Vitamin K and Osteoporosis.

Vitamin K acts as a coenzyme of carboxylase, catalyzing the carboxylation of several vitamin K dependent proteins. Beyond its well-known effects on blood coagulation, it also exerts relevant effects on bone and the vascular system. In this review, we point out the relevance of an adequate vitamin K intake to obtain sufficient levels of carboxylated (active form) vitamin K dependent proteins (such as Osteocalcin and matrix Gla protein) to prevent bone health. Another bone-related action of Vitamin K is being a ligand of the nuclear steroid and xenobiotic receptor (SXR). We also discuss the reco

Inhibition of Midkine Augments Osteoporotic Fracture Healing.

The heparin-binding growth and differentiation factor midkine (Mdk) is proposed to negatively regulate osteoblast activity and bone formation in the adult skeleton. As Mdk-deficient mice were protected from ovariectomy (OVX)-induced bone loss, this factor may also play a role in the pathogenesis of postmenopausal osteoporosis. We have previously demonstrated that Mdk negatively influences bone regeneration during fracture healing. Here, we investigated whether the inhibition of Mdk using an Mdk-antibody (Mdk-Ab) improves compromised bone healing in osteoporotic OVX-mice. Using a standardized f

Rejuvenation of the inflammatory system stimulates fracture repair in aged mice.

Age significantly reduces the regenerative capacity of the skeleton, but the underlying causes are unknown. Here, we tested whether the functional status of inflammatory cells contributes to delayed healing in aged animals. We created chimeric mice by bone marrow transplantation after lethal irradiation. In this model, chondrocytes and osteoblasts in the regenerate are derived exclusively from host cells while inflammatory cells are derived from the donor. Using this model, the inflammatory system of middle-aged mice (12 month old) was replaced by transplanted bone marrow from juvenile mice (4

The Importance of Closed Reduction and Internal Fixation done with Intramedullary Implants in Pathological Fractures - A Case Series.

INTRODUCTION: Pathological fractures resulting from metastatic bone involvement, multiple myeloma (MM), and atypical femoral fractures present complex therapeutic challenges. Intramedullary (IM) nailing offers a biomechanically stable solution that facilitates early mobilization and enhances functional outcomes. This case series aims to evaluate the clinical and radiological results of IM fixation in pathological fractures, emphasizing pain relief and early functional outcomes. CASE SERIES: We report a series of five patients with pathological fractures of long bones who underwent IM nailing,

Approach to Suspected Physeal Fractures in the Emergency Department.

Growth plate (physeal) fractures are defined as a disruption in the cartilaginous physis of bone with or without the involvement of epiphysis or metaphysis. These represent around 15-18% of all pediatric fractures. It is important to diagnose physeal injury as early as possible, as misdiagnosis or delay in diagnosis may result in long term complications. Physeal injuries may not be initially obvious in children who present with periarticular trauma, and a high index of suspicion is important for diagnosis. Differential diagnosis for a Salter-Harris fracture includes a ligamentous sprain, acute

Association between Glucocorticoid-Induced Osteoporosis and Myasthenia Gravis: A Cross-Sectional Study.

PURPOSE: To investigate the association between glucocorticoid-induced osteoporosis and myasthenia gravis (MG) using a cross-sectional survey in Japan. METHODS: We studied 363 patients with MG (female 68%; mean age, 57 ± 16 years) who were followed at six Japanese centers between April and July 2012. We evaluated the clinical information of MG and fractures, bone markers, and radiological assessment. Quality of life was measured using an MG-specific battery, MG-QOL15. RESULTS: Glucocorticoids were administered in 283 (78%) of 363 MG patients. Eighteen (6%) of 283 MG patients treated with pre…

Clinical utility of tomosynthesis in suspected scaphoid fracture. A pilot study.

BACKGROUND: Radiography alone will not detect all scaphoid fractures. There is a reported prevalence between 9 and 33% of occult scaphoid fractures. The evidence-based literature suggests that magnetic resonance imaging (MRI) is the most suitable secondary imaging modality due to the ability to evaluate the bone marrow directly and to also identify other injuries. However, there is no consensus on the choice of follow-up imaging strategy-computed tomography, MRI, or bone scan-across different institutions. Tomosynthesis is a new digital tomographic method creating multiple thin tomographic sec

Case reports: subtrochanteric femoral stress fractures after prolonged alendronate therapy.

BACKGROUND: Alendronate is known for its ability to reduce bone loss in osteoporotic and osseous metastatic conditions. Its long-term effects remain unclear although several reports describe cases of proximal femur stress fractures associated with long-term alendronate use. CASE DESCRIPTION: We report the cases of four women who sustained low-energy subtrochanteric or femoral shaft stress fractures while being on alendronate therapy for more than 5 years. All radiographs showed typical patterns consisting of a transverse fracture line with external cortical bone reaction and medial cortical s

Relationship of lung function to severity of osteoporosis in women.

To determine if the severity of osteoporosis and its resultant hyperkyphosis cause measurable impairment of lung function, 74 women referred for osteoporosis evaluation underwent pulmonary function testing. Women with thoracic wedge compression fractures secondary to osteoporosis had significantly lower percent predicted FVC than did those without fractures. In hierarchical regression analysis, after controlling for age and arm span, there was a significant effect on FVC of the degree of hyperkyphosis as measured by Cobb's angle (increment in R2 = 0.14, p less than 0.001). The addition of the

Fragile balance: Managing bone health in people with intellectual disability and epilepsy, an english multi-site study.

