The emerging role of the microbiome in bladder cancer: prognostic implications and treatment response.

Bladder cancer (BCa) is a histologically and molecularly heterogeneous disease and is one of the leading causes of cancer death globally. The main risk factors are sex (with incidence 3 to 4 times higher in men), tobacco usage, occupational exposure to carcinogens, and persistent infections, such as those caused by Schistosoma haematobium. Urine and the bladder were recently confirmed to be non-sterile, prompting investigations into the urinary and intratumoral microbiomes and their roles in tumor stage, prognosis, and therapy response. In this context, the role of the urinary and intratumoral

How frequent are overactive bladder symptoms in women with urodynamic verification of an overactive bladder?

OBJECTIVE: To determine the relationship between overactive bladder symptoms and urodynamic verification of overactive bladder. MATERIAL AND METHODS: Between June 2011 and November 2011, 159 patients underwent urodynamics (UDS) at our urogynecology unit in the Ege University Hospital. Of these, 95 patients who complained of urgency, did not have any overt neurological diseases, bladder outlet obstruction and did not take any medication affecting the lower urinary tract function were evaluated. SPSS (ver. 15.0) was used to evaluate the data and the chi-square test and t test for independent sa

Prenatal resolution of megacystis possibly caused by spontaneous rupture of posterior urethral valves.

We report herein a case of resolution of severe megacystis, possibly caused by spontaneous rupture of posterior urethral valves during follow-up on a prenatal ultrasound. A 32-year-old woman presented at gestational week 15 for evaluation of fetal bladder enlargement. Prenatal ultrasonography revealed megacystis and posterior urethral dilatation. The longitudinal diameter of the bladder was 25 mm. Megacystis spontaneously resolved at gestational week 16. No association with urinary ascites was observed, and amniotic fluid volume remained normal throughout gestation. A boy was delivered vaginal

Our experience with salvage genitourinary fistulae repair: technique and outcomes.

OBJECTIVES: The objectives are to present the long-term results of vaginal reconstructive operations using the labial fat pad flap (Martius flap) interposition. PATIENTS AND METHODS: Eight women, 27-65 years old (mean 40), suffering from urinary fistulae (five urethrovaginal and three vesicovaginal) who failed primary repair underwent salvage vaginal reconstruction for damaged urethra or bladder. Urethral or bladder and vaginal defect was closed and a Martius fat flap was interposed between urethra or bladder and vaginal wall flap to secure a watertight separation of the structures. A Martius

[Experimental bladder augmentation with Gore-tex amine: biomechanical, biochemical and biostructural aspects].

We conducted an experimental study on 20 dogs that were subdivided into two groups: the control group comprised 5 dogs that were submitted to resection of 50% of the bladder; the other group comprised 15 dogs submitted to resection of 50% of the bladder and replacement with Gore-tex patch, which was removed 3 months thereafter in 10 dogs. CPK, creatinine, ions, and urinary pH values were determined and urine cultures were performed. The biomechanical parameters of the bladder wall were determined by cystometry. A histological study with hematoxylin-eosin of bladder dome specimens was done. The

Urinary incontinence in children: suggestions for definitions and terminology.

The definition of childhood urinary incontinence, used here, is leakage of at least 1 ml of urine, at least once a week, in a child from 5 years of age. The types of incontinence are defined as detrusor incontinence (daytime urge incontinence and night-time enuresis), sphincter incontinence (presenting clinically as stress incontinence in the less serious form, as continuous dribbling of urine in the more serious one), and combined incontinence (the combination of detrusor hyperactivity and sphincter incompetence found in many children with neurogenic bladder dysfunction). Urinary incontinence

Follow the Stream: Imaging of Urinary Diversions.

Urinary diversion is a surgical technique to redirect the stream of urine, most often after cystectomy. Cystectomy may be performed both for benign and for malignant conditions. Bladder cancer is the most common indication for cystectomy, and most patients who undergo radical cystectomy and urinary diversion have muscle-invasive or high-risk non-muscle-invasive bladder cancer. There are two major surgical approaches for urinary diversions performed after radical cystectomy: continent and incontinent diversions. For incontinent urinary diversions, a cutaneous ostomy is used for continuous urine

Intravesical Agents for Prevention of Recurrent Urinary Tract Infections.

Recurrent urinary tract infections (rUTIs) are common and can significantly impact a patient's quality of life. The mainstay of treatment is antibiotic-based, which contributes to the global problem of antibiotic resistance among urinary pathogens, so many alternative therapies have been explored. In this mini-review we evaluate evidence for intravesical agents that prevent rUTI, including antibiotic and nonantibiotic options. The current evidence supports the use of intravesical agents that replenish the glycosaminoglycan layer and of aminoglycoside antibiotics as safe and effective therapies

[Male urinary incontinence].

*Urinary incontinence in males is gaining increasingly more attention. *Male urinary incontinence can be classified as storage incontinence due to overactive bladder syndrome or stress incontinence due to urethral sphincter dysfunction. *Most patients benefit from the currently available treatment options for overactive bladder, which include physiotherapy, medication, botulinum A toxin injections and neuromodulation techniques. *The number of radical prostatectomies performed for prostate cancer is increasing; this intervention can lead to stress incontinence due to sphincter weakness. *Vario

Male Incontinence: The Etiology or Basis of Treatment.

