AIMS: Chronic heart failure (HF) is recognized as an important public health problem but little attention has been focused on acute-stage HF.
METHODS AND RESULTS: Nationwide, prospective, observational study setting 206 cardiology centres with intensive cardiac care units. During 3 months, 2807 patients diagnosed as having de novo acute HF (44%) or worsening chronic HF (56%) were enrolled. The effects of combined aortic valve replacement, coronary bypass surgery, and passive containment surgery in a patient with long-standing aortic regurgitation and marked ventricular dilatation are described. We describe a patient with a subacute inferior myocardial infarction who developed a pseudo-aneurysm more than 18 days after the acute event. This is an unusual case with three different complications of a myocardial infarction: Firstly, ventricular rupture is usually the result following transmural myocardial infarction without reperfusion. Data regarding cardiac resynchronization therapy (CRT) in pediatric patients are limited. Prospective studies have convincingly shown that sudden death in asymptomatic patients with AS is rare and prognosis is good as long as LV function, exercise tolerance and exercise hemodynamics remain normal. The time interval to development of LV dysfunction is not known. Therefore the initial evaluation should be confirmed after 3-6 months including echocardiography and exercise testing to identify patients with rapid progression, LV dysfunction and other high risk patients in whom surgery can be expected to improve prognosis even if the patient remains asymptomatic.

Mitral valve prolapse is a common condition often associated with mitral regurgitation. Intuitively, one would expect a positive correlation between the severity of mitral valve prolapse and the associated regurgitation. This assumption is overturned by an unusual case of functional ventricularisation of the left atrium due to severe mitral valve prolapse which paradoxically resulted in minimal regurgitation.

We evaluated patients with end-stage heart failure who have a high likelihood of response to cardiac resynchronization therapy (biventricular pacing). It appears that 20% of patients do not respond to this expensive therapy despite the use of selection criteria (dilated cardiomyopathy, heart failure, New York Heart Association class II or IV, left ventricular ejection fraction <35%, left bundle branch block, and QRS >120 ms). The presence of left ventricular dyssynchrony is needed to result in improvement after cardiac resynchronization therapy.
