Resolution of Hepatic Artery Thrombosis in 2 Pediatric Liver Transplant Patients.

Hepatic artery thrombosis (HAT) is a serious complication after liver transplantation. This is the first report of spontaneous resolution of HAT in pediatric liver transplant patients on low molecular weight heparin therapy. A total of 2 patients, a 26-month-old boy who presented with acute liver failure and required emergent liver transplantation and a 15-year-old boy with ulcerative colitis and autoimmune hepatitis-primary sclerosing cholangitis overlap underwent liver transplantation for progressive cirrhosis; both developed HAT during the postoperative period. They were both treated with l…

[Rupture of a pancreaticoduodenal artery aneurysm into the common bile duct secondary to chronic pancreatitis: a case report].

A 57-year-old man with a history of excessive drinking presented to our emergency department complaining of chest discomfort. He had been attending a clinic for diabetes mellitus and hypertension, at which he had been informed that he had anemia that was worsening. Computed tomography (CT) of the abdomen revealed hemobilia and chronic pancreatitis. Due to complicated cholangitis, endoscopic nasobiliary drainage was performed, and a contrast-enhanced abdominal CT scan showed a pancreaticoduodenal aneurysm close to the common bile duct. We diagnosed a ruptured pancreaticoduodenal artery aneurysm…

Spontaneous pneumobilia revealing choledocho-duodenal fistula: A rare complication of peptic ulcer disease.

Spontaneous pneumobilia without previous surgery or interventional procedures indicates an abnormal biliary-enteric communication, most usually a cholelithiasis-related gallbladder perforation. Conversely, choledocho-duodenal fistulisation (CDF) from duodenal bulb ulcer is currently exceptional, reflecting the low prevalence of peptic disease. Combination of clinical data (occurrence in middle-aged males, ulcer history, absent jaundice and cholangitis) and CT findings including pneumobilia, normal gallbladder, adhesion with fistulous track between posterior duodenum and pancreatic head) allow …

Posttransplant bilioportal fistula with portal vein thrombosis: a case report.

An 8-year-old female patient, known to have post-Kasai biliary atresia with mild intrapulmonary shunting, underwent living donor liver transplantation because of recurrent cholangitis. After the treatment of postoperative biliary stricture with percutaneous transhepatic biliary drainage, the patient subsequently developed hematochezia with portal vein thrombosis. The intraoperative findings showed portal vein thrombosis with a bilioportal fistula. We performed closure of the bilioportal fistula and reconstruction of the portal vein with a native internal jugular vein interposition graft. A bil…

The first reported case of lichen planus following inactivated influenza vaccination.

Lichen planus (LP) is an idiopathic, inflammatory, pruritic dermatosis of unknown origin. An increased prevalence of a wide range of diseases such as viral hepatitis C, hepatitis B, primary sclerosing cholangitis, and primary biliary cirrhosis have been associated with LP. Recently, LP has been reported following administration of different types of hepatis B vaccines but a relationship with an inactivated influenza vaccine (Fluarix, GlaxoSmithKline) has not been reported previously. We present a case of generalized LP manifesting 7 days after the first 0.5-ml dose of influenza vaccine, admini…

Pseudotumoral azygos and paraesophageal varices of posterior mediastinum in a 15-month-old infant: a case report.

A case of azygos and paraesophageal varices presenting as a posterior mediastinal mass in a 15-month-old infant with biliary atresia is described. The patient was evaluated for living donor liver transplantation because of repeated cholangitis after Kasai operation, and plain CT scan demonstrated a mass in posterior mediastinum. The operation of mediastinal tumor resection was planned before liver transplantation in order to exclude malignant disease, however, possibility of paraesophageal varices remained. Contrast-enhanced magnetic resonance imaging clearly demonstrated azygos and paraesopha…

Ursodeoxycholic acid treatment of vanishing bile duct syndromes.

Vanishing bile duct syndromes (VBDS) are characterized by progressive loss of small intrahepatic ducts caused by a variety of different diseases leading to chronic cholestasis, cirrhosis, and premature death from liver failure. The majority of adult patients with VBDS suffer from primary biliary cirrhosis (PBC) and primary sclerosing cholangitis (PSC). Ursodeoxycholic acid (UDCA), a hydrophilic dihydroxy bile acid, is the only drug currently approved for the treatment of patients with PBC, and anticholestatic effects have been reported for several other cholestatic syndromes. Several potential…

Autoimmune hepatitis overlap syndrome: Clinical presentation, treatment response, and disease evolution: Tertiary centre experience.

BACKGROUND:: Autoimmune hepatitis (AIH) overlap syndromes are conditions wherein features of AIH coexist with those of primary sclerosing cholangitis (PSC) or primary biliary cholangitis (PBC). We aimed to evaluate their clinical presentation, treatment response, and disease outcome in an Arab cohort. METHODS:: AIH overlap patients, based on the Paris criteria for PBC/AIH overlap (n = 19), and revised International Autoimmune Hepatitis Group (IAIHG) score for PSC/AIH overlap (n = 17) were reviewed at a single center in Saudi Arabia, from 2001 to 2024. Biopsy-proven standalone AIH patients (n …

Ursodeoxycholic acid treatment did not show protective effect for severe COVID-19 outcomes - a nationwide register study.

BACKGROUND: Ursodeoxycholic acid (UDCA), used for treating cholestasis, was reported to protect against severe COVID-19 outcomes. We aimed to assess this potential effect using nationwide Swedish register data. METHOD: We included all SARS-CoV-2 test-positive subjects aged ≥ 18 years during 2020–2023, with previous diagnosis of potentially UDCA-treated diseases, including primary biliary cholangitis, liver cirrhosis, autoimmune hepatitis, or cholangitis. Subjects who had filled a UDCA prescription within 6 months before testing positive were considered exposed. Subjects were followed from test…

Western Experience of Hepatolithiasis: Clinical Insights from a Case Series in a Tertiary Center.

Background and Objectives: Hepatolithiasis (HL), or intrahepatic bile duct stone disease, shows regional variation and is a rare condition in Western countries. While cases from East Asia are often linked to chronic biliary infections and brown pigment stones, Western HL more frequently involves cholesterol or black pigment stones, typically in the context of prior cholecystectomy, biliary interventions, or congenital anomalies. The disease is generally associated with significant morbidity, including recurrent cholangitis, biliary strictures, and risk of cholangiocarcinoma. This study aimed t…
