Prevalence of symptoms of cervical and lumbar stenosis among participants in the Osteoporotic Fractures in Men Study.

STUDY DESIGN: Cross-sectional. OBJECTIVES: To determine the prevalence of symptoms typical of cervical and lumbar stenosis, evaluate the relationship between lumbar and cervical symptoms, and assess the impact of these symptoms on health status. SUMMARY OF BACKGROUND DATA: Degenerative changes of the spine frequently associated with aging, may result in stenosis, a narrowing of the spinal canal. Little is known about the prevalence or health impact of symptoms associated with stenosis in older individuals. METHODS: Between March 2000 and April 2002, 5995 men aged > or = 65 years participati

Nocturnal penile tumescence and effects of complete spinal cord injury: possible physiologic mechanisms.

OBJECTIVES: To determine the role of the spinal cord in the initiation and/or modulation of nocturnal erectile activity, we measured nocturnal penile tumescence (NPT) in men with complete spinal cord injuries at known locations. METHODS: Eighteen men between the ages of 27 and 57 years (mean 45) with known complete spinal cord lesions and no history of other medical problems adversely affecting erectile function completed International Index of Erectile Function questionnaires and underwent RigiScan testing for two consecutive nights. RigiScan tracings were evaluated for the presence and qual

A neurofibromatosis type 1 patient with severe kyphoscoliosis and intrathoracic meningocele.

The patient presented with neurofibromatosis and a dystrophic kyphoscoliosis around the cervico-thoracic junction. When the patient was 59 years old, he started to suffer from dyspnea caused by an intrathoracic meningocele in the upper left thoracic cavity. A wide laminectomy from T2 to T5 was performed and the meningocele was resected. Although the dyspnoea disappeared postoperatively, the patient started to neurologically deteriorate. Laminectomy alone caused instability around the apex of the kyphosoliosis and spinal cord compression. Halo cast was applied and brought remarkable recovery of

Therapeutic spinal corticosteroid injections for the management of radiculopathies.

Current literature and a recent meta-analysis suggest a favorable role for corticosteroid injections in the nonoperative treatment of radiculopathy [70]. The superior results reported in recent literature may be attributable to precise fluoroscopically guided transforaminal placement of injectate close to the disc-nerve root interface and near the dorsal root ganglia, maximizing the therapeutic effect. The favorable results of corticosteroid injections in the treatment of radiculopathy caused by a focal disc herniation are consistent with the biochemical construct of radicular pain. The prelim

Spinal tuberculosis: atypical observations at MR imaging.

PURPOSE: To evaluate atypical magnetic resonance (MR) imaging features of spinal tuberculosis. MATERIALS AND METHODS: Between 1990 and 1993, five of 11 consecutive patients with spinal tuberculosis (two men and three women, aged 30-57 years) had MR findings more suggestive of neoplasm than infection. One patient, a European immigrant, had acquired immunodeficiency syndrome (AIDS). RESULTS: Areas involved with tuberculosis were hypointense on T1-weighted images and hyperintense on T2-weighted images, and became enhanced with gadopentetate dimeglumine. In two patients, tuberculosis affected on

Three-dimensional response of a lumped parameter head-neck model due to impact and impulsive loading.

A numerical procedure developed previously for predicting sagittal-plane motion of the human head-neck system due to impact and impulsive loading has been extended to three dimensions. In both situations, a lumped parameter approach is employed, but the current model lumps the mechanical response of each intervertebral joint into a single force-deformation relation evaluated from mechanical properties assembled by various investigators. Computations were performed to obtain the response of the model to a two-dimensional case of flexion whiplash, to one three-dimensional case of side impact to

Giant cell tumors of the spine: a clinical study of six cases, with emphasis on the radiological features, treatment, and follow-up.

Six cases of giant cell tumor of the spine are reported. Based on cases reported in the literature and on their personal experience, the authors stress the difficulty of making a proper differential diagnosis between giant cell tumor and other giant cell variants (i.e., aneurysmal bone cyst and benign osteoblastoma) on the basis of the clinicoradiological findings alone. It is pointed out that the disappointing recurrence rate of about 50% for giant cell tumors in ordinary locations (such as the long bones) and in the sacrum is remarkably decreased when these tumors involve the movable part of

The Burden of Concomitant Spinal Injury in the Setting of Traumatic Brain Injury that Required Admission to ICU-Lessons from a Tertiary Neurosurgery Center.

INTRODUCTION: While individually both traumatic brain injury (TBI) and traumatic spinal injury have been studied extensively, the relationship between concurrent TBI and spinal column and/or cord injuries has not. We aimed to identify basic epidemiology, patterns of injury, and patient outcomes from a population served by a tertiary neurosurgery center. METHODOLOGY: A database was built of patient data on admissions to an adult intensive care unit with a TBI over a 12-year period. Electronic patient records, sourced from the database of the Scottish Intensive Care Society Audit Group (SICSAG)

Volume of Lytic Vertebral Body Metastatic Disease Quantified Using Computed Tomography-Based Image Segmentation Predicts Fracture Risk After Spine Stereotactic Body Radiation Therapy.

PURPOSE: To determine a threshold of vertebral body (VB) osteolytic or osteoblastic tumor involvement that would predict vertebral compression fracture (VCF) risk after stereotactic body radiation therapy (SBRT), using volumetric image-segmentation software. METHODS AND MATERIALS: A computational semiautomated skeletal metastasis segmentation process refined in our laboratory was applied to the pretreatment planning CT scan of 100 vertebral segments in 55 patients treated with spine SBRT. Each VB was segmented and the percentage of lytic and/or blastic disease by volume determined. RESULTS:
