Efficacy and safety of haemadsorption combined with continuous renal replacement therapy for rhabdomyolysis and acute kidney injury: a single-center, open-label, randomised controlled trial protocol.

INTRODUCTION: Myoglobin (Mb) exerts both direct and indirect nephrotoxic effects, contributing to the progression of kidney injury. For patients with rhabdomyolysis (RM) and acute kidney injury (AKI) requiring renal replacement therapy (RRT), Mb clearance is a critical therapeutic goal. Recent studies have indicated that haemoadsorption (HA) combined with continuous renal replacement therapy (CRRT) is an effective strategy for removing circulating Mb. However, clinical data regarding the efficiency of Mb clearance and long-term patient outcomes with this approach remain limited. This study aim

Clinical and metabolic phenotyping of continuous versus intermittent ENteral NUTrition in ventilated adults with shock: ENNUT trial protocol.

INTRODUCTION: Critically ill patients often require enteral nutrition, but the optimal feeding strategy-continuous or intermittent-remains uncertain. While continuous enteral nutrition ensures steady nutrient delivery, it may inhibit key metabolic and cellular processes such as autophagy and ketogenesis. Intermittent enteral nutrition, by mimicking fasting periods, could activate protective pathways, potentially improving clinical outcomes. However, evidence for its efficacy and safety in intensive care units (ICUs) is limited. This study evaluates the clinical and metabolic impacts of fasting

Life within: Unraveling the patterns and correlates of smokeless tobacco use in pregnant women across India.

OBJECTIVES: Tobacco use during pregnancy is of significant concern as it is associated with adverse outcomes in expecting mothers and their fetuses. Smokeless tobacco use is widespread among women in India. However, there is a dearth of studies on smokeless tobacco use and its determinants during pregnancy in India. The study examines the prevalence and predictors of smokeless tobacco use, quit attempts, advice from healthcare providers to quit, and intentions to quit among pregnant women in India. STUDY DESIGN: The study utilizes the Global Adult Tobacco Survey (GATS) 2 (2016-17) India data.

Health implications of established and emerging stressors: design of the prospective New Jersey Population Health Cohort (NJHealth) Study.

INTRODUCTION: Some stressors, like the death of a partner, are common and rigorously studied, while others, such as those related to climate change or social media, are just emerging and in need of systematic research. The New Jersey Population Health Cohort (NJHealth) Study aims to characterise established and emerging stressors and delineate the pathways through which they influence health, especially among groups likely to experience chronic exposure to stressors including immigrants, people of colour, multigenerational families and low-income families. METHODS AND ANALYSIS: A prospective

Feasibility study protocol: implementing consultation-based high-quality palliative care services in intensive care units.

INTRODUCTION: Critically ill patients in intensive care units (ICUs) receive life-sustaining treatments aimed at restoring or maintaining organ function. ICU admission often involves substantial multidimensional suffering that can burden patients, their families and surrogates. Multidisciplinary palliative care support can help alleviate their sufferings. In South Korea, however, palliative care has not yet been integrated into critical care settings, highlighting the need to explore the feasibility of its implementation within the ICU. METHODS AND ANALYSIS: This study aims to test the feasib

Insights from an invited expert panel using a World Café: Establishing a national human biomonitoring programme communication strategy.

Human biomonitoring (HBM) is a pivotal tool for assessing chemical exposures and guiding regulatory decisions to safeguard public and environmental health, particularly within the framework of the European Union's Chemicals Strategy for Sustainability and other EU policies for chemical safety. While many national HBM programmes include communication components, there is limited literature on how these strategies are systematically operationalised, evaluated, and scaled, or how participatory engagement, risk communication, and policy-oriented dissemination can be adapted to specific national co

Drink lemonade! Implementation of drinking 2 h before elective orthopedic surgery to reduce prolonged fasting.

