Percutaneous Aortic Valve Closure in Patient With Left Ventricular Assist Device and Dilated Aortic Annulus.

Percutaneous transcatheter intervention for aortic regurgitation secondary to implantation of a continuous-flow left ventricular assist device remains challenging, because of the minimal global experience with these procedures. Two treatment options are available: transcatheter aortic valve replacement, which is not always feasible when a dilated aortic annulus is present, and percutaneous aortic valve occlusion. We report a successful percutaneous closure of the aortic valve using an oversized Amplatzer patent foramen ovale multifenestrated device (St Jude Medical, Saint Paul, MN) to treat ao

Glycogen storage disease type IV: dilated cardiomyopathy as the isolated initial presentation in an adult patient.

Glycogen storage disease type IV (GSD IV, Andersen disease) is a rare autosomal recessive condition. The childhood neuromuscular subtype of GSD IV is characterised by a progressive skeletal myopathy with cardiomyopathy also reported in some individuals. We report a case of a 19-year-old man who presented with severe non-ischaemic dilated cardiomyopathy (NIDCM) necessitating heart transplantation, with biopsy showing aggregations of polyglucosan bodies in cardiac myocytes. He had no signs or symptoms of muscle weakness, liver dysfunction or neurologic involvement. A homozygous GBE1 c.607C>A (p.

Oculomotor behavior and the level of repetition in motor practice: Effects on pupil dilation, eyeblinks and visual scanning.

The benefits of less repetitive practice in motor learning have been explained by the increased demand for memory processes during the execution of motor skills. Recently, a new perspective associating increased demand for perception with less repetitive practice has also been proposed. Augmented information gathering and visual scanning characterize this higher perceptual demand. To extend our knowledge about mental effort and perceptual differences in practice organization, the association between oculomotor behavior and type of practice was investigated. We required participants to press fo

A case report: X-linked dystrophin gene mutation causing severe isolated dilated cardiomyopathy.

BACKGROUND: X-linked dilated cardiomyopathy (XLDCM) is a rare but rapidly progressive cardiomyopathy caused by dystrophin gene mutation. Mutations are more often associated with Duchenne and Becker Muscular Dystrophy, which are characterized by skeletal muscle weakness or limb girdle dystrophy. However, patients with isolated XLDCM have normal skeletal muscle but complete dystrophin loss in cardiac muscle resulting in isolated myocardial involvement without overt signs of skeletal myopathy. CASE SUMMARY: A previously well 16-year-old boy developed sudden onset dense left-sided weakness and fa

Value of Speckle Tracking-Based Deformation Analysis in Screening Relatives of Patients With Asymptomatic Dilated Cardiomyopathy.

OBJECTIVES: This study sought to investigate the prevalence of systolic dysfunction using global longitudinal strain (GLS) and its prognostic value in relatives of dilated cardiomyopathy (DCM) patients that had normal left ventricular ejection fraction (LVEF). BACKGROUND: DCM relatives are advised to undergo cardiac assessment including echocardiography, irrespective of the genetic status of the index patient. Even though LVEF is normal, the question remains whether this indicates absence of disease or simply normal cardiac volumes. GLS may provide additional information regarding (sub)clinic

Utility of functional lumen imaging probe in esophageal measurements and dilations: a single pediatric center experience.

BACKGROUND: Pediatric esophageal stenosis can be challenging to manage due to post-dilation tissue response involving fibroblast activity resulting in scar reformation. The functional lumen imaging probe (FLIP) uses high-resolution impedance planimetry to measure key luminal parameters during a volume-controlled distension. We sought to evaluate the safety as well as possible settings of EndoFLIP and EsoFLIP in the pediatric population. METHOD: We performed a retrospective chart review of all patients that had EndoFLIP (with and without balloon dilation) or EsoFLIP done between July 2017 and

Impact of Ascending Aorta Dilation on Mid-Term Outcome After Transcatheter Aortic Valve Implantation.

BACKGROUND: Severe aortic stenosis (AS) is often associated with ascending aorta dilation (AAD). AAD is amenable to surgical correction combined with aortic valve replacement. Transcatheter aortic valve implantation (TAVI) might represent a valid therapeutic option in these patients when AAD correction Is not indicated. The aim of the present study is to evaluate the impact of concomitant AAD on early and mid-term outcomes after TAVI for symptomatic severe AS. METHODS: This is a single-center observational study including patients undergoing transfemoral TAVI. All patients with previous surge

Vaginal Dilator Therapy: A Guide for Providers for Assessing Readiness and Supporting Patients Through the Process Successfully.

Vaginal dilator therapy is used to increase vaginal length for vaginal agenesis, to increase vaginal width for vaginal narrowing, and to prevent or treat stenosis after vaginal surgery. Although it is an effective therapy, many reproductive health providers have had little training on how to guide patients through this therapy. The purpose of this review is to educate providers on how to assess patient readiness and how to support patients through the process of vaginal dilation.

