Postural hypotension: its meaning and management in the elderly.

Aging is associated with a considerable number of alterations in function of the autonomic nervous system and with systems involved in the control of cardiovascular response to postural changes. However, these alterations themselves do not generally lead to symptomatic orthostatic hypotension. In combination with other factors, older patients can develop marked problems with orthostatic hypotension--notably, certain drug regimens, some degree of underlying heart failure, or such common geriatric illnesses as parkinsonism and diabetes. Treatment regimens must be designed to minimize side effect

When Minor Falls Are Overlooked in the Elderly: A Case Report of Vertebral Compression Fractures and Chronic Diaphragmatic Hernia.

Falls are common in the geriatric population, and the high prevalence of osteoporosis in this population increases the risk of their associated injuries. Such injuries can be subtle and be overlooked by the patient or even physicians. In this case, we present a 78-year-old woman who presented following a fall with associated acute vertebral compression fracture (VCF) and was found to have another chronic VCF and a chronic diaphragmatic hernia that had been overlooked for over two years. This case shall highlight that physicians must be astute when assessing the frail elderly who can develop se

[Fiber endoscopic evaluation of swallowing (FEES) - an important tool for diagnosing myasthenia gravis].

HISTORY AND ADMISSION FINDINGS: A 76-year old man was admitted with a globus sensation and weight loss for further investigations to our geriatric ward. A gastroscopic evaluation executed before had been unremarkable. INVESTIGATIONS: Physical examination was unremarkable, including a neurological examination. TREATMENT AND COURSE: All radiological findings (computer tomography of chest, abdomen and neck) were unremarkable. In a FEES investigation we found retentions and a reduction of frequency of swallowing after a while. During the investigation the patient complained about a muscular weak

[Assessment of cognition: dementia and delirium : In consideration of the AWMF guidelines 038-013 and 084-002LG].

Cognitive disorders are multifaceted and the range of neuropsychological instruments is correspondingly extensive; however, most examiners have to limit themselves to a small selection in order to master them safely. In geriatric patients the various forms and stages of dementia dominate. Delirium must be distinguished from these as an acute life-threatening event. The personal and external medical history as well as clinical observation are the first steps in the assessment of cognition; the selection of test procedures is graduated and bears the patient's benefit in mind. High-energy Femoral Neck Fractures in Young Patients.

Femoral neck fractures in physiologically young patients typically occur from high-energy axial loading forces through the thigh with the hip in an abducted position. These fractures have a high rate of associated head, chest, abdominal, and musculoskeletal injuries. High-energy hip fractures differ from traditional geriatric hip fractures regarding incidence, mechanism, management algorithms, and complications. After adequate resuscitation, goals of treatment include anatomic reduction and stable fixation while maintaining vascularity of the femoral head, which can be achieved through a varie

Sarcopenia: An Age-Related Multifactorial Disorder.

Sarcopenia is a disease characterized by a gradual decline in skeletal muscle mass and strength that accompanies the normal aging process. The disease is associated with various adverse health outcomes in the geriatric population, such as prolonged hospital admission, disability, poor quality of life, frailty, and mortality. Factors involved in the development of age-related sarcopenia include anorexia, hormone level alterations, decreased neural innervation, low muscular blood flow, cytokine dysregulation, altered mitochondrial activity, genomic instability, intracellular proteolysis, and ins

Peripheral Nerve and Plexus Blocks for Hip Surgery in a Nonagenarian with Severe Cardiac Disease.

The hemodynamic alterations and stress response associated with anesthesia and surgery are often poorly tolerated by elderly patients. Regional anesthesia techniques are useful in the elderly as they provide excellent perioperative analgesia with minimal hemodynamic perturbations. We report the case of a 90-year-old man with valvular heart disease and severe left ventricular systolic dysfunction, who underwent dynamic hip screw fixation of fractured femur neck under combined pericapsular nerve group block, lumbar plexus block, and para-sacral sciatic nerve block. We are not aware of any previo

[More attention should be paid to the influence of malnutrition on chronic limb threatening ischemia].

Chronic limb threatening ischemia (CLTI) is a severe condition of peripheral artery disease with high amputation and mortality. Pain and infection cause insufficient nutrient intake and protein loss in CLTI patients, while malnutrition is very prevalent in patients with CLTI and associated with poor prognosis.More and more studies have shown that malnutrition increases the risk of amputation and mortality, delays the time of wound healing.Moderate and severe malnutrition are independent risk factors for amputation and mortality in CLTI patients undergoing surgical or endovascular revascularizati

Managing Gallstone Disease in the Elderly.

Geriatric patients tend to have subtle presentations of biliary disorders and, if untreated, can decompensate acutely. Each biliary disorder warrants formulation of an individualized treatment plan with a multidisciplinary approach. Acute cholecystitis, a common complication of gallstones, is initially managed by conservative measures and subsequently, among patients with optimal surgical risk, through laparoscopic or open cholecystectomy. High-risk patients undergo temporization, percutaneous or endoscopic, followed by definitive intervention. Acute cholecystitis with complications (ie, perfo

[Chronic leg ulcers in older patients].

