Organophosphorus (OP) compounds remain a leading cause of self-poisoning and mortality, especially in South East Asia, China, and Africa. Organophosphorus causes an acute cholinergic syndrome by inhibiting acetylcholinesterase. Atropine remains the mainstay of treatment, but recently some promising therapies are in the pipeline. Oximes are used widely in the management of organophosphorus poisoning, however clinical efficacy remains to be established. Magnesium sulfate, calcium channel blockers (nimodipine), plasma alkalinizing agents, β-2 agonists, nicotinic receptor antagonists, clonidine, a…

Fasciolopsis buski is the largest fluke parasitizing the human small intestine. F. buski infections are not uncommon in Southeast Asia. The risk factors of F. buski infection mainly include eating of raw aquatic crops and infected snails. Most infections are asymptomatic. Heavy infection can be fatal as the flukes cause extensive intestinal inflammation, intestinal perforation, small bowel stricture, ulceration, hemorrhage, and abscess formation. Endoscopic diagnosis of this parasite has been described in a few case reports. Here, we describe and illustrate the endoscopic removal of F. buski f…

CME: Typhoid Fever - Clinical Manifestation, Diagnosis, Therapy and Prevention Abstract. Thypoid fever is rare in Western countries. It is, however, among the most common etiologies for febrile illness in the traveller returning from tropical areas (especially South(east) Asia and Sub-Saharan Africa). There are several signs that have been described as classical findings in typhoid fever: i) febrile temperatures with relative bradycardia, ii) eosinopenia, iii) slow defervescence, and iv) systemic manifestations (e.g. hepatitis). Diagnosis is confirmed by positive blood cultures. Pretravel vacc…

Japanese encephalitis (JE) is one of severe viral encephalitis that affects individuals in Asia, western Pacific countries, and northern Australia. Although 67,900 JE cases have been estimated among 24 JE epidemic countries annually, only 10,426 have been reported in 2011. With the establishment of JE surveillance and vaccine use in some countries, the JE incidence rate has decreased; however, serious outbreaks still occur. Understanding JE epidemics and identifying the circulating JE virus genotypes will improve JE prevention and control. This review summarizes the current epidemiology data i…

Malaria is the most important parasitic disease with global concern. Plasmodium knowlesi recently has emerged from its natural simian host as a significant cause of human malaria, particularly in Malaysian Borneo. Therefore, it has been added as the fifth human Plasmodium specie which is widely distributed in Southeast Asia. Recent developments of new molecular tools enhanced our understanding about the key features of this malaria parasite. Here, we review some of the ways in which molecular approaches might be used for epidemiology of P. knowlesi and finally lead to an efficient control of m…

Russell's viper envenomation and its related complications, especially acute kidney injury, is an important cause of morbidity and mortality in tropical developing countries of South Asia. Unusual complications, especially hypopituitarism, are rare and probably missed due to lack of clinical suspicion and diagnostic facilities. We report a rare presentation of growth retardation resulting from hypopituitarism due to Russell's viper envenomation along with central diabetes insipidus. Awareness of the fact that hypopituitarism may occur in this clinical setting is necessary for early diagnosis a…

Recurrent pyogenic cholangitis is endemic to South-east Asia but has been very rarely reported from natives of other parts of the world. A 43-years-old woman was presented with sepsis that had a history of recurrent epigastric pain and fever attacks. Her liver tests were unremarkable suggesting any hepatobiliary diseases. Recurrent pyogenic cholangitis and congenital extrahepatic biliary anomaly have been diagnosed after serial diagnostic and therapeutic procedures including endoscopic retrograde cholangiography, MR-cholangiography, percutaneous transhepatic cholangiography and finally left he…

Reduction malarplasty for making an oval facial shape is a popular procedure in Asia. However, a range of complications after malarplasty have been reported, including hematoma, orbital complications, asymmetric face, and depression of the chin. Fixation can be incomplete and the osteotome can penetrate the sinus during surgery due to the narrow surgical field, which can result in sinusitis caused by bony fragments after malarplasty. However, there are no reports of sinusitis caused by bony fragments after malarplasty. The authors report 2 cases of chronic sinusitis caused by displaced bony fr…

Melioidosis is endemic in Southeast Asia and northern Australia, but it has been rarely reported from India. Recent reports have shown that melioidosis is an emerging infection in this part of the world, but enteric fever is more commonly seen in India. We present a 50-year-old male with diabetes who presented with acute onset of febrile illness. Preliminary investigations were suggestive of enteric fever, and he showed a partial response to parenteral ceftriaxone; however, it later turned out that he had melioidosis. The widal titres were persistently elevated even following treatment with me…

A 42-year-old man with T-4 ASIA B thoracic paraplegia presented with chronic constipation, fecal incontinence, and bowel-related autonomic dysreflexia (AD) refractory to conservative bowel program. His usual toileting time lasted one to five hours. Antegrade continence enemas (ACE) were performed. The ACE technique, which requires creating a continent catheterizable appendicocecostomy, is described. Postoperatively, a daily enema was given through the stoma. The patient's toileting time was reduced to 20 minutes and his AD resolved. The role of the ACE is discussed in adult spinal cord injury …

Since the 1908s gnathostomiasis has occurred sporadically in Japanese who have eaten loaches, fresh-water fish imported from southeast Asia. Symptoms include creeping skin eruptions and eosinophilia. The symptoms disappeared in most patients within three months after the onset and the patients then remained symptom-free. To elucidate the causative factors, we fed one monkey with 80 gnathostome larvae collected from loaches and autopsied the animal 466 days later. Fifty-one viable larvae (64%) were recovered from muscle; these were in the advanced third stage and were encapsulated.

