Fatal haemophagocytic lymphohistiocytosis and atypical haemolytic uraemic syndrome following coronavirus disease 2019 infection.

A previously healthy woman in her early 30s developed rash, fever and arthralgia 3 weeks after SARS-CoV-2 infection, rapidly progressing to multiorgan failure. Laboratory studies revealed severe thrombocytopenia (23×109/L), microangiopathic haemolytic anaemia, acute kidney injury and ferritin >100 000 ng/mL. Kidney biopsy demonstrated thrombotic microangiopathy with negative antinuclear antibody, supporting atypical haemolytic uraemic syndrome (aHUS). Concurrently, low haptoglobin, elevated lactate dehydrogenase (2500 U/L), interleukin-2 receptor (18 644), triglycerides (425 mg/dL) and splenom

Necrotising fasciitis in chronic lymphocytic leukaemia: a diagnostic challenge, management and learning points.

Necrotising soft tissue infection (NSTI), or necrotising fasciitis (NF), is a life-threatening, rapidly spreading bacterial infection affecting soft tissues. Mortality rates can be up to 70%, emphasising the need for early diagnosis and intervention. However, timely identification remains challenging, particularly in immunocompromised patients, due to nonspecific early signs. This case involves a patient who developed NF 12 days after R-CHOP (Rituximab, Cyclophosphamide, Hydroxydaunorubicin, Oncovin, Prednisolone) chemotherapy. Despite clinical and radiological evidence of NF and a high Labora

Primary renal echinococcosis.

Hydatid disease is caused by Echinococcus species, which are a flat worm with three evolutionary stages during its life cycle. Renal echinococcal infection is an uncommonly encountered infection in northwest China but is endemic in many parts of the world. Renal echinococcosis accounts for about 2-4% of confirmed cases of cystic echinococcosis. We reported three cases of renal echinococcosis presenting with renal colic, which were confirmed by ultrasound, computed tomography (CT), magnetic resonance imaging (MRI), and laboratory examination. The treatment of choice for renal hydatid cyst is su

Osteomyelitis and aortic arteritis with thrombosis as primary manifestations of severe paracoccidioidomycosis: a case report.

Paracoccidioidomycosis (PCM) configures a deep mycosis caused by Paracoccidioides spp., a neglected tropical disease. We describe a fatal case of PCM that manifested itself as osteomyelitis with thrombosis in the iliac artery in a man with frequent contact with an endemic region in Sao Paulo, Brazil. A 67-year-old man living in an endemic area presented with osteomyelitis of the femur and iliac artery thrombosis on admission to the hospital. Computed tomography of the chest showed multiple cavitated pulmonary nodules. The patient rapidly progressed to irreversible respiratory failure. The auto

EBV associated epiglottitis in an immunocompetent child.

BACKGROUND: The article presents a case report of subacute epiglottitis in a healthy three-year-old child, secondary to Epstein-Barr Virus (EBV) infection. This case is particularly significant as EBV-related epiglottitis is extremely rare, with only one prior reported case in a pediatric patient. CASE PRESENTATION: A previously healthy three-year-old boy developed fever, cough, and difficulty breathing, initially diagnosed as croup but worsened, leading to hospital admission with respiratory distress, odynophagia and dysphagia. He had a hoarse voice, inspiratory stridor, swollen neck lymph n

Left middle cerebral artery occlusion associated with mycoplasma pneumonia in a child: A case report.

Cerebral infarction is a rare and severe manifestation of central nervous system damage caused by mycoplasma pneumoniae infection. We report that a 16-year-old girl was hospitalized with cough, expectoration and fever for 5 days and shortness of breath for 1 day. At the time of admission, the chest computed tomography showed double lung fields infiltration and pleural effusion. The detection of mycoplasma pneumoniae antibodies (IgG and IgM) were positive. The right limb movement of the patient was found incapacitated on the seventh day of hospitalization. Computed tomography, magnetic resonanc

Oligoclonal bands: a laboratory diagnosis of subacute sclerosing panencephalitis (SSPE).

The oligoclonal band indicates presence of antibodies specific to the disease, possibly due to the activation of certain clones of B lymphocytes. This intrathecal immunoglobin synthesis can be persistent for months to years, for example, in respons to paramyxoviruses, herpes virus, coxsackievirus, and Treponema pallidum; or can be synthesized for life, for example in multiple sclerosis and subacute sclerosing panencephalitis (SSPE). We report a case of SSPE in a 15-year-old male patient. The patient had myoclonic jerks that occurred in the thoracal femoral region. Necessary laboratory tests id

Detection of the serological markers hepatitis B virus surface antigen (HBsAg) and hepatitis B core IgM antibody (anti-HBcIgM) in the diagnosis of acute hepatitis B virus infection after recent exposure.

