Multidisciplinary management of intermediate and high-risk pulmonary embolism in the era of mechanical thrombectomy: a local practice guideline.

BACKGROUND: Risk stratification and management of pulmonary embolism (PE) continues to evolve, particularly for patients in the intermediate- and high-risk category, where treatment decisions are complicated by patient heterogeneity and limited guidance on reperfusion strategies. Catheter-based therapy (CBT) is becoming commonplace internationally; however, there is no published guidance on a standard approach to when to use these therapies in the Australian setting. AIM: We aimed to develop a consensus-based, evidence-informed algorithm for risk stratification and management of PE at our ins

Coexistence of Acute Appendicitis and Mesenteric Cystic Lymphatic Malformation in an Adult: A Case Report and Narrative Review of Intraoperative Management Strategies.

Background: Mesenteric cystic lymphatic malformations (MCLMs) are rare benign lymphatic malformations predominantly diagnosed in children. Adult cases are exceptional and typically discovered incidentally during imaging or surgery for unrelated conditions. Their intraoperative identification, particularly in emergency settings, poses diagnostic and surgical challenges due to anatomical complexity and potential vascular involvement. Methods: A literature review was performed in PubMed and Scopus to contextualize this case, focusing on intraoperative management strategies, recurrence risk, and s

Automatic detection of orthodontically induced external root resorption based on deep convolutional neural networks using CBCT images.

Orthodontically-induced external root resorption (OIERR) is among the most common risks in orthodontic treatment. Traditional OIERR diagnosis is limited by subjective judgement as well as cumbersome manual measurement. The research aims to develop an intelligent detection model for OIERR based on deep convolutional neural networks (CNNs) through cone-beam computed tomography (CBCT) images, thus providing auxiliary diagnosis support for orthodontists. Six pretrained CNN architectures were adopted and 1717 CBCT slices were used for training to construct OIERR detection models. The performance of

Treatment Barriers and Modality Preferences Among Individuals With Obsessive-Compulsive and Related Disorders.

OBJECTIVES: Facing numerous barriers to care, most individuals experiencing symptoms of obsessive-compulsive and related disorders do not receive evidence-based treatment. While much existing literature has identified barriers to mental health treatment broadly, little empirical work has assessed barriers specifically within a sample of individuals with obsessive-compulsive and related disorders, whose identified barriers and treatment preferences may differ from other clinical samples. In particular, this information is lacking for pediatric samples. METHODS: The current study examined treat

Neonatal Outcomes Following Elective Induction of Labor at 39 Weeks: A Systematic Review.

Elective induction of labor (eIOL) at 39 weeks of gestation has gained prominence in obstetric practice, yet its impact on neonatal outcomes remains debated. This systematic review aimed to synthesize evidence on neonatal outcomes following eIOL at 39 weeks compared to expectant management, addressing critical knowledge gaps to inform clinical decision-making. Following PRISMA guidelines, a comprehensive search of PubMed, Scopus, Web of Science, and Embase identified 16 eligible studies. Inclusion criteria focused on low-risk pregnancies undergoing eIOL at 39 weeks, with neonatal outcomes as p…

A quality improvement initiative to develop an interprofessional peripheral nerve transfer clinic for individuals with traumatic cervical spinal cord injury.

PURPOSE: Loss of upper extremity (UE) function impacts almost every aspect of daily life and upper limb recovery is reported to be a major priority of individuals living with tetraplegia. Surgical peripheral nerve transfer (PNT) offers the potential to restore volitional control of elbow, wrist and hand function of individuals with C5-C8 tetraplegia AIS A-C. Unfortunately, while there is growing evidence supporting the role of PNT in spinal cord injury (SCI) rehabilitation, there are currently no internationally-recognized consensus-derived best practices for provision of PNT following spinal

Double balloon enteroscopy in pediatrics: Evaluation of midgut pathologies at a national medical center.

INTRODUCTION AND AIM: Double balloon enteroscopy is used for the diagnosis and treatment of diseases of the small bowel. It is a less invasive means for carrying out a therapeutic intervention, compared with surgical alternatives. Experience with double balloon enteroscopy in pediatrics is in the early stages of development. We believe the performance of this procedure in children has a significant clinical impact on therapeutic decision-making in diseases of the small bowel. Our aim was to evaluate double balloon enteroscopy in children for the diagnosis and treatment of midgut pathologies.

Increasing transparency of computer-aided detection impairs decision-making in visual search.

Recent developments in artificial intelligence (AI) have led to changes in healthcare. Government and regulatory bodies have advocated the need for transparency in AI systems with recommendations to provide users with more details about AI accuracy and how AI systems work. However, increased transparency could lead to negative outcomes if humans become overreliant on the technology. This study investigated how changes in AI transparency affected human decision-making in a medical-screening visual search task. Transparency was manipulated by either giving or withholding knowledge about the accu

Analysis of The 2024 ESC/EACTS Guidelines For The Management Of Atrial Fibrillation.

The 2024 European Society of Cardiology guidelines for atrial fibrillation (AF) emphasize a patient-centered approach to management, structured around the AF-CARE pathway: Comorbidity and risk factor management (C), Avoiding stroke and thromboembolism (A), Reducing symptoms through rate and rhythm control (R), and Evaluation and dynamic reassessment (E). This framework ensures that comorbidities such as hypertension, heart failure, diabetes, and obesity are effectively managed to prevent disease progression and improve outcomes. A key principle of the guidelines is shared decision making invol

Indications for ascending aorta-bifemoral bypass grafting in patients with IHD and occlusion of the infrarenal aorta.

The problem concerning surgical decision-making in patients with significant atherosclerotic lesions of arteries of more than one basin will, probably, be important for cardiovascular surgeons for more than one decade. Even the centres possessing experience in several thousand cases of successful treatment of multifocal atherosclerosis have from time to time been facing non-trivial clinical situations requiring a non-standard decision the recommendations for which could hardly be found in the guidelines available, if at all. This article describes the technique and immediate results of an oper

Disability Experiences and Perspectives Regarding Reproductive Decisions, Parenting, and the Utility of Genetic Services: a Qualitative Study.

Genetic counselors adopt seemingly contradictory roles: advocating for individuals with genetic conditions while offering prenatal diagnosis and the option of selective termination to prevent the birth of a child with a disability. This duality contributes to the tension between the disability and clinical genetics communities. Varying opinions exist amongst the disability community: some value genetic services while others are opposed. However, there is limited research exploring the opinions of individuals with a disability regarding issues related to reproduction and genetic services in the
