Polymyositis--a further investigation.

In essence, this paper describes the model-based interpretation of measurements in order to gain insight into physiological processes. The paper extends the mathematical model developed previously to control and monitor a polymyositis patient. The extension considers two further yet different types of muscle attack. The model is applied to simulate, first, the total serum creatine kinase levels, which then are matched to the observed values. The outcomes of this approach yield considerable information about the reactions taking place including various concentration levels over time such as pre

Respiratory biofeedback-assisted therapy in panic disorder.

The authors describe a new methodologically improved behavioral treatment for panic patients using respiratory biofeedback from a handheld capnometry device. The treatment rationale is based on the assumption that sustained hypocapnia resulting from hyperventilation is a key mechanism in the production and maintenance of panic. The brief 4-week biofeedback therapy is aimed at voluntarily increasing self-monitored end-tidal partial pressure of carbon dioxide (PCO2) and reducing respiratory rate and instability through breathing exercises in patients' environment. Preliminary results from 4 pati

Determination of testis temperature rhythms and effects of constant light on testicular function in the domestic fowl (Gallus domesticus).

There is a wide range of opinions regarding the operating temperature of the testis in the domestic fowl. We used physiological monitoring techniques to investigate testis and body temperature over daily periods and under various light regimes to elucidate body temperature gradients in the fowl. We confirm that the operating temperature of the adult fowl's testes is equivalent to core body temperature (40-41 degrees C). Long-term continuous temperature monitoring showed that there was no difference between the temperature of the testis, liver, and peritoneum during a 24-h period either in a no

Clinical Implementation of Wearable-Derived Sleep and Activity Reporting for Inpatient Psychiatric Monitoring.

BACKGROUND: Sleep is a core component of psychiatric assessment, yet inpatient monitoring typically relies on brief observational checks that are subjective, variable, and sometimes disruptive. Wearable devices offer a means of capturing continuous, objective sleep and activity data without disturbing patients. Although digital health technologies are increasingly used in psychiatric research, little is known about how wearable-derived data can be integrated into routine inpatient workflows or used meaningfully by clinicians. OBJECTIVE: This implementation aimed to evaluate the feasibility, u

Standardized Lung Recruitment and Positive End-expiratory Pressure Titration in Acute Respiratory Distress Syndrome.

Acute Respiratory Distress Syndrome (ARDS) is characterized by diffuse alveolar injury, refractory hypoxemia, and high mortality. Lung recruitment maneuvers (RMs) aim to reopen collapsed alveoli and improve oxygenation, while positive end-expiratory pressure (PEEP) titration prevents decruitment and maintains alveolar stability. This protocol outlines five clinically feasible RM strategies: sigh, sustained inflation, stepwise PEEP recruitment, APRV-based recruitment, and prone positioning-assisted recruitment. Each maneuver is described with step-by-step procedures, parameter settings, monitor

Determining what patients admitted with a chronic obstructive pulmonary disease (COPD) exacerbation will use for remote clinical monitoring: a patient engagement survey.

BACKGROUND: Remote monitoring may improve the health of people with chronic obstructive pulmonary disease (COPD) through earlier detection and intervention before conditions worsen, but there are major challenges in recruitment and retention in these research studies. There are also increasing technologies and the uptake of specific technologies in people with COPD is not well known. OBJECTIVE: The objective of this study was to identify remote monitoring interventions that people admitted to hospital with an exacerbation of COPD would be willing to use upon discharge and to identify factors

Cellulose nanocomposite hydrogel-conductive, antimicrobial and high tenacity sensor for information encryption, human motion monitoring and animal behavior studies.

Conductive hydrogels have garnered significant attention in advanced applications, particularly in wearable strain sensors. Nevertheless, the challenge of fabricating wearable sensors that demonstrate both optimal mechanical properties and high conductivity persists. In this study, we successfully developed a multifunctional hydrogel sensor comprising acrylamide (AM), polyethylene glycol (PEG), and sodium carboxymethyl cellulose (CMC). The resulting hydrogel demonstrated favorable mechanical properties, electrical conductivity, adhesion, antimicrobial activity, antifreezing capability, moistur

Monitoring of cerebrovascular pressure reactivity in children may predict neurologic outcome after hypoxic-ischemic brain injury.

OBJECTIVES: Impaired cerebral blood flow is a first-line reason of ischemic-hypoxic brain injury in children. The principal goal of intensive care management is to detect and prevent further cerebral blood flow deficits. This can be achieved by actively managing cerebral perfusion pressure (CPP) using input from cerebrovascular autoregulation (CAR). The main objective of the current study was to investigate CAR after cardiac arrest in children. METHODS: Nineteen consecutive children younger than 18 years after cardiopulmonary resuscitation, in whom intracranial pressure (ICP) was continuously

Water removal during automated peritoneal dialysis assessed by remote patient monitoring and modelling of peritoneal tissue hydration.

