Current management and clinical outcomes for patients with haemorrhagic shock due to pelvic fracture in Korean regional trauma centres: A multi-institutional trial.

INTRODUCTION: The purpose of this study was to evaluate the current management and clinical outcomes in patients with hemodynamic instability due to pelvic fracture in three regional trauma centres in Korea. PATIENTS AND METHODS: Three regional trauma centres participated in this study, and 157 patients who were admitted between January 2015 and December 2018 were enroled. Clinical data were collected prospectively as part of the Korean trauma data bank and were analysed retrospectively. RESULTS: The mean age was 59.3 years, and 107 of 157 (68.2%) were male patients. The most common cause of

Antegrade nailing in femoral shaft fracture patients - comparison of outcomes of isolated fractures, multiple fractures and severely injured patients.

INTRODUCTION: Femoral shaft fractures in adults are high-energy fractures typically accompanied by additional fractures of the upper and lower extremities and brain, thoracic, or abdominal injuries. Intramedullary nailing enables early ambulation with a few complications, but rates of non-union remain high. Therefore, we aimed to compare bone union after femoral shaft fractures in adults (20-65 years old) depending on the injury severity and presence of multiple fractures. PATIENTS AND METHODS: This study retrospectively examined 178 patients (145 male and 33 female) who underwent intramedull

A Quartet of Elastic Stable Intramedullary Nails for More Challenging Pediatric Femur Fractures.

INTRODUCTION: The insertion of 2 elastic stable intramedullary nails (ESINs) is a common treatment for pediatric femur fractures. However, the use of this technique in length-unstable or metadiaphyseal fractures has historically been associated with higher complication rates. To improve stability, the addition of a third ESIN has been assessed biomechanically and clinically, but the addition of a fourth nail has only been evaluated biomechanically. The purpose of this study is to report our surgical technique and radiographic outcomes using a quartet of ESINs in pediatric femur fractures. MET

Forces and moments in cervical spinal column segments in frontal impacts using finite element modeling and human cadaver tests.

Experiments have been conducted using isolated tissues of the spine such as ligaments, functional units, and subaxial cervical spine columns. Forces and or moments under external loading can be obtained at the ends of these isolated/segmented preparations; however, these models require fixations at the end(s). To understand the response of the entire cervical spine without the artificial boundary/end conditions, it is necessary to use the whole body human cadaver in the experimental model. This model can be used to obtain the overall kinematics of the head and neck. The forces and moments at e

Deployment after limb salvage for high-energy lower-extremity trauma.

BACKGROUND: Many wounded warriors experienced high-energy lower-extremity trauma (HELET) that may be limb threatening. Volumetric muscle loss, posttraumatic osteoarthritis, nerve injuries, and pain may severely limit physical function. Several wounded warriors express a strong desire to return to their units and be deployed in their original military occupational specialty. We began the return-to-run (RTR) clinical pathway at our institution 2 years ago to facilitate high-performance goals such as these. It involves an energy storing ankle foot orthosis, the intrepid dynamic exoskeletal orthos

Brachytherapy management of the retroverted uterus using ultrasound-guided implant applicator placement.

PURPOSE: Patients with a retroverted uterus present a dilemma for brachytherapy in gynecologic malignancies because of the challenges of the procedure and the risk of uterine perforation. The purpose of this study was to evaluate the efficacy and outcome of ultrasound-guided brachytherapy applicator placement and intraoperative uterine anteversion in patients with gynecologic malignancies, who have a retroverted uterus. METHODS AND MATERIALS: Thirty-three brachytherapy insertions were performed in 18 patients with retroverted uterus (cervical cancer, 17; vaginal cancer, 1). The endocervical c

Spinal cord stimulation and early experimental cerebral spasm: the "functional monitoring" and the "preventing effect".

BACKGROUND: Clinical and experimental data on cerebral blood flow (CBF) changes during spinal cord stimulation (SCS) were published since 1986. The aims of the present work are: 1. To find an experimental model of reliable, simple and in vivo monitoring of "early" basilar artery spasm after subarachnoid haemorrhage (SAH) and 2. To investigate the effects of cervical spinal cord stimulation (CSCS) on it. Vasospasm due to SAH is both "acute" and "recurrent". Early spasm occurs within minutes of the SAH. its duration is approximately 1 hour. The need of different morphological and haemodynamic me

Radical correction of secondary nasal deformity in unilateral cleft lip patients presenting late.

