FAAH rs324420 Polymorphism in People With Opioid and Methamphetamine Use Disorders: Psychological Associations.

PURPOSE: The global increase in the prevalence of substance use disorders (SUD) has resulted in higher mortality rates. While heroin use has decreased or stabilized, methamphetamine use is increasing, and the combined use of opioids and methamphetamine is becoming more common. FAAH is an enzyme that hydrolyzes endocannabinoid substances and plays a role in neurobehavioral processes. The aim of this study was to investigate the effect of the FAAH rs324420 polymorphism on SUD. METHOD: The study included 562 individuals who used methamphetamine (MUD, n = 148), opioids (OUD, n = 114), or both met…

Patient characteristics associated with receipt of contingency management for stimulant use disorder in the Veterans Health Administration.

BACKGROUND: Contingency management (CM) is the most effective available treatment for stimulant use disorder (StUD). The Department of Veterans Affairs (VA) rolled out the largest national CM implementation effort to date, but ongoing efforts are needed to increase CM receipt. To inform these efforts, it is important to understand the reach of CM implementation across patient characteristics. This exploratory analysis examined whether patient characteristics are associated with CM receipt among VA patients with StUD. METHODS: We extracted electronic health record data for patients with diagno

Factors associated with placement along the HIV prevention and care continuum among sexual minority men who use methamphetamine.

Methamphetamine use among sexual minority men (SMM) has been associated with poor ART adherence, and reduced initiation and adherence to PrEP. From May 2021 to May 2023, 226 SMM were enrolled in Getting Off, a culturally responsive smartphone application to reduce methamphetamine use and improve sexual health. Using a status-neutral approach, an ordinal variable reflected participants' placement on the HIV Prevention/Care Continuum, from HIV-positive, not taking ART, to HIV-negative, currently taking PrEP. An ordinal logistic model was structured to identify factors associated with participant…

mHealth Incentivized Adherence Plus Patient Navigation (MIAPP): protocol for a pilot randomized controlled trial to improve linkage and retention on buprenorphine for hospitalized patients with methamphetamine use and opioid use disorder.

BACKGROUND: Initiation of buprenorphine for treatment of opioid use disorder (OUD) in acute care settings improves access and outcomes, however patients who use methamphetamine are less likely to link to ongoing treatment. We describe the intervention and design from a pilot randomized controlled trial of an intervention to increase linkage to and retention in outpatient buprenorphine services for patients with OUD and methamphetamine use who initiate buprenorphine in the hospital. METHODS: The study is a two-arm pilot randomized controlled trial (N = 40) comparing the mHealth Incentivized Ad…

Baseline Moderators of Response to Naltrexone and Bupropion in the Treatment of Methamphetamine Use Disorder: An ADAPT-2 Study.

OBJECTIVES: Extended-release naltrexone and once-daily bupropion (NTX-BUP) reduced methamphetamine (MA) use in the Accelerated Development of Additive Pharmacotherapy Treatment for Methamphetamine Use Disorder (ADAPT-1, ADAPT-2) trials. Using a proof-of-concept machine learning technique, we created a classification rule using baseline characteristics to identify individuals who would benefit more from NTX-BUP than from placebo (NTX-BUP-preferred). METHODS: ADAPT-2 (double-blind randomized-controlled sequential parallel comparison design trial, n=147) and ADAPT-1 (open-label pilot study, n=37…

Challenges recruiting and retaining people at risk for HIV who use methamphetamine in a randomized PrEP adherence trial in Seattle, WA.

BACKGROUND: Methamphetamine is associated with increased HIV risk and suboptimal adherence to pre-exposure prophylaxis (PrEP). Interventions to support PrEP adherence for people who use methamphetamine are needed. METHODS: We evaluated peer navigation to support adherence among people initiating PrEP who use methamphetamine. The HIV Prevention in Methamphetamine Users (HMU!) study enrolled participants from May 2018-January 2022 in Seattle, WA, and followed them for 6 months. Surveys collected sociodemographic, drug use, and sexual behavior data at enrollment, month 3, and month 6. Dried bloo…

The exploration of optimized protocol for repetitive transcranial magnetic stimulation in the treatment of methamphetamine use disorder: A randomized sham-controlled study.

BACKGROUND: The prefrontal-striatal circuit is a core circuit related to substance dependence. Previous studies have found that repetitive transcranial magnetic stimulation (rTMS) targeting the dorsolateral prefrontal cortex (DLPFC) (key region of executive network) had limited responses, while inhibiting hyperactivation of ventromedial prefrontal cortex (vmPFC) (key region of limbic network) may be another strategy. However, there is currently no comparison between these two treatment locations. METHODS: Seventy-four methamphetamine-dependent patients were randomly assigned to one of treatme…

Investigating the sequential patterns of methamphetamine use initiation in Iran.

BACKGROUND: Methamphetamine (MA) remains one of the most commonly used amphetamine-type stimulants, accounting for the second most widely-used substance after marijuana. Due to increased use of MA, a wide variety of research has focused on the patterns of MA use initiation among adolescents. Nevertheless, there are few data available for people who use MA. The present study set out to assess the sequential patterns of substance use initiation in patients with MA use disorders in Iran. MATERIALS AND METHODS: This cross-sectional study described substance initiation patterns for 302 patients wh…

Executive function moderates naltrexone effects on methamphetamine-induced craving and subjective responses.

Background: Emerging evidence suggests that opioid receptor antagonists, such as naltrexone, are effective pharmacotherapies for alcohol, opioid, and possibly stimulant use disorders. It is posited that naltrexone exerts its effects, in part, by increasing functional connectivity between neural reward circuitry and frontal systems implicated in executive function. Yet no studies had examined whether executive function moderates these effects. Objectives: This study examined whether a composite measure of executive function (EF) moderates the effect of naltrexone on craving for methamphetamine …

Sex influences susceptibility to methamphetamine cardiomyopathy in mice.

In this study, we created a mouse model of methamphetamine cardiomyopathy that reproduces the chronic, progressive dosing commonly encountered in addicted subjects. We gradually increased the quantity of methamphetamine given to C57Bl/6 mice from 5 to 40 mg/kg over 2 or 5 months during two study periods. At the fifth month, heart weight was increased, echocardiograms showed a dilated cardiomyopathy and survival was lower in males, with less effect in females. Interestingly, these findings correspond to previous observations in human patients, suggesting greater male susceptibility to the effec…

Methamphetamine Use: A New Wave in the Opioid Crisis?

BACKGROUND: The ongoing drug crisis in the United States continues to be headlined with numbers of deaths related to opioid overdose. Less known to the public and health care providers is the rise in methamphetamine use, often in conjunction with opioids or adulterated with fentanyl. An old practice with a new twist is the use of methamphetamine in conjunction with an opioid such as heroin. PURPOSE: Although there are no Food and Drug Administration-approved medications to treat individuals with stimulant use disorders, a review of available studies suggests a few promising medications that m…

Effect of methamphetamine on the fasting blood glucose in methamphetamine abusers.

Methamphetamine is a popular psychostimulant worldwide which causes neurotoxicity and neuroinflammation. Although previous studies have characterized potential associations between addictive drugs and fasting blood glucose, the influence of methamphetamine on the blood glucose is still largely unknown. The present study was designed to investigate the change of fasting blood glucose of methamphetamine abusers and to confirm the impairment of liver and kidney. Fasting blood glucose was significantly decreased in methamphetamine abusers and in a high-fat diet mouse model with methamphetamine tre…
