Philanthropic donor perspectives about providing harm reduction services for people living with HIV/AIDS in a hospital setting.

BACKGROUND: Hospital-based harm reduction services are needed to reduce drug-related harms, facilitate retention in care, and increase medical treatment adherence for people who use drugs. Philanthropic donor support plays a key role in delivering such innovative services which might fall outside current funding streams. However, little is known about how the principles, implementation, and practice of harm reduction services, which are often highly stigmatized, may impact donor behaviours. We explored this issue within Casey House, a speciality hospital in Toronto, Canada. METHODS: Our mixed

Incidence of Otitis Media with Effusion in Cases of Head and Neck Malignancies Undergoing Radiotherapy: A Prospective Observational Study.

Background. The gram-negative Neisseria is associated with two of the most potent human epidemic diseases: meningococcal meningitis and gonorrhoea. In both cases, disease is caused by bacteria colonizing human mucosal membrane surfaces. Overall, the genus shows great diversity and genetic variation mainly due to its ability to acquire and incorporate genetic material from a diverse range of sources through horizontal gene transfer. Although a number of databases exist for the Neisseria genomes, they are mostly focused on the pathogenic species. In this present study we present the freely avail

Investigation of a Scabies Outbreak in Drought-Affected Areas in Ethiopia.

The impact of the severe drought in Ethiopia, attributed to El Niño weather conditions, has led to high levels of malnutrition that have, in turn, increased the potential for disease outbreaks. In 2015, Ethiopia faced a scabies outbreak in drought-affected areas where there was a shortage of safe water for drinking and personal hygiene. Following a house-to-house census to assess the prevalence of scabies, a detailed study was conducted looking at the disease burden. Following the outbreak report, training was provided on scabies identification and management for zonal and district health offi

Perches and elevated platforms in commercial broiler farms: use and effect on walking ability, incidence of tibial dyschondroplasia and bone mineral content.

Modern fast-growing broilers spend excessive periods resting and their activity further decreases with age. Inactivity has been suggested to increase impaired gait and the incidence of leg disorders. Tibial dyschondroplasia (TD) is a common leg pathology in broilers. A more complex environment might facilitate more activity and improve leg health. Perches or elevated platforms bring variety to broilers' environment and could motivate more locomotion. This study examined the impact of perches and elevated platforms on walking ability, the occurrence of TD and level of bone ash and mineral conte

Assessing the Impact of Sociodemographic Factors on Artificial Intelligence Models in Predicting Dementia: Retrospective Cohort Study.

BACKGROUND: Artificial intelligence (AI) is increasingly applied to health care, yet concerns about fairness persist, particularly in relation to sociodemographic disparities. Previous studies suggest that socioeconomic status (SES) and sex may influence AI model performance, potentially affecting groups that are historically underserved or understudied. OBJECTIVE: This study aimed to (1) assess algorithmic bias in AI-driven dementia prediction models based on SES and sex (biological sex), (2) compare the utility of an individual-level SES measure (the Housing-Based Socioeconomic Status [HOUS…

Community Hospital Interventions Addressing the Medical and Social Needs of Patients : Patient Perspectives From the CHART Investment Program.

High utilizers of acute care in nonurban settings are at risk for poor health outcomes. Much of Massachusetts is nonurban, with many residents experiencing limited access to health care providers, fragmented health care services, inadequate housing, and low health literacy. This study examines patient perspectives on the Community Hospital Acceleration, Revitalization, and Transformation (CHART) investment program, a state-based grant program focused on advancing community hospitals toward value-based care. We found that CHART staff engaged patients in care coordination and patient advocacy, p…

Experience of Visiting an Urban Slum: A Medical Student's Perspective.

Urban slums are densely populated areas characterized by inadequate housing, poor sanitation, and limited access to essential services. As part of the MBBS curriculum at Patan Academy of Health Sciences (PAHS), we visited an urban slum in Balkhu to assess the community's health-seeking behaviour, water facilities, and education. This article highlights the significance of such postings in equipping medical students with critical skills, including empathy, teamwork, and leadership, necessary for addressing the socio-economic determinants of health. The experience provided an invaluable opportun…

Building Power on "Mass&Cass": A Community-Centered Approach to Addressing Health Resource Gaps for Persons Experiencing Homelessness in Boston, MA, 2021.