BACKGROUND: Prolonged fasting before surgery remains common despite guidelines recommending liquid fasting for 2 h and solid fasting for 6 h preoperatively. Consuming carbohydrate-rich drinks until 2 h before surgery improves patient well-being and reduces postoperative complications and insulin resistance. Given the persistence of implementation barriers, including logistical challenges and insufficient patient education, this study aimed to implement drinking up to 2 h before elective orthopedic surgery. METHODS: The Grol and Wensing implementation approached was followed in a before-after

From Pandemic to Practice: How COVID-19 Has Reshaped Haemostasis in Cardiac Surgery: A Narrative Review.

The utilisation of cardiopulmonary bypass (CPB) during cardiac surgery is often associated with complex haemostatic perturbations, frequently manifesting as a paradoxical risk of both bleeding and thrombosis. This is postulated to be driven by systemic inflammation, endothelial activation and contact activation of the coagulation cascade due to extracorporeal circulation. However, the coronavirus disease 2019 (COVID-19) pandemic revealed a unique hypercoagulable state, termed COVID-19-associated coagulopathy (CAC), also observed in those vaccinated against COVID-19. CAC displays similar physio

Effect of high-flow nasal oxygen therapy on cardiac cycle efficiency in coronary artery disease patients undergoing endoscopy: study protocol for randomised controlled trial.

INTRODUCTION: Patients with coronary artery disease (CAD) have diminished tolerance for gastrointestinal endoscopy procedures (upper/lower) performed under deep sedation, resulting in increased incidence rates of hypotension and myocardial ischaemia among these patients. Avoiding hypoxemia caused by deep sedation and increased oxygen consumption caused by inadequate sedation is particularly important in these patients. Based on recent data indicating that high-flow nasal oxygen therapy (HFNO) is beneficial for preventing hypoxaemia in high-risk patients, this study investigated whether HFNO ca

Enteric-coated Mycophenolate Sodium plus hydroxychloroquine therApy versus hydroxychloroquine for the Remission of Proteinuria in IgA Nephropathy (EMSAR-IgAN trial): study protocol for a randomised trial.

INTRODUCTION: The management of immunoglobulin A (IgA) nephropathy remains a topic of debate. Hydroxychloroquine and mycophenolate mofetil (MMF) are two immunosuppressive agents that have recently garnered increased attention among patients with IgA nephropathy in China. Several studies have shown the comparable efficacy between MMF and enteric-coated mycophenolate sodium (EC-MPS), with lower adverse event rates for EC-MPS. The present study aims to evaluate the efficacy and safety of EC-MPS combined with hydroxychloroquine as an immunosuppressive regimen for patients with high-risk progressiv

Anxiolysis for laceration repair in children: study protocol for an open-label multicenter adaptive trial (ALICE).

BACKGROUND: Lacerations are the most common traumatic reason for children to visit an emergency department (ED), accounting for almost half of all procedures performed. Children experience considerable distress during laceration repair, despite routine application of local anesthetic. Pharmacologic anxiolysis may mitigate the negative practice of forcefully restraining a child, however, evidence for the most effective agent is lacking. We aim to determine the most effective anxiolytic agent for laceration repair in children. METHODS: This is a multicentre, phase III, three-arm, adaptive, rand

Effectiveness of peritoneal lavage fluid circulating tumour cells and circulating tumour DNA in the prediction of metachronous peritoneal metastasis of gastric cancer (pT4NxM0/pT1-3N+M0) after radical resection: protocol of a prospective single-centre clinical study.

BACKGROUND: Gastric cancer (GC) is the fourth leading cause of cancer mortality worldwide. Peritoneal metastasis (PM) is a significant cause of death in patients with GC, and presents a major challenge in clinical diagnosis and treatment. Predicting the occurrence of PM in high-risk patients, and diagnosing and treating PM in advance to improve patient survival, remains an unsolved problem in clinical practice. Given the low positive rate of cytology and difficulty in diagnosing occult PM, new molecular markers and detection technologies for early diagnosis require urgent validation. The prima
