Morbidity associated with total colon Hirschsprung's disease.

Increased awareness of total colon Hirschsprung's disease (TCH) has led to improvement in patient care and a decrease in the associated mortality. Morbidity has remained high and necessitates close follow-up to prevent the development of life-threatening complications. We retrospectively reviewed the records of 20 children with TCH treated at this institution since 1961. Follow-up averaged 40.4 months (range, 1 to 150). All patients were diverted with an ileostomy or jejunostomy. Eight children had a Swenson pull-through, three had a Martin procedure, and eight had a Kimura procedure. One chil

Congenital pyloric atresia and junctional epidermolysis bullosa: a report of long-term survival and a review of the literature.

The coexistence of congenital pyloric atresia (PA) and epidermolysis bullosa (EB) in newborns is a rare but distinct association. Mortality is high. In particular, a universally fatal outcome has been reported in neonates born with the junctional type of EB and PA. This has led some investigators to advocate that surgical correction of PA be withheld to obviate needless suffering. We treated five patients, including one set of siblings. Maternal hydramnios and nonbilious vomiting were constant features. Delayed passage of meconium was found in four. Plain x-rays demonstrated gastric dilatation

The worldwide use of steroidal contraception.

Historically, concerns about rapidly growing populations and resources that could not support them persuaded governments of developing countries to initiate family planning programs. Between the early 1960s and the early 1980s, the number of governments supporting these programs increased from 7 to approximately 120. Today, 52 governments in developing countries provide support to family planning programs because of the demographic rationale, and 65 governments do so because of human rights and health considerations: The effective use of family planning significantly diminishes infant, child,

Demography of dialysis and transplantation in children in Europe, 1984. Report from the European Dialysis and Transplant Association Registry.

The demography of treatment of children by renal replacement therapy in Europe is presented based on returns of individual patient questionnaires to the EDTA Registry up until the close of 1984. Patient questionnaires for 1984 were completed by all centres which defined themselves as special paediatric units. A total of 4983 patients have been reported to the Registry up until 31 December 1984 as having commenced renal replacement therapy under the age of 15. Of these, 1570 were known to be alive on a defined form of treatment at the end of 1984 and still under the age of 15. The numbers of th

A preliminary study in medical anthropology in Brunei, Borneo.

Nine rural village communities in the jungle of Brunei, Borneo were studied to ascertain possible effects of rapid social change on the health of the inhabitants. The social mores and religious beliefs of the rural tribes--Iban, Dusun, and Punan--have undergone but little change over many hundreds of years. During the past three decades, however, enormous social pressures for change have envolved from extraordinary economic prosperity of the region, due to the exploitation of large discoveries of oil and gas. Much of the money has been invested in health care, with the result that malaria, typ

Evolution in caesarean section practices in North Kivu: Impact of caregiver training.

INTRODUCTION: A caesarean section is a major obstetric procedure that can save the life of mother and child. Its purpose is to protect the mother's health from the complications of childbirth and to protect the baby's health. In sub-Saharan Africa (SSA), there are major inequalities in access to caesarean sections and significant variations in practices to determine the indications for the procedure. Periodic analyses of maternal deaths have shown that more than half of maternal and new born deaths are due to suboptimal care and are therefore potentially preventable. The objective of our study

MeningiSSS: A New Predictive Score to Support Decision on Invasive Procedures to Monitor or Manage the Intracerebral Pressure in Children with Bacterial Meningitis.

BACKGROUND: Knowing the individual child's risk is highly useful when deciding on treatment strategies, especially when deciding on invasive procedures. In this study, we aimed to develop a new predictive score for children with bacterial meningitis and compare this with existing predictive scores and individual risk factors. METHODS: We developed the Meningitis Swedish Survival Score (MeningiSSS) based on a previous systematic review of risk factors. From this, we selected risk factors identified in moderate-to-high-quality studies that could be assessed at admission to the hospital. Using d

A novel animal model of thymic tumour: development of epithelial thymoma in transgenic rats carrying human T lymphocyte virus type I pX gene.

