[Out-of-hospital buffer therapy in heart arrest].

The effects of infusing a buffer solution on resuscitability and outcome were tested in patients during out-of-hospital cardiac arrest. 502 adults with ventricular fibrillation or asystole with failure of first attempt at defibrillation were entered into a prospective, randomized, double-blind, controlled trial where one group received buffer and the other group placebo (saline). 87 of 245 (36%) patients who received a buffer were admitted to hospital and 24 (10%) were discharged alive, as against 92 of 257 (36%) admitted to hospital and 35 (14%) discharged alive for those who received placebo…

[Cardiovascular complications of interferon therapy in chronic hepatitis C].

Severe cardiovascular complications (myocardial infarction, ventricular fibrillation et al.) have been reported in patients who have been administrated extremely high dose interferon (IFN) or cardio-toxic drugs, or who have had cardiovascular diseases. But cardiomyopathy, myocarditis and atrioventricular block were reported in patients who administered low dose IFN and no cardio-toxic drugs or had no cardiovascular diseases. We report here cardiovascular complications during IFN therapy for chronic hepatitis C. Cardiovascular complications that necessitated stopping IFN administration in 643 t…

How to detect and treat arrhythmias in chronic lung disease.

Investigators suspect that arrhythmias are behind the high incidence of sudden death in patients with severe chronic bronchitis and emphysema. Continuous electrocardiographic monitoring may help boost survival statistics, since it has repeatedly shown its ability to detect irregularities that standard ECGs miss. So-called benign arrhythmias, such as atrial fibrillation, frequent ventricular premature contractions, and atrial flutter, are anything but harmless. In patients whose heart and lung function is already impaired by severe airway obstruction, these rhythm disorders can rapidly progress

[Incidence and relevance of tricuspid-valve insufficiency in acquired mitral-valve defect. Analysis based on right ventricular angiograms].

To detect tricuspid incompetence (TI) right ventricular angiography was performed in 167 patients suffering from moderate to severe mitral valve disease. Holosystolic reflux of contrast medium to the right atrium through the central part of the tricuspid valve was thought to represent true TI, whereas a jet of contrast medium following the injection catheter and originating from the region of its valve passage was assumed to reflect arteficial regurgitation. True TI was found in 35% of the total group (30% mild to moderate, 5% severe TI). TI was often accompanied by atrial fibrillation (91%),

Takotsubo cardiomyopathy in amiodarone-induced hyperthyroidism.

SUMMARY: Takotsubo cardiomyopathy (TC) is an atypical, severe but reversible form of acute heart insufficiency. It typically presents with left ventricular failure, transient apical and mid-segments hypokinesis, absence of significant coronary stenosis and new electrographic abnormalities and/or elevation in serum cardiac enzymes. Although TC ('broken heart syndrome') has classically been associated with emotional trauma, evidence suggests that other precipitants might exist, including iatrogenic and thyroid-mediated forms. Thyroid disease is a relatively common comorbidity in TC patients. We

Characterization of external defibrillator output and its impact on defibrillation protection of medical equipment.

BACKGROUND: Defibrillators constitute a critical component of emergency care for ventricular fibrillation and pulseless ventricular tachycardia. Advances in defibrillation technology point to the potential need for reassessing current requirements for defibrillation protection and associated resuscitation guidelines. OBJECTIVE: This study aimed to establish reference information for future directions in defibrillator requirements, defibrillation protection requirements, and resuscitation guidelines based on the parameters of commercially available defibrillators. METHODS: Defibrillator brand…

Acute Liver Failure Associated With Intravenous Amiodarone: A Case Report.

Amiodarone is a widely used class III antiarrhythmic agent that is indicated for the management of supraventricular and ventricular arrhythmias. Although mild elevations in liver enzymes are relatively common during therapy, severe hepatotoxicity and acute liver failure remain rare, yet potentially fatal complications, particularly when following intravenous administration. We report the case of an 80-year-old male who presented with palpitations and was diagnosed with atrial fibrillation with rapid ventricular response. Intravenous amiodarone was initiated for rhythm control. Within 14 hours,

Influence of Cardiac Resynchronization Therapy in Patients With Left Ventricular Assist Device.

