Immune thrombocytopenic purpura increased risk of subsequent pancreatitis: A Nationwide population cohort study.

Immune thrombocytopenic purpura (ITP) is characterized by thrombocytopenia and bleeding diathesis. Pancreatitis is a very rare complication but may be fatal. We analyzed data of newly diagnosed ITP patients, excluding those with a history of splenectomy, unknown sex or date of birth, or preexisting pancreatitis at the time of ITP diagnosis, and compared these with selected age-, gender-, and index-year-matched controls, using the Taiwan National Health Insurance Research Database from 1996 to 2013. The study enrolled 100,177 ITP patients and 100,177 controls. We found that pancreatitis risk wa

Atypical secondary syphilis presentation in a patient with human immunodeficiency virus infection: a case report.

INTRODUCTION: Untreated syphilis may lead to severe complications. This infection has recently re-emerged in developed countries with a high number of cases coinfected with human immunodeficiency virus. In these patients, the skin lesions of secondary syphilis can be very atypical. CASE PRESENTATION: We report the case of a 38-year-old Bulgarian homosexual man who was coinfected with human immunodeficiency virus and syphilis. His skin contained multiple extensive necrotic lesions with abundant purulent secretion that covered his face, lips, scalp, and torso. Initial clinical diagnoses include

A causal mechanism for childhood acute lymphoblastic leukaemia.

In this Review, I present evidence supporting a multifactorial causation of childhood acute lymphoblastic leukaemia (ALL), a major subtype of paediatric cancer. ALL evolves in two discrete steps. First, in utero initiation by fusion gene formation or hyperdiploidy generates a covert, pre-leukaemic clone. Second, in a small fraction of these cases, the postnatal acquisition of secondary genetic changes (primarily V(D)J recombination-activating protein (RAG) and activation-induced cytidine deaminase (AID)-driven copy number alterations in the case of ETS translocation variant 6 (ETV6)-runt-relat

What's new in autoinflammation?

The pathogenesis of autoinflammatory diseases has shed light on the concept of inflammation in general and on our understanding of the role of the innate immune system. The autoinflammatory diseases have a large spectrum with varying features of inflammation. The most common autoinflammatory diseases are those associated with periodic fevers. The delay in diagnosis of these four common diseases (familial Mediterranean fever, cryopyrin-associated periodic fever syndrome, mevalonate kinase deficiency, and TNF receptor-associated periodic fever syndrome) results in secondary amyloidosis of the ki

Amantadine for treatment of fatigue in Guillain-Barre syndrome: a randomised, double blind, placebo controlled, crossover trial.

OBJECTIVE: Fatigue is a major complaint in patients with immune mediated polyneuropathies. Despite apparently good physical recovery after Guillain-Barré syndrome (GBS), many patients remain restricted in daily and social activities, and have a decreased quality of life. In this trial, the effect of amantadine on severe fatigue related to GBS was studied. METHODS: During the pre-treatment phase, all patients were monitored for 2 weeks. Only patients with severe fatigue, defined as a mean fatigue score of > or = 5.0 on the Fatigue Severity Scale (FSS), were randomised for this double blind, pl

Anti-very late antigen-1 monoclonal antibody modulates the development of secondary lesion and T-cell response in experimental arthritis.

Rats injected in the hind paw with a mixture of Mycobacterium butirricum emulsified in mineral oil (FA) developed a severe polyarthritis that shared some immunological features with human rheumatoid arthritis. After this local administration, rats developed a secondary lesion (edema) in the contralateral paw, which is a hallmark of immune system activation. In vivo intravenous treatment with a monoclonal anti-very late antigen (VLA)-1 antibody (HA31/8) significantly reduced the edema formation in the contralateral paw. T cells isolated from contralateral paw draining lymph nodes of FA rats tre

Timeliness of routine childhood vaccination in the Democratic Republic of the Congo: data from national vaccine surveys in 2021/2022, 2023 and 2024.

