An experimental study of the failure modes of the Gamma Locking Nail and AO Dynamic Hip Screw under static loading: a cadaveric study.

The sliding compression screw is widely regarded as the optimum treatment for intertrochanteric fractures of the femur, allowing bone fragments to impact until a bony support has been established across the fracture site. This study carried out biomechanical, cadaveric tests to establish the influence of direct static loading situations on the modes of failure of the Gamma Nail compared with the Dynamic Hip Screw (DHS). Clinical studies report DHS failures of lag screws cutting-out, bending of the lag screws and cortical screws pulling out causing plate loosening. Gamma Nail failures include l

Comparison of the effect of reamed and unreamed locked intramedullary nailing on blood flow in the callus and strength of union following fracture of the sheep tibia.

This study was performed to compare the effects of reamed and unreamed locked intramedullary nailing on blood flow in the callus and early strength of union in a fractured sheep tibia model. After the creation of a standardized short spiral fracture by three-point bending with torsion, each tibia was stabilized by the insertion of a locked intramedullary nail. Ten animals were allocated randomly into two groups: one that had reaming prior to nail insertion and one that did not. Blood flow was measured in real time with use of laser Doppler flowmetry. Endosteal perfusion was determined at the f

The new trochanteric nail TFN-A has a higher complication rate in the treatment of proximal femoral fractures than the predecessor model PFN-A.

AIMS: Trochanteric fractures (TF) are among the most frequent fractures and have major impact on the affected patients and the healthcare system. Cephalomedullary nails (CMN) are widely used for these fractures. Complications remain frequent, either due to comorbidities or concerning the implant. The goal of this retrospective, single-centre study was to compare the complication rates of two CMN from one manufacturer. MATERIALS AND METHODS: All internal fixations TF performed between 01/2017 and 07/2021 in our institution were analysed retrospectively. Follow-up was at least two years. The CM…

Lateral parapatellar approach versus transpatellar splitting approach in intramedullary nailing of diaphyseal tibia fractures, a prospective randomized controlled study.

BACKGROUND: Anterior knee pain (AKP) is a frequent complaint after tibial intramedullary nailing (IMN), and the choice of entry approach may contribute to its occurrence. This trial compared the lateral parapatellar semi-extended (LPP) approach versus the midline transpatellar splitting (MLTP) approach for IMN of closed diaphyseal tibial fractures, focusing on AKP and knee function. METHODS: In this single-blinded randomized controlled trial, 50 adults with acute closed AO/OTA 42 tibial shaft fractures were allocated 1:1 to IMN performed through either the LPP or MLTP approach. Follow-up asse

Evaluating polymethylmethacrylate (PMMA) augmentation in cephalomedullary nail fixation of hip fractures: a step towards evidence-based patient selection criteria.

INTRODUCTION: Intertrochanteric (IT) hip fractures are increasingly common among older adults and are associated with significant morbidity and mortality. Cephalomedullary nailing is widely adopted for the treatment of intertrochanteric (IT) fractures; however, implant cut-out or cut-through (COCT) continues to be a significant complication. Polymethylmethacrylate (PMMA) augmentation has been introduced to mitigate this risk, though its use is associated with potential drawbacks including thermal necrosis and cement leakage into the joint or fracture site. Therefore, the aim of this study was

Antifungal activity of free and alginate-encapsulated Punica granatum fruit peel extract against dermatophytes using a human nail model.

BACKGROUND: Dermatophytosis and onychomycosis are among the most prevalent superficial fungal infections worldwide and remain difficult to treat because of prolonged therapy, recurrence, and emerging antifungal resistance. Punica granatum fruit, pomegranate, has a peel rich in phenolic compounds with documented antimicrobial properties; however, the therapeutic application of plant-derived extracts may be limited by physicochemical instability and low bioavailability. AIMS: To evaluate the antifungal activity of a hydroalcoholic extract of pomegranate peel and its alginate-encapsulated formul

Elastic stable intramedullary nailing vs. hip spica casting management for postoperative femoral fractures in children with developmental dysplasia of the hip.

