A careful history points the urodynamic examination in the right direction and enables the examiner to ask the correct questions. The individual who does the test is the only reliable interpreter of the results of that study. No urodynamic technique is as sensitive or specific as a blood glucose, or even an electrocardiogram. A history of urgency and urge incontinence suggests uninhibited contractility and is a better index of that condition than a cystometrogram. Leakage occurring shortly after a previous operative procedure for stress incontinence suggests type III stress incontinence. A pas

A further series of 100 consecutive gauze hammock sling operations for urinary stress incontinence is presented with preliminary urodynamic assessment in all cases and follow-up from 6 months to 5 years. In 63 women the operation was a primary procedure. Modification to the operation included redesign of the shape of the sling, use of increased tension where minor degrees of bladder detrusor instability exist, and use of a suprapubic catheter postoperatively. Of the 100 women 87 were reviewed directly and 12 indirectly; only one was lost to follow-up. After operation, 78% were continent, 17% s

When an individual is unable to empty the bladder fully they are at risk of developing long-term bladder and renal damage. Urinary catheterization may be required if other treatments fail to resolve the problem. Urinary catheters can be indwelling or intermittent. Intermittent catheterization (IC) can be performed by healthcare professionals or carers. Intermittent self-catheterization (ISC) is performed by the person with bladder dysfunction and is considered to be the 'gold standard' in managing bladder dysfunction. ISC can spare the individual the unpleasantness of an indwelling catheter an

Bladder dysfunction is a common problem for patients with multiple sclerosis. The severity of symptoms often correlate with the degree of spinal cord involvement and, hence, the patient's general level of disability. The emphasis of management is now mainly medical and is increasingly offered by nonurologists. Treatments can be highly effective, relieving patients of what are otherwise very troublesome symptoms that would compound their neurological disability. This article gives an overview of the neural control of the bladder, followed by an explanation of the pathophysiology of detrusor ove

This paper reviews the possible mechanisms underlying bladder overactivity and discusses the targets for pharmacological treatment of this disorder. Damage to the brain (cerebrovascular disease, etc.) induces bladder overactivity by reducing suprapontine inhibition. Currently, attention has focused on C-fiber bladder afferents that may concern the mechanisms for bladder overactivity resulting from various etiologies such as spinal cord lesions, bladder outlet obstruction and bladder hypersensitivity disorders. With regard to the pathophysiology of idiopathic overactive bladder, both myogenic a

From 1988 to 1992, 78 patients with genuine stress urinary incontinence underwent bladder neck suspension under ultrasonic monitoring. Tightness of suspension was adjusted by setting posterior urethrovesical angle to approximately 90 degree by transrectal ultrasonography during operation. Urinary continence was achieved in 68 of 78 patients. In the remaining 10 patients, slight incontinence recurred within 6 months after operation. In 60 patients undergoing postoperative chain cystourethrogram, the posterior urethrovesical angles set during operation were about the same as those after operatio…

OBJECTIVE: To determine the accuracy of an analysis of symptoms alone for the diagnosis of genuine stress incontinence.
DESIGN: A comparison of results of symptoms analysis with urodynamic findings.
SETTING: A gynaecological video-urodynamic unit.
SUBJECTS: 252 consecutive patients referred for urodynamic investigations.
INTERVENTIONS: A questionnaire of 20 symptoms of lower urinary tract dysfunction, midstream specimen of urine, pad testing, uroflowmetry, and video-cystourethrography.
MAIN OUTCOME MEASURES: Using the urodynamic diagnosis as the 'gold standard', the accuracy of discrimina
