decreased Frequency of Splenectomy for High-Grade Splenic Injuries Following the Implementation of an Interventional Radiology Alert Protocol.

Background A vascular interventional radiology (VIR) protocol was initiated at a mature level 2 trauma center on 3/1/2023 to reduce time to vessel puncture to < 60 minutes as per American College of Surgeons (ACS) guidelines. The average time to vessel puncture in the two years prior to protocol initiation was 102 minutes. Following initiation, time to vessel puncture was reduced to 48.2 minutes (p < 0.001). A statistically significant increase in the number of splenic embolizations was noted in the post-protocol group. The primary outcome of the current study was to evaluate if the initiation

Reliability of patient and proxy reported pre-injury functional status in orthopaedic trauma: A prospective observational study.

BACKGROUND: Patient-reported outcome measures (PROMs) are essential for evaluating functional recovery after orthopaedic trauma. However, obtaining accurate pre-injury baseline PROM scores is often infeasible due to the emergent nature of traumatic injuries. This study aimed to assess the reliability of retrospective patient- and proxy-reported pre-injury functional status over time. METHODS: This prospective observational study was conducted at a level I trauma center and included 173 adult patients presenting with acute upper or lower extremity fractures. Participants completed Patient-Repo
