[A man with severe traumatic brain injury].

We present a 41-year-old man with severe traumatic brain injury. Cranial imaging studies revealed cerebral contusion and a longitudinal fracture of the temporal bone. Several days later brain herniated into the left external auditory canal. Imaging studies showed the known skull fracture with a direct connection between the external acoustic meatus and the intracranial structures.

Neuromuscular hamartoma of the cochlear nerve: a rare occurrence in the internal auditory meatus.

The authors present a case of neuromuscular hamartoma of the cochlear nerve, an unusual occurrence in the internal auditory meatus (IAM). A review of the literature shows no previous report of neuromuscular hamartoma of the cochlear nerve. This tumour was clinically and radiologically difficult to distinguish from acoustic neuroma. It is important to consider the diagnosis of these rare small tumours pre-operatively, as it may be appropriate to manage this conservatively.

Posterior fossa meningiomas intimately involved with the endolymphatic sac.

Approximately 10% of all intracranial meningiomas occur in the cerebellopontine angle (CPA). Although their clinical presentation depends on their site of origin, most are first seen with nonspecific otologic symptoms like those of acoustic neuroma. We present two cases of CPA meningioma that were intimately involved with the endolymphatic sac and had symptoms and signs suggestive of endolymphatic hydrops.

Audiological evaluation.

This chapter describes the basic audiological tests used in otoneurological evaluation. The interpretation of pure tone and speech audiometry in the discrimination of specific disorders caused by lesions in the middle ear, cochlea, or auditory nervous system is described. The value of the acoustic middle ear reflex and brainstem auditory evoked potentials in the differential diagnosis of disorders affecting the cochlea and auditory nervous system is discussed.

Digital phonoarthrometry of temporomandibular joint sounds: a preliminary report.

A new technique is presented for recording and evaluating the dysfunctional TMJ sounds on an objective basis. The physical implications related to TMJ phonoarthrometry (PAM) are discussed and three cases are presented. The phonoarthrography and the digital phonoarthrometry presented in this paper can be a useful harmless diagnostic tool. Differential diagnosis of TMJ pathology is possible on the basis of an acoustical analysis and it is hoped that future investigation will confirm these observations.

Clinical neurophysiological examination of deafness associated with juvenile motor neurone disease.

Three patients with the syndrome of progressive severe deafness, associated with juvenile onset of motor neurone disease, were examined using conventional audiometry and brainstem auditory evoked response (BAER) recordings, and electrocochleographic recordings were made in two of them. Findings indicate that the deafness had resulted from loss of acoustic nerve fibers and/or sensory cells of the spiral ganglion.

The aging ear.

Otosclerosis begins in the second and third decades of life but usually does not produce a hearing loss until after the fourth decade. Chalky white plaques in the tympanic membrane may be benign, but when associated with hearing loss may point to tympanosclerosis. Presbycusis is the most common cause of hearing loss. Unilateral hearing loss raises the specter of acoustic neuroma.

[A case of paroxysmal tinnitus and nystagmus accompanied by facial spasm].

Trigeminal neuralgia and facial spasm are usually caused by cross vascular compression of the trigeminal root entry zone and facial nerve exit zone. A similar mechanism is believed to affect the acoustic nerve as well, giving rise to tinnitus and nystagmus. We present one case of paroxysmal tinnitus and nystagmus, accompanied by facial spasm, and discuss the mechanism and the significance of the neurotological examination.

[Contribution of EMG to the diagnosis of facial nerve and muscle involvement in neurinoma of the VIIIth nerve].

An examination of 5 cases of acoustic nerve neurinoma, and 3 cases where the diagnosis was suspected, showed that EMG signs of a possible facial nerve compression in the ponto-cerebellar angle were represented by abnormal activity at rest, sub-clinical signs of progressive neurogenic atrophy in the hemi-facial muscles, and a significant increase in the latency of the oligosynaptic R1 reflex response.

Computer processing of audiological and vestibular data. II. A further note.

This paper provides an addendum to an earlier paper describing the development of a computer record system for patient data. The specific problems addressed pertain to the storage and retrieval of historical information, physical signs and diagnosis. Some preliminary comparisons of audiological and vestibular test results are given for groups of patients with diagnoses of acoustic neuroma, Ménière's disease, temporal bone fracture and vestibular neuronitis.

