Pharmacist-managed titration of urate-lowering therapy to streamline gout management.

The treat-to-target approach for serum uric acid is the recommended model in gout management according to the 2012 American College of Rheumatology (ACR) guidelines. Adherence to urate-lowering therapy (ULT) can be difficult for patients due to barriers, which include medication burden, financial hardship, and lack of medical literacy. Our aim was to create a pharmacist-managed referral for the titration of ULT to target serum uric acid (sUA) levels in a complex patient population. We utilized a clinical database to query patients seen at a rheumatology clinic over a 12-month period with an IC

Impact of a Psychological Opioid-Risk Evaluation on Opioid Prescribing in Primary Care.

BACKGROUND AND OBJECTIVES: The misuse and abuse of opioids has increased across the United States in recent years, associated with a rise in opioid-related overdose deaths. Physicians report having difficulty discerning substance abuse or drug diversion, which can lead to over- or under-prescribing of opioids and poor pain management. Additionally, research suggests that patient characteristics (eg, sex, ethnicity/race, age) may unduly influence the pain management decisions of health care providers. This investigation aimed to assist in physicians' prescribing decisions and reduce prescribing

Health at hand: A systematic review of smart watch uses for health and wellness.

INTRODUCTION: Smart watches have the potential to support health in everyday living by: enabling self-monitoring of personal activity; obtaining feedback based on activity measures; allowing for in-situ surveys to identify patterns of behavior; and supporting bi-directional communication with health care providers and family members. However, smart watches are an emerging technology and research with these devices is at a nascent stage. METHODS: We conducted a systematic review of smart watch studies that engaged people in their use by searching PubMed, Embase, IEEE XPlore and ACM Digital lib

The European TREatment of severe Atopic eczema in children Taskforce (TREAT) survey.

BACKGROUND: There is a paucity of evidence for the use of systemic agents in children with atopic eczema refractory to conventional therapy, resulting in considerable variation in patient management. OBJECTIVES: The European TREatment of severe Atopic eczema in children Taskforce (TREAT) survey was established to collect data on current prescribing practice, to identify factors influencing the use of specific systemic agents, and to inform the design of a clinically relevant intervention study. METHODS: Consultant physician members of the paediatric dermatology societies and interest groups

Foscarnet in the management of cytomegalovirus infections in hematopoietic stem cell transplant patients.

Despite significant advances in the day-to-day management of patients receiving hematopoietic stem cell transplantations, including the introduction of new antiviral drugs, cytomegalovirus (CMV) infection continues to be a major cause of morbidity and mortality. The aim of this article is to undertake a literature-based review of foscarnet in this therapeutic setting and to align current best-published evidence with recent recommendations presented at the European Conference on Infections in Leukaemia. Ganciclovir remains the mainstay of CMV infection/disease antiviral management protocols. Ho

Long-acting beta(2)-agonists in management of childhood asthma: A critical review of the literature.

This review assesses the evidence regarding the use of long-acting beta(2)-agonists in the management of pediatric asthma. Thirty double-blind, randomized, controlled trials on the effects of formoterol and salmeterol on lung function in asthmatic children were identified. Single doses of inhaled salmeterol or formoterol cause prolonged bronchodilatation (>12 h) and extended bronchoprotection against exercise-induced bronchoconstriction in children, some children achieving full protection for more than 12 h. Heterogeneity in bronchoprotection has been observed, and individual dose-titration ma

Management and Outcome of Invasive Clindamycin-Resistant MRSA Community-Associated Infections in Children.

BACKGROUND: Clindamycin resistance among community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) complicates the management of a challenging infection. Little data exist to guide clinicians in the management of invasive clindamycin-resistant CA-MRSA infections in children and studies using oral regimens such as trimethoprim-sulfamethoxazole (TMP-SMX) and linezolid for treatment of these infections are limited. We sought to reevaluate antibiotic management among invasive CA-MRSA at a tertiary children's hospital. METHODS: Cases of invasive clindamycin-resistant MRSA infectio

Case Report: Lymphangiogram and embolization for malignant chylothorax in cancer patients.

Tumor-related lymphatic obstruction can cause malignant chylothorax, which can be debilitating. Conventional management includes dietary modifications, percutaneous drainage, and medical management (octreotide), most of which prove refractory in high-output chylothorax cases. Lymphangiogram and embolization in such cases offers a minimally-invasive alternative; however, its use in non-iatrogenic malignant chylothorax is underreported. We present three cases of malignant chylothorax managed with lymphangiogram followed by therapeutic embolization. Case 1: A 70-year-old female with relapsed angi

Use of polyethylene glycolelectrolyte solution (GoLYTELY) for management of acute fecal impaction and chronic constipation in a cat.

A 13-year-old spayed female domestic shorthair cat was presented to the University of Wisconsin-Madison School of Veterinary Medicine (Madison, Wisconsin, USA) because of severe, persistent fecal impaction following cholecystoduodenoplasty performed 4 d earlier. The impaction did not resolve with common medical management, prompting initiation of polyethylene glycol-electrolyte solution (PEG-ES, GoLYTELY; Braintree Laboratories) via esophageal feeding tube. The cat showed rapid improvement, and PEG-ES was discontinued following resolution of the impaction. However, 23 d later, the cat was re-p

[Establishment of an Onco-Ophthalmology Collaboration for Tisotumab Vedotin and Initial Experience with Ocular Adverse Events].