The serological diagnosis of acute hepatitis B virus (HBV) infection after recent exposure has been established by the hepatitis B virus surface antigen (HBsAg) and anti-hepatitis B core IgM antibody (anti-HBcIgM) detection in serum, sometimes accompanied by the detection of hepatitis B "e" antigen (HBeAg). Despite this characteristic serological profile, misdiagnosis can occur in cases of unexpected or atypical behavior of the serological markers in the bloodstream, or if the true meaning of its expression is not properly investigated, or even if there is a possibility of interference from fa

Antibiotic resistance pattern of Staphylococcus aureus with reference to MRSA isolates from pediatric patients.

AIM: The extent of methicillin-resistant Staphylococcus aureus (MRSA) infection in Nepalese children is largely unknown. MATERIALS & METHODS: Six hundred and seventy-two clinical samples collected from 232 patients between June and November 2016 were processed in a microbiology laboratory. RESULTS: Out of 300 culture-positive samples, 52 (17.3%) were S. aureus isolates. Among those 52, 39 (75.0%) were found to be MRSA. The infection rate of S. aureus was shown to be higher in inpatients (55.7%) compared with outpatients (44.3%) at p = 0.637, 95% CI. Thirteen types of antibiotics were used in

[Sporotrichoid nocardiosis by Nocardia brasiliensis].

Nocardia brasiliensis is a gram-positive, branched, aerobic, acid-resistant, rod-shaped bacillus that inhabits in soil, rotten organic matter and waters. Cutaneous nocardiosis in immunocompetent individuals can manifest in three different forms: actinomycetoma, superficial skin infection or lymphocutaneous infection. A case of an 85-year-old woman with an ulcerated lesion on the back of her left hand with erythematous papules in the lymphatic tract infected by N. brasiliensis is described. The microorganism was presumptively identified by conventional and inexpensive methods for a medium compl

Examining sociodemographic risk factors for Chlamydia trachomatis infection: a population-based cohort study.

AIM: Chlamydia trachomatis is the most prevalent bacterial sexually transmitted infection in developed nations and is an important public health concern. We aimed to assess the factors associated with testing and positivity of C. trachomatis in a large population. METHODS: A retrospective study of a large Canadian health region was undertaken using 2011 census and laboratory data. Demographic and socioeconomic data from the national household survey were linked to microbiologic data for C. trachomatis. Multivariable generalized estimating equation models were constructed to examine relative r

Liver granulomatosis: a case of Chlamydophila pneumoniae infection.

An 18-year-old man was referred to the Internal Medicine ward because of a 2-week history of intermittent high fever, weight loss and cough. Clinical examination revealed hepato-splenomegaly and multiple lymph nodes swelling while laboratory tests showed elevated C-reactive protein, gamma glutamyl transferase and lactate dehydrogenase. All serologic testes for auto-immune antibodies, viruses and bacteria were negative except for Chlamydophila pneumoniae. An 18-FDG PET computed tomography scanner showed hypermetabolism in the liver, spleen and lymph nodes. We therefore conducted a liver biopsy

[Rubella: a current issue?].

Sporadic cases of rubella infection are reported each year in France due to insufficient vaccination coverage. Rubella virus is a very unstable enveloped RNA virus. For this reason, transportation and storage of samples collected for its detection require particular conditions. The genetic stability of rubella virus has allowed the development of very effective vaccines. During the recent rubella outbreaks in Algeria and Tunisia, an unusual high rate of encephalitis was reported. The role of the laboratory is crucial in the management of rubella infection during pregnancy. Rubella serological

[Use of immunological and molecular biological methods to diagnose cerebral toxoplasmosis in HIV infection].

Cerebral toxoplasmosis is one of the leading causes of neurologic diseases with high mortality rates in patients with HIV infection. Invasion was difficult to diagnose for a number of objective reasons. The objective of the investigation was to determine the clinical sensitivity of different laboratory techniques as both a single study and their various combinations to verify the diagnosis of cerebral toxoplasmosis in HIV-infected patients. Blood and cerebrospinal fluid were tested in 51 patients with Stage 4B HIV infection (AIDS) with the verified diagnosis of cerebral toxoplasmosis. Separate

Listeriosis during Pregnancy: A Public Health Concern.

Listeria was first described in 1926 by Murray, Webb, and Swann, who discovered it while investigating an epidemic infection among laboratory rabbits and guinea pigs. The role of Listeria monocytogenes as a foodborne pathogen was definitively recognized during the 1980s. This recognition was the consequence of a number of epidemic human outbreaks due to the consumption of contaminated foods, in Canada, in the USA and in Europe. Listeriosis is especially severe in immunocompromised individuals such as pregnant women. The disease has a low incidence of infection, although this is undeniably incr