Valve-Sparing Tetralogy of Fallot Repair With Intraoperative Dilation of the Pulmonary Valve. Mid-Term Results.

Deleterious long-term effects of chronic pulmonary regurgitation after repair of tetralogy of Fallot have become evident during the last decades. Subsequently, some groups have developed strategies to spare the pulmonary valve function at the time of repair with good early results. However, mid-term outcomes are scarce in the literature and in some cases controversial. The aim of our study is to report our results mid-term with valve-sparing repair of tetralogy of Fallot. We retrospectively reviewed patients undergoing tetralogy of Fallot repair and having preservation of the pulmonary valve w

Regional Multi-View Learning for Cardiac Motion Analysis: Application to Identification of Dilated Cardiomyopathy Patients.

OBJECTIVE: The aim of this paper is to describe an automated diagnostic pipeline that uses as input only ultrasound (US) data, but is at the same time informed by a training database of multimodal magnetic resonance (MR) and US image data. METHODS: We create a multimodal cardiac motion atlas from three-dimensional (3-D) MR and 3-D US data followed by multi-view machine learning algorithms to combine and extract the most meaningful cardiac descriptors for classification of dilated cardiomyopathy (DCM) patients using US data only. More specifically, we propose two algorithms based on multi-view

RNA binding protein 24 deletion disrupts global alternative splicing and causes dilated cardiomyopathy.

RNA splicing contributes to a broad spectrum of post-transcriptional gene regulation during normal development, as well as pathological manifestation of heart diseases. However, the functional role and regulation of splicing in heart failure remain poorly understood. RNA binding protein (RBP), a major component of the splicing machinery, is a critical factor in this process. RNA binding motif protein 24 (RBM24) is a tissue-specific RBP which is highly expressed in human and mouse heart. Previous studies demonstrated the functional role of RBM24 in the embryonic heart development. However, the

A PLN nonsense variant causes severe dilated cardiomyopathy in a novel autosomal recessive inheritance mode.

BACKGROUND: Pathogenic variants in human phospholamban coding gene (PLN) are known to cause hereditary dilated cardiomyopathy with heart failure in an autosomal dominant mode. METHODS: We performed high-depth targeted next-generation sequencing using a cardiomyopathy-panel containing 80 disease-related genes in 650 unrelated patients with non-ischemic cardiomyopathy to identify potential pathogenic PLN variants. To comprehensively evaluate the genetic cause of the proband and his pedigree, whole-exome sequencing and Sanger sequencing were performed. RESULTS: A novel homozygous nonsense varia

Angiotensin II Overstimulation Leads to an Increased Susceptibility to Dilated Cardiomyopathy and Higher Mortality in Female Mice.

Heart failure (HF) is associated with high mortality and affects men and women differently. The underlying mechanisms for these sex-related differences remain largely unexplored. Accordingly, using mice with cardiac-specific overexpression of the angiotensin II (ANGII) type 1 receptor (AT1R), we explored male-female differences in the manifestations of hypertrophy and HF. AT1R mice of both sexes feature electrical and Ca2+ handling alterations, systolic dysfunction, hypertrophy and develop HF. However, females had much higher mortality (21.0%) rate than males (5.5%). In females, AT1R stimulati

Endomyocardial biopsy in patients with acute myocarditis, idiopathic dilated cardiomyopathy, and arrhythmogenic right ventricular dysplasia.

Endomyocardial biopsy (EMB) is useful for the diagnosis of myocarditis, cardiac sarcoidosis, and non-ischemic cardiomyopathy. In this mini-review, we discuss the diagnostic potential of EMB in cases of acute/chronic-active myocarditis, sarcoidosis, idiopathic dilated cardiomyopathy and arrhythmogenic right ventricular dysplasia. We also summarize the complications caused by endomyocardial biopsy procedures. Importantly, we finally review the emerging molecular biology technologies as well as biological engineering techniques that can help improve the diagnostic accuracy of EMB to diagnose myoc

Familial clustering of bicuspid aortic valve and its relationship with aortic dilation in first-degree relatives.

OBJECTIVE: Bicuspid aortic valve (BAV) is the most common congenital heart disease. This study aimed to determine the prevalence rate of BAV in first-degree relatives (FDR) and the inheritance pattern according to different morphotypes and aortic dilation. METHODS: BAV probands were consecutively studied at eight tertiary referral centres. After sequential screening, FDR were included in the study. The BAV morphotype, aortic dilation and aortic phenotype were assessed by transthoracic echocardiography. RESULTS: Seven hundred and twenty-four FDR of 256 BAV probands agreed to undergo family sc