BACKGROUND: Tisotumab vedotin (TV), an antibody-drug conjugate targeting tissue factor (TF), was approved in Japan in 2025 for the treatment of recurrent/metastatic cervical cancer. As TF is also expressed on the ocular surface, TV frequently causes ocular adverse events (OAEs), necessitating strict management. This paper reports on the established'onco-ophthalmology'collaboration system developed to manage these risks. METHODS: A multidisciplinary protocol involving medical oncologists, ophthalmologists, pharmacists, and nurses was implemented. Key components included pre-treatment ophthalmi

Kidney Injury in Children after Hematopoietic Stem Cell Transplant.

Hematopoietic cell transplant (HCT), used for treatment of many malignant and non-malignant pediatric diseases, is associated with serious complications, limiting this therapy's benefit. Acute kidney injury (AKI), seen often after HCT, can occur at different stages of the transplant process and contributes to morbidity and mortality after HCT. The etiology of AKI is often multifactorial, including kidney hypoperfusion, nephrotoxicity from immunosuppressive and antimicrobial agents, and other transplant-related complications such as transplant-associated thrombotic microangiopathy and sinusoida

Polycystic ovarian syndrome-current pharmacotherapy and clinical implications.

Polycystic ovary syndrome (PCOS), the most common endocrinopathy in women is characterized by polycystic ovaries, chronic anovulation and hyperandrogenism. The treatment in PCOS is mainly symptomatic and involves lifestyle interventions and medications such as Metformin, Oral contraceptives and Antiandrogens. However, the management of PCOS is challenging and current interventions are not able to deal with outcomes of this syndrome. This review encompasses latest pharmacotherapeutic and non-pharmacotherapeutic interventions currently in use to tackle various symptomatic contentions in PCOS. Ou

Understanding the use of immunosuppressive agents in the clinical management of IBD.

The advent of the biological era has seen many improvements in the management of inflammatory bowel disease (IBD). These agents, however, are not a ubiquitous panacea as they are neither universally available nor are they universally efficacious in the short or long-term. There is, therefore, still a need for other therapies and it is important to remember about the medications that have been effective in the past. The use of azathioprine and 6-mercoptopurine has been the mainstay of long-term therapy for many IBD patients for many years. Their role as steroid sparing agents and in the mainten

Seton management of complex anorectal fistulas in patients with Crohn's disease.

Anorectal fistulas associated with Crohn's disease are difficult to manage, particularly when the rectum is diseased. Significant morbidity has been associated with both medical and surgical therapy. Although conventional therapy is acceptable in the management of simple fistulas in Crohn's disease, these approaches often exacerbate rather than ameliorate problems in patients with complex fistulas. The authors report ten cases of complex fistulas in patients with Crohn's disease managed with their technique of long-term, indwelling setons. These setons are placed through the fistula tract and

Polycythemia vera as a cause of systemic hypertension.

An underlying medical condition as a cause for elevated blood pressure is suspected when hypertension occurs in younger children or those with markedly high readings (hypertensive urgency or emergency). We highlight a unique presentation and underlying medical condition, polycythemia vera (PV), as the cause of secondary hypertension in an asymptomatic 5-year-old child presenting for a routine physical examination and found to have hypertensive urgency. Laboratory evaluations revealed leukocytosis, polycythemia, thrombocytosis, and low erythropoietin level. JAK2-V617F mutational analysis confir

Technical challenges to anterior temporal lobectomy after laser interstitial thermal therapy for mesial temporal lobe epilepsy: technical note.

Mesial temporal sclerosis (MTS) is a frequent cause of medically refractory epilepsy, for which laser interstitial thermal therapy (LITT) is an effective treatment. However, experience with the technical considerations posed by additional surgery after an initial LITT procedure is lacking. The authors present the case of a 12-year-old female with medically refractory temporal lobe epilepsy and left MTS who underwent LITT at a separate institution prior to referral. This patient had no change in early postoperative seizure control (Engel class IVB) and then her seizures worsened despite ongoing

Evaluation and management of patients with chronic fatigue.

Chronic fatigue is a common and disabling problem in primary care practice. The differential diagnosis of chronic fatigue is extensive and includes medical disorders, altered physiologic states (eg, pregnancy, exertion), psychiatric disorders, lifestyle derangements, drugs, and controversial entities (eg, chronic candidiasis, food allergies, environmental illness, and chronic fatigue syndrome). The most common diagnoses are psychiatric disorders, including mood, anxiety, and somatoform disorders. A comprehensive approach to diagnosis and management is necessary, including structured psychiatri

[Expert consensus on the diagnosis, treatment, and management of liver nodules (version 2026)].

Liver nodules are one of the common and complex clinical conditions, with certain ones developing into malignant lesions, hence significantly affecting patient prognosis. The detection rate of liver nodules has significantly improved with advances in modern imaging technology; however, there is still a lack of unified guidance on their diagnosis, treatment, and management. The "Expert Consensus on the Diagnosis, Treatment, and Management of Liver Nodules (version 2026)" systematically summarizes and standardizes aspects of liver nodules, including definition, classification, screening, diagnos

Clinical diagnosis and management of spinocerebellar ataxia in a resource-constrained setting: a case report from Eastern Nepal.

INTRODUCTION: Spinocerebellar ataxias (SCA) are a diverse group of neurodegenerative disorders with autosomal dominant inheritance, primarily affecting the cerebellum and its connections. Diagnosis typically involves genetic testing, but in resource-limited settings, clinical and neuroimaging assessments become critical. This case report highlights the role of nongenetic methods in diagnosing SCA and outlines management strategies in such settings. CASE PRESENTATION: A 41-year-old male presented with a 2-year history of progressive gait imbalance, headache, and abnormal speech. He had a famil