BACKGROUND: While crude vaccine coverage is a frequently reported coverage measure in the Democratic Republic of the Congo (DRC), timeliness of vaccine delivery is comparatively understudied. To achieve full vaccine benefits, both high coverage and timely delivery are required. This analysis investigated the timeliness of childhood vaccinations and associated factors across three national Vaccine Coverage Surveys conducted in 2021/2022, 2023, and 2024 in the DRC. METHODS: We conducted a secondary analysis of vaccination coverage data from 79,761 children aged 12-23 months. Weighted coverages

Determinants of tetanus-unprotected births in Tanzania: a multilevel analysis using the 2022 Tanzania Demographic and Health Survey data.

BACKGROUND: Tetanus is a preventable bacterial disease that poses a significant public health challenge, particularly in low-income regions where immunization rates are low and hygienic birth practices are often inadequate. Neonates are especially vulnerable when their mothers lack sufficient antitoxins due to incomplete vaccination. Although Tanzania has achieved the elimination of maternal and neonatal tetanus, challenges in attaining high vaccination coverage persist. Therefore, this study aimed to investigate the determinants of tetanus-unprotected births in Tanzania. METHODS: The study wa

Long-Term Follow-Up After Surgical Treatment of Anogenital and Oropharyngeal Condyloma Acuminata with Simultaneous HPV Vaccination Initiation in Patients with Concurrent HPV-16 or -18 Co-Infection.

Background/Objectives: Persistent infection with high-risk HPV genotypes accounts for the development of a substantial portion of anogenital and oropharyngeal malignancies. Patients with primary and secondary immunodeficiency or those receiving immunosuppressive therapy, including transplant recipients and patients affected by rare diseases, are vulnerable to particularly severe clinical manifestations of infection. While HPV vaccination constitutes primary prevention, patients with HPV-associated disease remain at risk of reinfection or subsequent infection with alternative genotypes followin

Bioinformatics-driven design and protective efficacy assessment of a multi-epitope vaccine for Toxoplasma gondii.

INTRODUCTION: Toxoplasma gondii can cause toxoplasmosis. It is an important type of pathogen within the broad category of emerging and re-emerging zoonoses. As an infectious disease featuring a complex multi-host transmission cycle, it poses an increasingly severe threat to global public health. No licensed vaccines are currently available for pets and humans, and thus a novel high-efficiency vaccine is urgently required. METHODS: Six antigens (GRA1, MIC17A, OWP2, LEA880, LEA870, and a hypothetical protein LEA530) representing different stages of the parasite lifecycle were selected from Toxo

Is health expenditure on immunisation associated with immunisation coverage in sub-Saharan Africa? A multicountry analysis, 2013-2017.

OBJECTIVES: The designing of contextually tailored sustainable plans to finance the procurement of vaccines and the running of appropriate immunisation programmes are necessary to address the high burden of vaccine-preventable diseases and low immunisation coverage in sub-Saharan Africa (SSA). We sought to estimate the minimum fraction of a country's health budget that should be invested in national immunisation programmes to achieve national immunisation coverage of 80% or greater depending on the context, with and without donors' support. DESIGN: Multicountry analysis of secondary data usin

Nasopharyngeal Carriage of Streptococcus pneumoniae Among Young Children in Haiti Before Pneumococcal Conjugate Vaccine Introduction.

BACKGROUND: Streptococcus pneumoniae, or pneumococcus, is a leading cause of morbidity and mortality in children worldwide. Pneumococcal conjugate vaccines (PCV) reduce carriage in the nasopharynx, preventing disease. We conducted a pneumococcal carriage study to estimate the prevalence of pneumococcal colonization, identify risk factors for colonization, and describe antimicrobial susceptibility patterns among pneumococci colonizing young children in Port-au-Prince, Haiti, before introduction of 13-valent PCV (PCV13). METHODS: We conducted a cross-sectional study of children aged 6-24 months

[Vaccinations for immunocompromised hosts – focussing on patients after a hematological stem cell or organ transplantation, with HIV or with functional or anatomical asplenia].

Patients with an acquired immune deficiency, for example due to HIV-infection, after a solid organ or haematological stem cell transplantation or due to functional or anatomical asplenia, have a greater risk to experience severe complications or a chronic course of infection compared to healthy individuals. Vaccinations would pose an ideal primary preventive method. However, their efficacy is reduced if applied during the immunosuppressed period. Therefore, whenever possible, vaccinations should be administered before the period of immunosuppression starts – or caught up later during the perio
