Beyond maternal mortality--magnitude, interrelationship, and consequences of women's health, pregnancy-related complications and nutritional status on pregnancy outcomes.

The magnitude of maternal mortality in developing countries and its disparity with similar statistics from the developed world has touched a responsive chord among policy makers and health services program officials. What is not well appreciated, however, is that maternal mortality is only the tip of the iceberg--for every one maternal death, acute obstetrical complications cause suffering in nearly 100 women, 250 women contract a sexually transmitted disease, and 1000 women suffer stunting and/or anemia. All of these problems impact on the pregnancy outcome, both for the woman as well as for …

HIV-preventive cognitions amongst secondary school students in Uganda.

A self-completion questionnaire was used to survey HIV-preventive cognitions among secondary school students in Kabarole district, Uganda. An encouraging pattern of HIV-preventive beliefs was observed, suggesting that health education efforts have been effective in establishing basic transmission knowledge, accurate risk perception and an understanding of effective precautions. Further analysis explored the prerequisites of men and women's HIV-preventive self-efficacy, and intentions and tolerance of people with AIDS. Approximately a third of variance in men and women's condom use self-efficac

Development of a practical screening tool to identify preterm, low-birthweight neonates in Ethiopia.

Preterm, low-birthweight (LBW) newborn infants are at high risk of neonatal mortality and morbidity and need early referral for special paediatric care. In developing countries, birthweight and gestational age often cannot be measured and a practical screening tool based on surrogate neonatal body measurements to identify high-risk infants would be very useful. We studied a consecutive series of 843 singleton infants born at a referral hospital in Addis Ababa, Ethiopia. Gestational age, birthweight, and four body measurements (chest, head, and mid-arm circumferences and length) were accurately…

Complicated falciparum malaria.

We studied 50 cases of complicated falciparum malaria in order to evaluate the different clinical presentations. Thirty five had cerebral malaria while 15 presented with extracerebral features including diarrhea and vomiting (n = 6), hepatitis (n = 4), acute renal failure (n = 3), and gastrointestinal bleeding (n = 2). These cases were treated with quinine. Mortality was higher in extracerebral form (33.3%) as compared to cerebral malaria (22%). Our study suggests that even though cerebral malaria remains the single most important cause of high mortality in complicated falciparum malaria, extr

Experience with poliovaccines in the control of poliomyelitis in India.

OPV was introduced in the National Immunisation Programme (NIP) in 1979. The reported number of cases of poliomyelitis was about 20,000 in 1979, 38,000 in 1981; it gradually declined to the pre-immunisation level of 20,000 in 1986, but increased to 28,000 in 1987. In 1989, 10 years after NIP, the reported number of cases fell below the pre-immunisation level, and has further declined to 5669 in 1991. The 3-dose OPV coverage during infancy was estimated to be about 80% in 1991. If reporting efficiency is 10%, then about 60,000 cases would have occurred in 1991, for an incidence of disease of ab

Impact of national immunization schedule on vaccine preventable diseases: a hospital based study.

An analysis of vaccine preventable diseases (VPD) in two block years, i.e., 1972-1975 and 1986-1989 showed an overall decline in morbidity and mortality. Improvement in morbidity was most noticeable in typhoid fever followed by polio and tetanus. However, in tuberculosis and measles with compilations, there was a significant increase in admission rates 3.8 vs 4.4% and 1.8 vs 2.2%, respectively. Mortality in vaccine preventable diseases except polio has declined significantly.

Strategies to maintain health in the Third World.

International cooperation with Third World countries aims at reducing the high morbidity and mortality of the population to a tolerable level. The main health problems are caused by poverty. Thus, the range of diseases in tropical countries can be explained more readily by the socio-economic situation than solely by the climate. Health services, in Africa in particular, have had to reduce drastically their budgets in the last ten years and now have only approximately 1/1000th of the funds usually available in industrialised countries. High population growth reduces the resources available per
