A prospective review of transfusion practices in paediatric trauma patients at level 1 trauma centre.

BACKGROUND AND OBJECTIVES: Transfusion practices in paediatric trauma patients are frequently based on adult protocols, despite notable differences in physiology and injury mechanisms between children and adults. This study was aimed to evaluate the current transfusion practices, among the paediatric trauma population, which will help in guiding management and planning along with research opportunities among paediatric trauma patients MATERIALS AND METHODS: We conducted a prospective observational study over one year, including trauma patients under 18 years who received transfusions. Data col

The carbon footprint of group and save in elective and emergency surgery.

INTRODUCTION: Climate change is a significant threat to human health, and surgical care is a major contributor to the carbon footprint of hospital medicine. There is wide variation in perioperative group and save (G&S) blood testing that lacks an evidence base. Eliminating low-value clinical investigations in surgical pathways such as the G&S could lead to significant carbon and cost savings. METHODS: All operations within the trust over a 6-month period and all packed red cell requests made within the same timeframe were analysed retrospectively. Patients were categorised by operation and cr

Expanding restrictive transfusion evidence in surgical practice: a multicentre, prospective cohort study.

BACKGROUND: Findings of observational studies investigating the impact of transfusions are at odds with those of randomised controlled trials, raising concern that observational studies may be inappropriate to inform transfusion decisions. We examined whether observational data could replicate evidence from randomised controlled trials on restrictive transfusion in cardiac and orthopaedic surgery, and be generalised to broader specialties as well as to a lower haemoglobin transfusion threshold (7 g/dL). MATERIAL AND METHODS: A multicentre, prospective cohort study was performed at three repre

Diagnosis and treatment of cardiac iron overload in transfusion-dependent thalassemia patients.

Thalassemia is among the most common genetic diseases. Patients with severe forms of the disease are transfusion-dependent, leading to iron overload. A condition which can eventually develop in the iron-loaded heart is iron overload cardiomyopathy, a debilitating disease that accounts for the majority of deaths in thalassemia patients. Areas covered: This review article provides a comprehensive summary of the diagnosis and treatment of cardiac iron overload in transfusion-dependent thalassemia patients, with discussion covering current weak points and potential improvements of the relevant dia

Intrauterine transfusion combined with partial exchange transfusion for twin anemia polycythemia sequence: modeling a novel technique.

INTRODUCTION: Twin anemia-polycythemia sequence (TAPS) is a newly described disease in monochorionic twin pregnancies, characterized by large inter-twin hemoglobin differences. Optimal management for TAPS is not clear. One of the possible treatment modalities is intrauterine blood transfusion (IUT) in the donor with or without combination of partial exchange transfusion (PET) in the recipient. METHODS: We applied a computational model simulation to illustrate the mechanism of IUT with and without PET in TAPS occurring after laser surgery for twin-twin transfusion syndrome (TTTS). Model simula…

Detection of blood-transmissible agents: can screening be miniaturized?

Transfusion safety relating to blood-transmissible agents is a major public health concern, particularly when faced with the continuing emergence of new infectious agents. These include new viruses appearing alongside other known reemerging viruses (West Nile virus, Chikungunya) as well as new strains of bacteria and parasites (Plasmodium falciparum, Trypanosoma cruzi) and finally pathologic prion protein (variant Creutzfeldt-Jakob disease). Genomic mutations of known viruses (hepatitis B virus, hepatitis C virus, human immunodeficiency virus) can also be at the origin of variants susceptible

Transfusion vs. alternative treatment modalities in acute bleeding: a systematic review.

BACKGROUND AND METHODS: The practice of transfusion varies a great deal between countries and hospitals. Therefore, a systematic literature review was performed to evaluate the evidence underlying practice of transfusion and alternative treatment modalities in acute bleeding. After a stepwise evaluation, 79 out of 2438 abstracts were approved as the evidence base. RESULTS: Albumin for volume therapy is not better than artificial colloids or crystalloids and may be detrimental in trauma patients. No outcome difference has been proved between artificial colloids and crystalloids. Use of hyperto

[Report of experiences in 20 years use of intrauterine fetal transfusion in severe Rh-induced hemolytic disease of newborn].

136 intrauterine transfusions in 90 fetuses between the 25th and 34th gestational week with severe hemolytic disease because of Rh-incompatibility have been performed after a prenatal step by step diagnostic in the years 1967 till 1986. The reason of the immunization was mistransfusion or omitted immune prophylaxis in 25 cases. 25 from 67 live born children died. There were 23 still births. 34 from 43 fetuses without hydrops survived, but only 8 from 47 fetuses with hydrops. The survival rate over all was 46.7 per cent.

Automated erythrocytopheresis for relief of priapism in sickle cell hemoglobinopathies.

Priapism, a complication of male patients with sickle hemoglobinopathies, has been managed by a variety of surgical and nonsurgical forms of therapy that often are unsuccessful. The application of automated erythrocytopheresis (red blood cell exchange) by continuous-flow and semicontinuous-flow procedures appears to offer distinct advantages in the treatment of complications resulting from sickle hemoglobinopathies. The successful application of erythrocytopheresis for the relief of priapism in a patient with hemoglobin SC disease is presented and probably represents the first case reported on

[Resuspension of erythrocyte concentrates: clinical examinations with human albumin and oxypolygelatine].

The acid-base status of packed red cells and the influence of resuspension with human albumin, oxypolygelatine (Gelifundol), Human-serum and physiological saline were examined. It was found that the severe metabolic acidosis of packed red cells increased only slightly with storage time, while the resuspension exerted divergent effects: Human albumin and physiological saline increased the metabolic acidosis slightly, but significant. Humanserum and oxypolygelatine diminished it because of their bicarbonate content. The clinical significance especially in massive transfusion is discussed.

Deficiency of plasma prostacyclin or PGI2 regenerating ability in sickle cell anaemia.

We have demonstrated that patients with sickle cell disease greater than 2 years of age have a marked decrease in their plasma PGI2 or prostacyclin regenerating ability (0.05 +/- 0.08 ng/mg of vascular tissue) when compared to normal controls (0.44 +/- 0.12). This abnormality was not present in other chronic haemolytic states, and was not related to concomitant hyposplenism. Normal activity was observed in patients with sickle cell disease following a period of prophylactic red cell transfusions (0.32 +/- 0.13). Since vascular prostacyclin is antiaggregatory and antithrombotic, the deficiency

Transfusion utilization during adnexal or peritoneal cancer surgery: effects on symptomatic venous thromboembolism and survival.

OBJECTIVE: To determine whether perioperative packed red blood cell (PRBC) and fresh frozen plasma (FFP) transfusions during ovarian, tubal, or peritoneal cancer surgery increase the risk of symptomatic postoperative venous thromboembolism (VTE) and adversely affect overall survival. METHODS: We conducted a retrospective review of all cases of surgical exploration for resection of stage IIIC-IV adnexal/peritoneal cancer between November 1998 and May 2002 at Memorial Sloan-Kettering Cancer Center. Patients with a history of prior or active preoperative VTE were excluded. Routine intraoperative

[Management of feto-maternal red cell allo-immunizations].

Feto-maternal red cell alloimmunization is defined by the presence in a pregnant woman of alloantibodies directed against blood group antigens present on the red blood cells of the fetus and inherited from the father. It arises from the immune response to a first contact to these same antigens during a prior transfusion, transplant or pregnancy. The placental transfer and the fixation of the antibodies on the fetal red cells antigenic targets lead to a haemolysis in the fetus and the newborn. The resulting haemolytic disease can show different clinical forms, from a mild anaemia with neonatal
