Disorders of the patella are the most common cause of anterior knee pain. The etiologies of anterior knee pain are reviewed. A case report of an acute displaced patella fracture in a bipartite union is presented. Bipartite patellar development, incidence, radiographic findings, and clinical symptoms follow. Treatment of excision of displaced fragment provides an excellent result.

Accidental trauma associated with an epileptic convulsion in a 10-year-old, male chimpanzee (Pan troglodytes) resulted in incomplete paraplegia from fracture and subluxation of T4. Dorsal laminectomy and segmental spinal fixation were used in treatment. The segmental spinal fixation consisted of sublaminar wires attached to a contoured 316L stainless steel U-rod. The chimpanzee recovered sufficient function to allow reintroduction into the chimpanzee colony at a zoological park during the 12 months after surgery and continues to do well 24 months after surgery.

The displaced supracondylar fracture of the humerus in children is a very difficult injury to treat. Manipulation and splint immobilization is perhaps the most common method. A review of the literature on 1708 comparable cases revealed that several different treatments were used. The worst results occurred with manipulation and splint immobilization alone. The best results occurred with traction techniques and well-performed Kirschner-pin transfixation, either open or closed. A case of total dorsal luxation of the lunate, combined with an avulsion of the radial styloid is recorded. Radiographs did not reveal any other signs of carpal collapse. We suggest that a concurrent hyperflexion, ulnar deviation and pronation of the wrist joint may reasonably be regarded as the etiology of this lesion, which was treated by delayed, open reduction and partial intracarpal arthrodesis. Similar cases have not been reported.

After complicated distal radius fractures with persistent symptoms, a variety of radiological methods may be indicated, including plain films, magnification views, examination under stress and arthrography. CT accurately differentiates abnormalities of the distal radio-ulnar joint and aids in deciding on surgical treatment. Demonstration of degenerative changes and of subluxations is much more satisfactory in the third plane, since there is no superimposition.

Twenty-five children with combined elbow injury and forearm bone fractures have been reviewed. The incidence of serious complications was low, and only one patient was significantly handicapped. Open reduction and internal fixation of the supracondylar fracture with closed treatment of the forearm fracture gave the best results.

Essex-Lopresti injuries (ELIs) are characterized by fracture of the radial head, disruption of the forearm interosseous membrane, and dislocation of the distal radioulnar joint. This injury pattern results in axial and longitudinal instability of the forearm. Initial radiographs may fail to reveal the full extent of the injury, and therefore diagnosis in the acute setting requires a high index of suspicion. Reverse total shoulder arthroplasty (RTSA) has become the treatment of choice for patients with rotator cuff arthropathy. Complications and results of medial patellofemoral ligament (MPFL) reconstruction, performed between 2000 and 2003, were studied retrospectively in 24 randomised patients (24 knees). All procedures were performed for recurrent dislocation or subluxation of the patella, using an autogenous semitendinosus tendon. Two different methods of anchoring of the transplant were compared. After a 2-year follow-up, patellar stability was found to be similar, when the transplant was sutured to the fibrous tissue and periosteum overlying the patella or when it was fixed in a bone tunnel through the patella.

Between 1995 and 2000 twenty-two patients were treated for frontal sinus mucocele in the Department of Otorhinolaryngology of Medical University of Warsaw. History and physical examination revealed several-year-old chronic rhinosinusitis (CRS) in all but one cases. Thirteen patients had been previously operated on for this reason at least once. OBJECTIVE: To discuss cerebellar infarct in a patient with bilateral hypoplasia of the vertebral arteries.
CLINICAL FEATURES: A 67-year-old woman suffered neck pain and headaches immediately after a minor motor vehicle accident. Fracture and dislocation were radiographically ruled out. Within a few days, the patient began to experience symptoms of vertigo and dizziness. Computed tomography scanning of the brain revealed a cerebellar infarct, and an angiogram of the vertebral arteries demonstrated bilateral hypoplasia.