BACKGROUND: People with intellectual disability (ID) experience poorer health outcomes than the general population, with epilepsy and polypharmacy contributing to further risks. Bone health is a neglected area, despite the established association between antiseizure medications (ASMs), (especially those cautioned by the UK Medicines and Healthcare products Regulatory Agency (MHRA)), and reduced bone mineral density. We aimed to evaluate prescribing practices, fracture risk, and bone health management in adults with ID and epilepsy attending specialist ID services. METHODS: A cross-sectional s

Increased susceptibility to microdamage in Brtl/+ mouse model for osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a genetic disease of collagen or collagen-related proteins that adversely impacts bone mass and fracture resistance. Little is known regarding the role that microdamage plays in OI and whether or not OI bone is more prone to damage accumulation than bone with unaffected collagen. The Brtl/+ mouse is a heterozygous model for OI which contains a Gly349Cys substitution in one COL1A1 allele, and demonstrates a low ductility phenotype. At 8 weeks of age, Brtl/+ demonstrates an increase in osteoclast number, which mimics the upregulated bone turnover often found in OI

Morphometric X-ray absorptiometry in the assessment of vertebral fractures in renal transplant patients.

BACKGROUND: Bone mineral density (BMD) is widely employed to evaluate the risk of fractures, but more than mineral content is bone quality that accounts for bone strength. In fact, occasionally, subjects with normal or only mildly reduced BMD also experience pathologic fractures. In metabolic bone disease, like renal transplantation, the agreement between fractures and BMD is less predictable. We assessed the prevalence of vertebral fractures by means of a new, DEXA-based method (morphometric absorptiometry, MXA) and compared its concordance with the two mostly employed radiological techniques

The Treatment of Periprosthetic Fractures Around the Knee in 2025: A Comprehensive Evidence-Based Approach.

Periprosthetic fractures around total knee arthroplasty are associated with significant morbidity and can be difficult to manage because of the presence of implants, compromised bone stock, and patient-specific comorbidities such as osteoporosis. The management of periprosthetic fractures requires a thorough understanding of epidemiology, classification systems, diagnostic approaches, and evidence-based treatment strategies. The treatment strategy is primarily dictated by implant stability and bone quality, influencing the choice between open reduction and internal fixation and revision total

Isolated trapezoid fractures: a case report with compilation of the literature.

Isolated fractures of the trapezoid bone have been rarely reported in the literature, the mechanism of injury being an axial or bending load transmitted through the second metacarpal. We report a case of an isolated, nondisplaced trapezoid fracture that was sustained by direct trauma and subsequently treated successfully in a short-arm cast. Diagnostic and treatment strategies for isolated fractures of the trapezoid bone are reviewed as well as the results of operative and nonoperative treatment.

Evaluation of multiple implant-bone parameters on stress characteristics in the mandible under traumatic loading conditions.

PURPOSE: The inter-relationships between various implant and bone parameters were evaluated for their influence on the von Mises stress distribution within the mandible using the finite element procedure. The maximum compressive stresses in cancellous and cortical bone were compared to the published stress-strain data to determine bone fracturing status when the maximum (traumatic) loading is applied. MATERIALS AND METHODS: Parameters considered herein include the implant diameter and length. Also considered are Young's modulus of cancellous bone and that of cortical bone, along with its thic

Investigation of distal femur microarchitecture and factors influencing its deterioration: An ex vivo high-resolution peripheral quantitative computed tomography study.

While fractures of the distal femur are often considered as fragility fractures, detailed knowledge of the bone microarchitecture at this skeletal site is largely unavailable. Initial evaluation of a patient cohort with distal femur fractures showed a markedly increased occurrence in elderly women. The purpose of this study was to determine the extent to which demographic characteristics of distal femur fractures are reflected by general age- and sex-specific variations in local microarchitectural parameters. Fifty cadaveric femora were collected from 25 subjects (12 females, 13 males, age 25-

[Indications for osteosynthesis of metatarsal fractures].

Fractures of metatarsal bones range from 4 to 5% of all fractures and to less than 1% of all injuries occurring. The usual treatment is non-operative by plaster cast. In case of special physical performance (profession, sports) the indication for operative treatment can be extended. This should be taken into consideration in dislocated multiple fractures as well as in single ones of the first and fifth metatarsal bone.

Protective effect of Du-Zhong-Wan against osteoporotic fracture by targeting the osteoblastogenesis and angiogenesis couple factor SLIT3.

ETHNOPHARMACOLOGICAL RELEVANCE: Du-Zhong-Wan (DZW) is a traditional Chinese medicine (TCM) composed of Eucommia ulmoides Oliv. and Dipsacus asper Wall. ex C.B. Clarke in the ratio 1:1. Based on the TCM theory, DZW nourishes the kidney to strengthen the bones. The literature research revealed that DZW possesses anti-fatigue, anti-depressant, and anti-osteoporotic properties. However, the action and mechanism of DZW on osteoporotic fracture remains slightly unclear. AIM OF THE STUDY: To evaluate the pharmacological effect of DZW on ovariectomized mice with an open femoral fracture and reveal th

TiNbSn alloy plates with low Young's modulus modulates interfragmentary movement and promote osteosynthesis in rat femur.

Orthopedic implants such as arthroplasty prostheses, fracture plates, and intramedullary nails often use materials like Ti6Al4V alloy and commercially pure titanium (CP-Ti), which have Young's modulus significantly higher than that of human cortical bone, potentially causing stress shielding and inhibiting effective fracture healing. TiNbSn alloy, a β-type titanium alloy with a lower Young's modulus (40-49 GPa), has shown promise in reducing stress shielding and enhancing bone healing by promoting effective load sharing with bone. This study used 5-hole plates made from TiNbSn alloy and CP-Ti