It is universally acknowledged that correction of a cleft lip nasal deformity continues to be a difficult problem. In developing countries, it is common for patients with cleft lip deformities to present in their early or late teens for correction of severe secondary lip and nasal deformities retained after the initial repairs were carried out in infancy or early childhood. Such patients have never had the benefit of primary nasal correction, orthodontic management, or alveolar bone grafting at an appropriate age. Along with a severe nasal deformity, they present with alveolar arch malalignmen

Distraction osteogenesis after acute limb-shortening for segmental tibial defects. Comparison of a monofocal and a bifocal technique in rabbits.

BACKGROUND: Segmental bone defects can be treated with immediate limb-shortening followed by monofocal or bifocal distraction osteogenesis. In the present study, the efficacy of monofocal distraction osteogenesis was compared with that of bifocal distraction osteogenesis in a rabbit model. METHODS: Twenty-four skeletally mature New Zealand White rabbits were divided into two equal groups: one group had monofocal distraction osteosynthesis, and the other had bifocal distraction osteosynthesis. In both groups, a one-centimeter-long segment of bone was resected from the midpart of the tibial sha

[Two cases of pyogenic cervical discitis presenting tetraparesis].

The authors reported two cases of pyogenic cervical discitis presenting tetraparesis. Case 1: A 66-year-old male patient entered the hospital because of tetraparesis. Two weeks before the hospitalization, he had become feverish and awakened with motor weakness in all extremities. Magnetic Resonance Imaging (MRI) study revealed a lesion filling the anterior epidural space from C4 to C6 levels and posterior displacement of the spinal cord. Findings suggesting discitis of C5/6 and osteomyelitis of C5 and C6 were also obtained on MRI. These findings suggested that the tetraparesis was caused by co

Comparative studies of fertility and histologic development of contralateral scrotal testes in two rat models of unilateral cryptorchidism.

Fertility and the development of the contralateral scrotal testis in patients with unilateral cryptorchidism (UCO) remain controversial. This study investigated these controversies in two different UCO rat models using 43 Wistar King A rats. The animals were divided into three groups: I: an endocrinologic model of UCO was obtained by injecting pregnant dams with flutamide 100 mg/kg per day on days 15-17 of gestation (n = 12); II (n = 21): a mechanical model of UCO was obtained by extra-abdominal fixation of the gubernaculum in the neonatal period, III (n = 10): non-treated rats were used as co

Eye-movement-based machine learning for identifying Parkinson's disease dementia in a retrospective cohort: an interpretable model-development study.

BACKGROUND: Parkinson's disease dementia (PDD) is a major non-motor complication of Parkinson's disease (PD), but conventional cognitive screening is influenced by education, language, motor impairment, fatigue, and testing conditions. We developed and interpreted machine-learning models using eye-movement and demographic features to identify PDD among patients with PD. METHODS: In this retrospective single-center study, 486 patients were screened between December 2024 and February 2025. After exclusions, 297 patients with idiopathic PD were analyzed, including 84 without dementia and 213 wit

A morphology-based model for treatment allocation in condylar head fractures.

Condylar head fractures (CHFs) represent a heterogeneous injury spectrum, and indications for surgical versus conservative treatment remain inconsistently defined. Clinical decision-making is frequently based on qualitative morphological assessment rather than standardized criteria. This study aimed to develop and internally validate a morphology-based severity framework reflecting routine clinical treatment allocation patterns and to evaluate its ability to predict management strategies in CHFs. In this retrospective cohort study, all consecutive adult patients (≥18 years) with radiologically

Open Fractures Beyond Long Bones: Rethinking Age-Dependent Injury Mechanisms and Anatomical Patterns in Pediatric Trauma.

Background: Open fractures are traditionally associated with high-energy trauma, such as falls and traffic accidents, in healthy adults. However, the epidemiological characteristics of such injuries in children and adolescents remain incompletely defined. Most studies focus on long-bone injuries, while comprehensive epidemiological data across all anatomical regions are limited. This study aimed to analyze and provide an overview of age-dependent patterns of injury mechanisms and anatomical distribution in pediatric open fractures. Methods: This retrospective cohort study included all open fra

Development and Internal Evaluation of a Biomarker-Based Model for Preoperative Diagnosis of Infectious Nonunion.

OBJECTIVE: Preoperative identification of infectious nonunion remains challenging, particularly in patients without overt clinical signs of infection. Conventional inflammatory markers alone often provide insufficient diagnostic accuracy, creating uncertainty in surgical planning. This study aimed to develop and internally evaluate a predictive model combining novel and conventional biomarkers for preoperative diagnosis of infectious nonunion in long bone fractures. We hypothesized that integrating procalcitonin (PCT) with classical inflammatory markers would enhance diagnostic accuracy. METH