A strain of Shigella dysenteriae type 1 resistant to ampicillin, chloramphenicol, streptomycin, sulphonamides, and tetracyclines has caused an epidemic of shiga dysentery in northeast Zaire. Since November 1979, it has caused many deaths in an extensive area which now includes neighbouring Rwanda. The strain carries a resistance plasmid of compatibility group X, indistinguishable from the plasmid previously found in strains from Somalia. This contrasts with multiply resistant strains of Sh. dystenteriae type 1 from Central America and South-East Asia in which resistance is determined by plasmi…

BACKGROUND AND METHODS: The evolutionary divergence of the A(H5) hemagglutinin (HA) gene of high pathogenicity avian influenza (HPAI) viruses (A/goose/Guangdong/96 lineage) was analyzed by phylogenetic and average pairwise distance methods to identify clades that merit nomenclature updates.
RESULTS AND CONCLUSIONS: Based on this assessment, 12 new clade designations were recommended based on division of clade 2.3.2.1c and 2.3.4.4 viruses, which were reported in Africa, Antarctica, Asia, Europe, the Middle East, the Americas, and Oceania since the most recent WHO/WOAH/FAO H5 Evolution Working …

We report a case of necrotizing pneumonia caused by hypervirulent Klebsiella pneumoniae in an athletic, 54-year-old apnea diver. Since hypervirulent Klebsiella pneumoniae strains prevail in Southeast Asia and are mainly associated with "invasive liver abscess syndrome", we considered and tested for hypervirulence only when the necrotizing pneumonia turned out to be refractory to initial antibiotic treatment. Although the hypervirulent Klebsiella pneumoniae strain in our patient did not show multi-drug resistance, a total of 6 months of high-dose antibiotic therapy was necessary to cure the inf…

INTRODUCTION: Pain management in patients with complete spinal cord injury is complex.
CASE REPORT: We report a successful case of managing neuropathic, phantom limb, and back pain below the level of spinal cord injury (T5 American Spinal Injury Association [ASIA] A) using a 10 kHz high-frequency spinal cord stimulator (SCS) over a 6-month follow-up period.
CONCLUSION: The effectiveness of this approach may be attributed to its ability to modulate supraspinal pain processing, allowing for targeted relief of various pain mechanisms below the level of injury.

Haemoglobin E (HbE) affects at least 1 million people around the world. The carrier frequency of HbE/beta-thalassaemia (HbE/β-thalassaemia) is highest in Southeast Asia. In India, the highest frequency is observed in the northeast region. Distinguishing between homozygous HbE disease and HbE/β-thalassaemia is a challenge to the haematopathologist as well as to the treating obstetrician because both are clinically and haematologically similar, posing a difficulty in managing anaemia and assessing the fetal risk for the same disease. This article reports a case of compound heterozygote HbE/β-tha…

Tick Borne Encephalitis (TBE) is endemic to an increasing number of countries and is a common cause of meningoencephalitis in Europe and Asia making any potential complications of the disease increasingly relevant to clinicians. We present, what is to our knowledge, the second reported case of N-methyl-d-aspartate receptor (NMDAR) encephalitis following Tick Borne Encephalitis (TBE) in a 47-year-old Lithuanian man. The case provides further evidence of TBE being a possible trigger of NMDAR encephalitis and highlights the importance of being aware of symptoms of autoimmune encephalitis in patie…

Typhoid fever is an infectious febrile illness caused by Salmonella typhi that is rare in the United States but is endemic in regions of South Asia and Africa. Typhoid fever initially presents with nonspecific symptoms such as fever, malaise, and abdominal pain. We describe a case of typhoid fever in an adult in the United States with recent travel to Mexico. After a nonspecific presentation, the patient developed Faget sign and computed tomography (CT) of the abdomen revealed mesenteric adenitis, which prompted additional workup. Diagnosis of typhoid fever was established by blood culture and…

Melioidosis is a pyogenic infection that is potentially fatal, caused by the bacterium Burkholderia pseudomallei, which is commonly a soilborne pathogen. It is endemic in the Indian subcontinent, northern Australia, and Southeast Asia. Melioidosis has a wide spectrum of clinical manifestations that can mimic various diseases. Septic arthritis is a rare but well-known clinical presentation. Here, we report a case of an adult presenting with acute knee joint pain and swelling. He was subsequently found to have septic arthritis with other system involvement and diagnosed as a case of melioidosis; …

The past year has underscored the threat that emerging viruses pose to global health. The 2021 John Dirks Canada Gairdner Global Health award recognizes the contributions of Joseph Sriyal Malik Peiris and Yi Guan toward understanding the origins and options for control of newly emerging infectious disease outbreaks in Asia, notably zoonotic influenza and severe acute respiratory syndrome (SARS). Nicole Neuman of Cell corresponded with Malik Peiris about his path to studying emerging infectious diseases and the challenges of this work. Excerpts of their exchange are included here.