A significant number of patients with left ventricular assist devices (LVAD) are also treated with cardiac resynchronization therapy (CRT). However, the effects of CRT on LVAD-recipient survival and ventricular arrhythmias (VAs) remain uncertain. To address this issue, we analyzed patients from the multicenter ASSIST-ICD registry using a 1:1 propensity score-matched cohort to evaluate the association between CRT and all-cause mortality. Secondary outcomes included cardiovascular/noncardiovascular mortality, and early (≤30 days postimplant) or late VAs. Among 652 LVAD recipients, 198 patients (

Gene Therapy for Cardiac Arrhythmias: Mechanisms, Modalities and Therapeutic Applications.

Cardiac arrhythmias remain a major source of morbidity and mortality, often stemming from molecular and structural abnormalities that are insufficiently addressed by current pharmacologic and interventional therapies. Gene therapy has emerged as a transformative approach, offering precise and durable interventions that directly target the arrhythmogenic substrate. Across the spectrum of inherited and acquired arrhythmias-including long QT syndrome, Brugada syndrome, catecholaminergic polymorphic ventricular tachycardia, atrial fibrillation, and post-infarction ventricular tachycardia-gene-base

A Case of Success: Guidelines-Based Treatment to Control Atrial Fibrillation-Induced Cardiomyopathy-Atrioventricular Node Ablation and Cardiac Resynchronization Therapy to the Rescue.

Background and Clinical Significance: Heart failure with reduced ejection fraction (HFrEF) and atrial fibrillation (AF) frequently coexist, creating a complex clinical interplay that exacerbates morbidity and mortality. AF can directly precipitate or worsen HFrEF through mechanisms such as tachycardia-induced cardiomyopathy, loss of atrial contribution to ventricular filling, and irregular ventricular response. The use of evidence-based therapies improves clinical outcomes in patients with HFrEF. Case Presentation: We present a clinical case of a 58-year-old man with left bundle branch block (

Emergency zero-fluoroscopy catheter ablation for refractory ventricular tachycardia in third-trimester pregnancy: a case report.

BACKGROUND: The incidence of symptomatic arrhythmias during pregnancy has markedly increased in recent years. Atrial fibrillation/flutter and life-threatening ventricular arrhythmias are of particular concern due to their association with elevated maternal and fetal risks. Cardioversion, antiarrhythmic drugs, or catheter ablation are treatment options for hemodynamically unstable arrhythmias. However, indications of therapeutic options and standardized protocols remain scarce due to the limited shared clinical experiences and evidence. CASE SUMMARY: On 17 June 2025, our team managed a 32-week

Termination of figure-of-eight reentry via resonant feedback pacing.

Sudden cardiac death (SCD) is often precipitated by reentrant arrhythmias such as ventricular tachycardia (VT) and ventricular fibrillation (VF), whose underlying dynamics are frequently sustained by spiral waves of electrical activity. Disrupting these waves can restore normal rhythm, but conventional low-energy pacing strategies are often ineffective in VF, where high-frequency, multi-wave interactions dominate. Resonant feedback-controlled antitachycardia pacing (rF-ATP), which times global electrical stimuli based on real-time feedback from the tissue, has been shown to robustly terminate …

A rare case of left ventricular cardiac rupture in the acute phase of takotsubo syndrome.

UNLABELLED: Takotsubo syndrome (TTS), often triggered by emotional or physical stress, is characterized by transient left ventricular dysfunction that mimics acute coronary syndrome. We present a case involving an 85-year-old woman with a history of atrial fibrillation who developed sudden severe chest pain during sleep. Initial evaluation suggested ST elevation myocardial infarction; however, emergency coronary angiography revealed patent coronary arteries, leading to a diagnosis of TTS. Immediately after angiography, the patient experienced sudden loss of consciousness, which resulted in dea

Anomalous Right Coronary Artery in the Setting of Active Tuberculosis: A Multidisciplinary Management Challenge.

Anomalous origin of the right coronary artery from the pulmonary artery (ARCAPA) is a rare congenital coronary anomaly, with an uncertain prevalence and often diagnosed incidentally. This case report presents a 62-year-old male with ARCAPA diagnosed during an evaluation for chest surgery. The patient had a history of colon cancer and active tuberculosis, complicating the clinical management. He reported chest pain, shortness of breath, and palpitations, with atrial fibrillation observed on a 24 h Holter ECG. Coronary angiography revealed robust collateral circulation and a suspected anomalous