CONTEXT: Elderly men are an increasing proportion of the aging population in the 21st century. Urinary incontinence reduces the quality of life and increases the burden of care for the aging population. OBJECTIVE: The primary objective of this review is to explore the etiopathology of common causes of incontinence in aging male patients. The focus is on the algorithm of the initial evaluation of these patients from both a primary care and a urologic standpoint. EVIDENCE ACQUISITION: A nonsystematic review of the literature was performed in September 2017. The data and evidence of this paper

Botulinum Neurotoxin Type A in paediatric non-neurogenic therapy resistant overactive bladder: a cohort study.

INTRODUCTION AND OBJECTIVE: Intradetrusor Botulinum Neurotoxin Type A (BoNT-A) is an increasingly applied treatment modality for overactive bladder (OAB) in children with refractory urinary incontinence. Despite that, evidence is sparse, and the potential not fully understood. The aim of this study was to evaluate the effectiveness and safety of intradetrusor injection in children with refractory functional OAB and urinary incontinence. Furthermore, we aimed to identify predictors of efficacy and side effects to BoNT-A treatment. MATERIALS AND METHODS: We conducted a cohort study of children

Urinary incontinence in women.

Urinary incontinence symptoms are highly prevalent among women, have a substantial effect on health-related quality of life and are associated with considerable personal and societal expenditure. Two main types are described: stress urinary incontinence, in which urine leaks in association with physical exertion, and urgency urinary incontinence, in which urine leaks in association with a sudden compelling desire to void. Women who experience both symptoms are considered as having mixed urinary incontinence. Research has revealed overlapping potential causes of incontinence, including dysfunct

Pelvic-floor musculature exercises in treatment of anatomical urinary stress incontinence.

The purpose of this study was to investigate the effects of pelvic-floor musculature exercises in the treatment of women with anatomical urinary stress incontinence. Fourteen female subjects, ranging in age from 33 to 67 years, participated in a four-week pelvic-floor musculature exercise program. Subjects underwent a pretraining urodynamic evaluation and were reexamined after four weeks of exercise training. Paired t tests of subjects' pretraining and posttraining values of bladder capacity, functional urethral length, and static urethral pressure profile showed no significant differences. Mu

A peripheral opioid antagonist for treating urinary retention induced by opioids: A case report.

BACKGROUND: Urinary retention is a poorly studied opioid-related adverse effect. There is a paucity of data regarding the treatment of such disturbance in patients with advanced cancer receiving opioids. ACTUAL CASE: A young man, without comorbidities, was receiving 30 mg/day of oxycodone for abdominal pain due to pancreatic cancer, unsuccessfully. He also complained of severe urinary retention that developed after initiation of opioid therapy. Methadone therapy was effective on pain intensity, but bladder dysfunction persisted. POSSIBLE COURSES OF ACTION: Only anedoctal experience exists fo

How to treat neurogenic bladder and sexual dysfunction after spinal cord lesion.

Neurogenic bladder and sexual dysfunction after spinal cord lesions are highly prevalent. The treatment algorithm for neurogenic bladder is well described. Clean intermittent self-catheterisation associated with treatment of neurogenic detrusor overactivity is the gold standard. Goals of treatment are twofold: i) control risk factors to avoid upper urinary tract complications, and ii) improve quality of life by treating incontinence when feasible. Lower urinary tract dysfunction is still a major cause of complications and hospitalisation. Sexual dysfunction must be addressed and treated and is

More Than Dysuria: Corynebacterial Encrusted Cystitis.

Corynebacterium species, typically considered contaminants in urine culture, can cause encrusted cystitis (EC), a form of chronic urinary tract infection causing pain, bladder necrosis, renal failure, and death. Delayed diagnosis is common due to its rarity and the fastidious nature of Corynebacterium urealyticum. Reported mostly in elderly and immunocompromised patients, EC is rare in pediatric patients. A female adolescent on high dose steroids developed persistent dysuria after urinary catheterization. Abnormal bladder ultrasound and characteristic cystoscopy led to the diagnosis of EC. App

"Far beyond the shape": an early head-to-head comparative urodynamic analysis of three contemporary totally intracorporeal robotic orthotopic neobladder techniques.

The expanding use of totally intracorporeal orthotopic neobladder (ICONB) reconstruction after robot-assisted radical cystectomy has fostered multiple configuration variants, yet robust comparative functional data remain limited. This early head-to-head study aimed to assess whether three contemporary ICONB configurations-Padua Ileal Neobladder, Florence Robotic Intracorporeal Neobladder (FloRIN), and a Y-shaped neobladder-deliver comparable urodynamic and continence outcomes despite anatomical nuances. Three prospectively maintained institutional cystectomy databases were merged. Patients who

Traumatic catheterisation: a near miss.

A 68-year-old man presented with acute urinary retention. An indwelling catheter was inserted by a junior doctor, which immediately caused perineal pain to the patient. When asked about the catheterisation technique, the junior doctor admitted that he did not insert the catheter to the hilt prior to inflating the balloon. The patient was investigated with a pelvic CT scan and a senior doctor in the emergency department interpreted that the catheter was inserted extravesically with possible haemorrhage. However, subsequent cystoscopy revealed no urethral trauma but a small bladder and a diverti