The incidence of lower respiratory tract infection (LRTI) in women of child-bearing age is approximately 64 per 1000 population. The spectrum of illness ranges from acute bronchitis, which is very common, through influenza virus infection and exacerbations of underlying lung disease, to pneumonia, which, fortunately is uncommon (<1.5% LRTI), but can be severe. Acute bronchitis is generally mild, self-limiting and usually does not require antibacterial therapy. Influenza virus infection in pregnant women has been recently related to increased hospitalization for acute cardiorespiratory conditio

Verbal autopsy as a tool for identifying children dying of sickle cell disease: a validation study conducted in Kilifi district, Kenya.

BACKGROUND: Sickle cell disease (SCD) is common in many parts of sub-Saharan Africa (SSA), where it is associated with high early mortality. In the absence of newborn screening, most deaths among children with SCD go unrecognized and unrecorded. As a result, SCD does not receive the attention it deserves as a leading cause of death among children in SSA. In the current study, we explored the potential utility of verbal autopsy (VA) as a tool for attributing underlying cause of death (COD) in children to SCD. METHODS: We used the 2007 WHO Sample Vital Registration with Verbal Autopsy (SAVVY) V

[Pre-eclampsia screening in first and second trimester].

Pre-eclampsia is a pregnancy-associated disease of the second part of the pregnancy, occurring mainly after 20th weeks gestation. The prevalence of hypertension in pregnancy is between 5 to 11% and affects mainly women under 20 years of age. An inadequate invasion of trophoblasts with consequential placental ischemia as a result of insufficiently dilated uterine spiral arteries is thought to be an initial cause in the pathogenesis of pre-eclampsia. The clinical symptoms of pre-eclampsia, such as loss of intravascular volume and edema, are caused by generalized endothelial dysfunction. These sy

Consequences of in-utero death in a twin pregnancy.

BACKGROUND: Twins have a higher mortality and morbidity than singletons and, among twins, the surviving co-twin of a fetus that dies in utero is particularly at risk. We did a cohort study to quantify mortality and serious morbidity in co-twin survivors of fetuses that died in utero. METHODS: We collected data of all registered twin births in England and Wales between 1993 and 1995 in which one twin was registered as having died in utero. Copies of all death certificates of these fetuses and death certificates of live-born co-twins of fetuses that died in utero were obtained from the Office f

Older maternal age and pregnancy outcome: a review of the literature.

We are in the early phase of a period when the increased numbers of women born during the 1947 to 1965 baby boom are entering their later child-bearing years. They are also part of a generation of women who are increasingly delaying childbirth until their 30s. These two factors will likely increase the proportion of total births accounted for by this 35- to 49-year age group by 72 per cent, from 5.9 per cent in 1982 to 8.6 per cent by the turn of the century. There are important and specific risks related to pregnancies for older women as compared to younger women. It is likely that a woman's

Congenital abnormalities in Pacific Island Countries and Territories: a scoping review.

BACKGROUND: Pacific Island Countries and Territories experience disproportionately high rates of stillbirth, neonatal, and childhood deaths, yet the drivers of these outcomes remain unclear. The impact of congenital abnormalities within the Pacific has not been clearly established, despite these abnormalities being a leading global cause of infant and child mortality. This scoping review maps available literature to determine the current knowledge of congenital abnormalities in Pacific Island Countries and Territories. METHODS: Two independent reviewers searched six scholarly databases (PubMe…

Centring the voices of birth mothers who faced infant removal through the co-production of the MUMS@RISC artwork: an arts-based evaluation of trauma-informed research collaboration and participation.

BACKGROUND: In the UK, newborn babies are a growing group of children entering the care system due to safeguarding concerns. Their mothers often experienced a life course of adversity and trauma, resulting in fractured trust in public services and health and social care professionals. Moreover, pregnant and postnatal women with involvement from child protection agencies are known to have increased risk of perinatal morbidity and mortality. To address the existing evidence gaps around perinatal healthcare and outcomes for this group of women, the MUMS@RISC study was designed, with cyclical invo