Water removal which is a key treatment goal of automated peritoneal dialysis (APD) can be assessed cycle-by-cycle using remote patient monitoring (RPM). We analysed ultrafiltration patterns during night APD following a dry day (APDDD; no daytime fluid exchange) or wet day (APDWD; daytime exchange). Ultrafiltration for each APD exchange were recorded for 16 days using RPM in 14 patients. The distributed model of fluid and solute transport was applied to simulate APD and to explore the impact of changes in peritoneal tissue hydration on ultrafiltration. We found lower ultrafiltration (mL, median

Higher premature atrial complex burden from the Holter examination predicts poor cardiovascular outcome.

Premature atrial complexes (PACs) have been suggested to increase the risk of adverse events. The distribution of PAC burden and its dose-response effects on all-cause mortality and cardiovascular death had not been elucidated clearly. We analyzed 15,893 patients in a medical referral center from July 1st, 2011, to December 31st, 2018. Multivariate regression driven by ln PAC (beats per 24 h plus 1) or quartiles of PAC burden were examined. Older group had higher PAC burden than younger group (p for trend < 0.001), and both genders shared similar PACs distribution. In Cox model, ln PAC remaine

Usefulness of hematopoietic progenitor cell monitoring to predict autologous peripheral blood stem cell harvest timing: A single-center retrospective study.

INTRODUCTION: In autologous peripheral blood stem cell harvest (APBSCH), CD34-positive cells have been measured to assess the numbers of hematopoietic stem cells, but measurement requires specialized equipment. Recently, there was a report that peripheral blood hematopoietic progenitor cells (HPCs) are useful indicators of the presence of hematopoietic stem cells. We examined the usefulness of HPC monitoring to predict APBSCH timing. METHODS: We retrospectively analyzed the relationship between HPC and collected CD34-positive cells in 84 consecutive patients who underwent APBSCH. RESULTS: Ac

FITC-Labeled Alendronate as an In Vivo Bone pH Sensor.

pH is a critical indicator of bone physiological function and disease status; however, noninvasive and real-time sensing of bone pH in vivo has been a challenge. Here, we synthesized a bone pH sensor by labeling alendronate with the H+-sensitive dye fluorescein isothiocyanate (Aln-FITC). Aln-FITC showed selective affinity for hydroxyapatite (HAp) rather than other calcium materials. An in vivo biodistribution study showed that Aln-FITC can be rapidly and specifically delivered to rat bones after caudal vein injection, and the fluorescence lasted for at least 12 h. The fluorescence intensity of

Implication of Hemodynamic Assessment during Durable Left Ventricular Assist Device Support.

Durable left ventricular assist device therapy has improved survival in patients with advanced heart failure refractory to conventional medical therapy, although the readmission rates due to device-related comorbidities remain high. Left ventricular assist devices are designed to support a failing left ventricle through relief of congestion and improvement of cardiac output. However, many patients still have abnormal hemodynamics even though they may appear to be clinically stable. Furthermore, such abnormal hemodynamics are associated with an increased risk of future adverse events including

Neurological outcome in patients after successful resuscitation in out-of-hospital settings.

Neurological outcome is an important determinant of death in admitted survivors after out-of-hospital cardiac arrest (OHCA). Studies demonstrated several significant pre-hospital predictors of ischemic brain injury (time to resuscitation, time of resuscitation, and cause of OHCA). Our aim was to evaluate the relationship between post-resuscitation clinical parameters and neurological outcome in OHCA patients, when all recommended therapeutic strategies, including hypothermia, were on board. We retrospectively included consecutive 110 patients, admitted to the medical ICU after successful resus

Acute spinal subdural hematoma: A case report of spontaneous recovery from paraplegia.

RATIONALE: Spontaneous spinal subdural hematoma (SSDH) is a rare disease that can cause severe permanent neurological dysfunction. Here we present a case of spontaneous SSDH, in which a series of magnetic resonance images (MRIs) taken through the course of the disease facilitated understanding of the resolution process of the hematoma and the diagnosis of SSDH. PATIENT CONCERNS: A 59-year-old male presented with sudden severe back pain and rapid onset of paraplegia. This symptom had continued developing while he was transferred to the emergency department. Initial physical examination showed

Validation of Single Centre Pre-Mobile Atrial Fibrillation Apps for Continuous Monitoring of Atrial Fibrillation in a Real-World Setting: Pilot Cohort Study.

BACKGROUND: Atrial fibrillation is the most common recurrent arrhythmia in clinical practice, with most clinical events occurring outside the hospital. Low detection and nonadherence to guidelines are the primary obstacles to atrial fibrillation management. Photoplethysmography is a novel technology developed for atrial fibrillation screening. However, there has been limited validation of photoplethysmography-based smart devices for the detection of atrial fibrillation and its underlying clinical factors impacting detection. OBJECTIVE: This study aimed to explore the feasibility of photopleth