Influence of static middle ear pressure on transiently evoked otoacoustic emissions and distortion products.

Otoacoustic emissions (OAE) are influenced in their amplitude and frequency spectra by the middle ear. The effects of changes in the middle ear transmission mechanisms on transiently evoked OAE (TEOAE) and distortion product emissions (DPOAE) were investigated as a function of static ear canal pressure in 25 normal-hearing test persons aged 18-35 years. The ear canal pressure was varied stepwise between positive and negative values of 200 daPa. TEOAE and DPOAE amplitudes were attenuated significantly with changes of the static ear canal pressure, with greatest changes at low frequencies (< 2 k

Sound perception induced by extracranial magnetic stimulation in deaf patients.

Two profoundly hard-of-hearing and deaf patients were examined by non-invasive extracranial magnetic stimulation (EMS) in an effort to determine whether EMS could evoke auditory sensations. The patients were fitted with standard earplugs and were stimulated at the auricle, the mastoid and the temporal lobe area. The threshold of auditory sensation (TAS) was determined at each stimulus position and found to be approximately 20-40% of the maximum EMS level (2.0 Tesla). The TAS was generally lowest in mastoid stimulation, but was variable, and dependent on the angle and position of the stimulatin

The effect of auditory stimulation on the tensor tympani in patients following stapedectomy.

CONCLUSION: We believe that a tensor tympani reflex, in response to loud sound, is present in a minority of people, although its functional significance with regard to sound transmission is questionable. The absence of startle response in our stapedectomized subjects makes us question whether the tensor component of the startle response is large enough to be identified by manometry. OBJECTIVES: This study was organized to examine reflex activity to stimulation by loud sound or by startle in the tensor tympani. Although many previous studies have been carried out, results have been contradicto

Bilateral clicking tinnitus due to myoclonus of middle ear muscles.

Isolated middle ear myoclonus can be a cause of objective tinnitus. We present an acoustically documented case of irregular bilateral middle ear myoclonus with loud clicking, and roaring tinnitus associated with essential palatal tremor. A palatal botulinum toxin injection did not eliminate the tinnitus. Division of both middle ear tendons in both ears abolished the clicking tinnitus with no effect on hearing.

Internal Auditory Canal Tumor as an Incidental Finding on a Choline PET/CT Scan in a Prostate Cancer Patient.

An 84-year-old man with a history of prostate cancer, treated with radiotherapy, presented with an increase in PSA levels. F-choline PET/CT showed foci of increased choline uptake in L4 and L5 vertebrae, suggestive of bone metastases and another focus in the right cerebellopontine angle. A brain MRI revealed a focus of intense contrast enhancement in the same region, consistent with an acoustic neuroma.

Audit of regional screening guidelines for vestibular schwannoma.

An audit of 334 patients who underwent magnetic resonance imaging (MRI) as investigation for a possible diagnosis of vestibular schwannoma was carried out to assess adherence to previously agreed screening guidelines. This represents one year's activity. A posterior fossa tumour was identified in 12 patients. Scan requests were judged to be inappropriate for 28 cases. The issues surrounding the screening for acoustic neuroma are discussed.

Brainstem auditory evoked responses in rats with experimental chronic renal failure.

1. Chronic renal failure was induced in rats by five-sixths nephrectomy. Brainstem auditory evoked response (BAER) was recorded after 3 months. 2. In the uraemic rats latency of the first wave was delayed, while the interpeak I-V latency was similar to that of the controls. 3. These results suggest a delayed neural conduction along the acoustic nerve or cochlear changes in uraemic rats.

Intraoperative sonography during lumbar laminectomy: work in progress.

The feasibility of intraoperative sonography during lumbar laminectomy was demonstrated in six cases. The most obvious advantage was the ability to image the spinal canal anterior to the spinal cord or cauda equina, which cannot be seen directly from the posterior operative approach. A water bath of warm saline serves as an ideal acoustic window. This technique has the potential to be a significant contribution to intraoperative exploration and localization.

Late onset of hearing in the ferret.

Onset of hearing in the ferret was judged by simple behavioral, physiological and anatomical indices. The ear canals do not open until the end of the first postnatal month. This coincides with the appearance of a startle response to loud hand claps and the recording of acoustically activated neurons in the midbrain. The late onset of hearing in the ferret (around 32 days postnatal) contrasts with the cat (6 days) and the mouse (12 days).