INTRODUCTION: Proximal femoral osteotomy (PFO) produces excellent clinical results for developmental dysplasia of the hip (DDH) in children. Femur fractures in the hips of children after PFO have been reported, and both elastic stable intramedullary nailing (ESIN) and hip spica casting have been used to treat these fractures. Usually, decision-making between ESIN and hip spica casting has been based on the fracture pattern and age. The purpose of this study was to compare the outcomes of elastic stable intramedullary nailing with those of hip spica casting management for postoperative femoral

Submuscular Plating, Titanium Elastic Nailing, and External Fixation for Pediatric Femoral Shaft Fractures: A Comparative Systematic Review and Meta-Analysis.

Pediatric femoral shaft fractures are commonly treated with titanium elastic nailing (TENS), submuscular plating (SMP), or external fixation (EF), but the optimal fixation strategy remains controversial. This systematic review and meta-analysis compared perioperative metrics, union, complications, and functional outcomes between SMP, TENS, and EF in skeletally immature patients. A comprehensive search of PubMed, Scopus, Web of Science, Embase, and Google Scholar (up to November 2025) identified comparative studies of diaphyseal pediatric femoral shaft fractures treated with at least two of the

Independent Risk Factors for Lag Screw Cut-Out: The Role of Open Reduction in Intertrochanteric Fracture Fixation With the Proximal Femoral Nail.

BACKGROUND Intertrochanteric femoral fractures are common in older adults, and proximal femoral nail (PFN) fixation is widely preferred due to its biomechanical advantages. Lag screw cut-out, defined as the progressive migration of the lag screw through the femoral head with loss of fixation, is one of the most serious mechanical complications after PFN fixation. This complication can result in mechanical failure and require revision surgery. Therefore, this study aimed to identify independent risk factors for lag screw cut-out and evaluate lag screw stability in patients without cut-out. MATE

Intraoperative Guidewire Breakage During Proximal Femoral Nailing Managed Through Direct Anterior Approach: A Case Report and Review of Literature.

CASE: We report a rare intraoperative complication during proximal femoral nailing for an intertrochanteric femur fracture in a 54-year-old male. During helical blade preparation, the guidewire migrated intra-articularly and fractured during withdrawal. Retention risked cartilage damage, while retrieval through the blade tract was unsuitable and threatened implant stability. With the nail in situ, safe surgical dislocation was not feasible. The fragment was successfully removed through an direct anterior approach. At 14 months follow-up the patient had returned to his pre injury activity level

Locking plate fixation versus intramedullary nailing fixation in displaced proximal humerus fractures: A meta-analysis of randomized controlled trials.

BACKGROUND: Proximal humerus fractures (PHFs) are common injuries in older adults. However, the optimal approach to open reduction and internal fixation (ORIF) of PHFs remains a subject of debate. We conducted an updated systematic review and meta-analysis of randomized controlled trials (RCTs) comparing the clinical outcomes between locking plate (LP) and intramedullary nailing (IMN) fixation in ORIF of PHFs with a focus on fracture type. METHODS: We searched PubMed, EMBASE, the Cochrane Library, Scopus, and the ClinicalTrials.gov registry for RCTs published before December 2022. The effects

The effect of Poller screw or monocortical plate augmentation on the stability of intramedullary nailing in proximal extra-articular tibial fractures: a biomechanical study.

BACKGROUND: Proximal extra-articular tibial fractures present considerable biomechanical and technical challenges due to the short proximal fragment, the wide metaphyseal canal, and the deforming forces acting around the knee. These factors predispose isolated intramedullary (IM) nailing to malalignment, prompting the use of adjunct reduction aids such as Poller screws or supplemental plates. This study aimed to compare the mechanical behavior of IM nailing alone with that augmented by Poller screws or a monocortical plate in a standardized proximal tibia fracture model. METHODS: Fifteen synt

Twenty-year Delayed Failure of an Intramedullary Humeral Nail Due to Shaft Non-union and its Surgical Management: A Case Report.

INTRODUCTION: Non-union following humeral shaft fractures is an uncommon yet disabling complication that may ultimately lead to implant failure in the presence of persistent mechanical instability. Delayed, non-traumatic breakage of an intramedullary nail several decades after fixation is exceedingly rare. CASE REPORT: We report the case of a 44-year-old male who presented 20 years after primary intramedullary interlocking nailing for a humeral shaft fracture with progressive arm pain, functional limitation, established mid-diaphyseal non-union, and spontaneous nail breakage in the absence of