In November 2021, two grassroots organizations in Boston, Massachusetts-a housing and health justice organization and a student-led nonprofit-established an initiative to provide persons experiencing homelessness (PEH) near the Massachusetts Avenue and Melnea Cass Boulevard ("Mass&Cass") intersection in Boston with access to free COVID-19 education and other wrap-around services. They partnered with hospitals, public health organizations, and advocacy groups to make this happen. This community-driven initiative serves as a model for how to enact a sustainable pipeline for PEH to receive health…

A User-Friendly Technique for Replacing Nylon Inserts of a Locator Attachment.

Rehabilitation of edentulous patients with implant-supported overdentures has gained prominence over the use of conventional dentures because of the improved quality of life it provides patients. Various attachment systems are available that facilitate the insertion and removal of prostheses, and among them, the Locator attachment system has become popular due to its self-aligning property, simplicity of use, and minimal space requirement of male inserts within the denture. However, the loss of retention of nylon inserts over time requires their removal from the metal housing and subsequent re…

Making sense of plasma cell heterogeneity.

Plasma cells (PCs) are essential for the quality and longevity of protective immunity. The canonical humoral response to vaccination involves induction of germinal centers in lymph nodes followed by maintenance by bone marrow-resident PCs, although there are many variations of this theme. Recent studies have highlighted the importance of PCs in nonlymphoid organs, including the gut, central nervous system, and skin. These sites harbor PCs with distinct isotypes and possible immunoglobulin-independent functions. Indeed, bone marrow now appears unique in housing PCs derived from multiple other o…

Brain Imaging Databases.

The availability of neuroimaging-based databases is helping immensely to understand the brain function in healthy and diseased conditions. This viewpoint highlights the objectives, commonalities, and differences within these existing databases and pointers for researchers to choose a particular database. We introduce a multimodal multidisease database, SWADESH, and its comparison with the existing databases. A futuristic database blueprint is proposed for housing multidisease, multimodal, and longitudinal brain imaging data systematically organized in a matrix form along with neuropsychologica…

Homelessness in the United States: Implications for Critically Ill Children.

Homelessness is a growing crisis in the United States (U.S.). Across the country, children represent a large proportion of the homeless population. When these children experience critical illness, it poses significant and specific burdens to the child and family, compounded by the social stressors inherent in being housing insecure. Yet research on homelessness in critically ill children remains limited. Here, we provide an overview of the current U.S. homeless population, discuss what is currently known about homelessness and critical illness to inform future research, and close with a propos…

"He Needed Just About Everything": Caring for Aging Adults Postincarceration.

METHODS: We conducted a thematic analysis on semi-structured interviews with 11 caregivers. RESULTS: Caregivers discussed how prior incarceration coupled with aging present barriers to housing, employment, and safety-net benefits-making caregiving more difficult. Caregivers assist their older care recipients to develop essential life skills (e.g., scheduling tasks, applying for services) and navigate the dynamic aging process (e.g., loneliness, illness). Caregivers struggle to gain care recipients' trust, often due to their past trauma. DISCUSSION: Caregivers play essential roles supporting …

Religious and Spiritual Practices Among Home-less Urban American Indians and Alaska Natives with Severe Alcohol Problems.

Engagement in religious and spiritual practices may be protective for homeless individuals with alcohol-related problems. However, little is known in this regard for urban-dwelling American Indians and Alaska Natives (AI/ANs) who have disproportionately high rates of homelessness and co-occurring alcohol use problems. Using secondary data from a nonrandomized controlled study testing a Housing First intervention, AI/AN participants (n = 52) and non-AI/AN participants (n = 82) were compared on demographic variables, alcohol use problems, religious affiliations, and religious/spiritual practices…

Homeless Children and Their Families' Perspectives of Agency Services.

While numerous programs aim to mediate the risks for children experiencing homelessness, there is a lack of research representing the children's and parents' perspectives in supportive housing programs. With this phenomenological qualitative study, the authors share the voices of 22 participants, including children and their families, regarding their experiences while receiving services from a homeless agency. Participating parents perceived that the program provided resources to the children that they could not provide themselves, opportunities for exposure to positive new experiences, and im…

"Couch surfing" of Latino foster care alumni: reliance on peers as social capital.

Youth exiting foster care often experience difficulties transitioning into adulthood. This paper focuses on Latino foster care youth in a major southwestern U.S. city and addresses the importance of peer networks as a crucial form of social capital as youth leave foster care. Case studies illustrate experiences of foster care alumni ranging in age from 18 to 26. Findings suggest that lack of housing forces youth into residential mobility or "couch surfing" and episodes of homelessness. Familial connections continue to be important to Latino youth. When Latino youth are unsuccessful in re-estab…