Chronic ulcers of the lower extremities are one of the most common medical problems encountered in western societies. The prevalence of leg ulcers is estimated to be 0.5-1.0% of the German population and is clearly associated with age. Therefore, in an aging society chronic leg ulcers are an important health issue with respect to increased morbidity and healthcare costs. The most frequent causes of leg ulcers are chronic venous insufficiency, peripheral arterial occlusive disease and diabetes mellitus. Efficient treatment necessitates an exact diagnosis and a close interdisciplinary collaborat

Treatment of aggressive non-Hodgkin's lymphoma in frail patients: cardiac comorbidities and advanced age.

The decisive factor in selecting a treatment regimen for a frail patient with aggressive non-Hodgkin's lymphoma is identifying whether a patient is fit enough to tolerate curative-intent anthracycline-containing regimens or too frail and therefore at risk of being undertreated. As cardiac comorbidities are an important contributor to both the health status and the selection of treatment, cardiovascular profiling and baseline risk stratification prior to treatment should be considered. Comprehensive geriatric assessment is an efficient means of identifying elderly patients with non-Hodgkin's ly

[Treatment of pain in people with dementia].

The causes of pain must always be clarified, as long as it can be justified. In dementia the placebo effect can be lacking. A treatment on demand is difficult to implement. Correctly applied nonmedicinal treatment has few unwanted effects but mostly needs constant support by personnel. Medications with anticholinergic effects should be used with caution due to the high risk of delirium and falling. With analgesics and coanalgesics the principles of geriatric treatment must also be adhered to: start low, go slow. Complaints that can be triggered by analgesics or coanalgesics should be recorded

Pelvic Ring Fractures: Role of Physical Therapy in the Emergency Department.

An 87-year-old woman with a history of osteoporosis presented to the emergency department with complaints of low back pain and difficulty walking due to falling onto her left hip 2 weeks prior. Given the delay in diagnostic evaluation and persistent symptoms, computed tomography (CT) of the lumbar spine was ordered. The results of the CT were read as noncontributory and a physical therapist was consulted. Following examination, the physical therapist recommended left hip radiographs. A CT scan of the hip, which was ordered due to its superior specificity in detecting subtle fractures and to as

Hair and nail diseases in the mature patient.

The elderly population is growing, lifespans are increasing, and a greater emphasis on geriatric care is being implemented in hospital systems. With a higher percentage of the population living longer, hair and nail diseases associated with the advanced stages of life are becoming more prevalent. Common hair diseases in the elderly include androgenetic alopecia, senile alopecia, frontal fibrosing alopecia, and erosive pustular dermatosis of the scalp. Nail diseases associated with advanced age include onychomycosis, brittle nails, onychocryptosis, onychoclavus, onychogryphosis, subungual hemat

A case of lithium-induced parkinsonism presenting with typical motor symptoms of Parkinson's disease in a bipolar patient.

Lithium is a mood stabilizer rarely associated with drug-induced parkinsonism (DIP). We present a case of an elderly woman with bipolar disorder who developed parkinsonian symptoms after chronic lithium administration despite therapeutic serum levels. Upon evaluation, classic parkinsonian signs of muscle rigidity, tremor, bradykinesia, freezing of gait, and cognitive decline were observed. Initially, she was diagnosed with Parkinson's disease (PD); however, DaTscan SPECT imaging clarified the diagnosis as DIP. As the daily lithium dosage was reduced, the patient's motor symptoms improved. This

Sarcopenia in cases of chronic and acute illness. A mini-review.

BACKGROUND: Loss of muscle mass and muscle weakness are often found in cases of acute or chronic illness in elderly patients. Sarcopenia is a risk factor for complications and higher mortality. Based on an exact diagnosis and knowledge of the risk factors for developing sarcopenia, it is now possible to improve the prognosis by providing effective treatment options. METHODS: This review was carried out based on a PubMed search in the period from 1998 to 2015 using original articles and reviews and posting the terms "sarcopenia", "elderly" and "acute illness". CONCLUSION: Given the evidence f

Clinical implications from drug-drug and drug-disease interactions in older people.

This clinical review summarizes the evidence in relation to clinical outcomes from drug-drug and drug-disease interactions in older people. Exposure to drug-drug interactions is associated with increased risk of hospitalization-related outcomes in older people. Drug-disease interactions have been linked with adverse drug interactions and adverse drug events in studies of older people. Although the prevalence of drug-drug and drug-disease interactions is common in older people, there are very limited empirical data on important clinical outcomes from drug-drug and drug-disease interactions. Update on newer antipsychotic drugs.

This article briefly reviews the novel atypical second-generation antipsychotic drugs iloperidone (Fanapt®), asenapine (Saphris®), and lurasidone (Latuda®), all of which have been approved by the U.S. Food and Drug Administration since 2009. Each is indicated for the treatment of schizophrenia, and asenapine has an additional indication for bipolar disorder. Very little information is available on their use in other disorders, pediatric and geriatric patients, and during pregnancy and breastfeeding. Their overall efficacy is no different than other antipsychotic drugs, but they do have differe
