Wooden sticks used to suspend feces obtained for surveillance cultures were found to be the source of Rhizopus microsporus var. rhizopodiformis causing a pseudo-outbreak among 17 immunocompromised patients cared for in three different wards. Nonsterile wooden products should therefore not be used for collecting, handling, and processing specimens for microbiological examination.

Handwashing is the most important and least expensive measure to prevent transmission of nosocomial infections. However, compliance rarely exceeds 40% under study conditions. Alcoholic hand disinfection (AHD) generally is used in Europe. In contrast, handwashing with medicated soap is practiced most frequently in the United States. The life of Vincent van Gogh is illustrative of the natural history of psychosocial stressors and their relationship to a person's states of health and disease. In the author's opinion, there is a lack of such understanding in the current, established criteria for psychosocial stressors in the diagnosis of adjustment disorder. By use of a life chart, which chronologically documents a person's major life events and concomitant health status over his or her life span, a fuller understanding can be reached regarding why an individual becomes ill at a particular time.

Elderly persons are vulnerable to nutritional risks. Factors such as living alone make the elderly prone to poor nutritional status as a result of inadequate diets. The results of a Dutch national nutrition survey of food consumption and nutritional status among 539 independently living, apparently healthy elderly persons, aged 65 to 79 years, show that in general the intake of macronutrients deviates from the Dutch recommendations. In particular, saturated fat intake and P:S ratio were unfavorable, especially for the solitary elderly persons (saturated fat intake 19.0 +/- 3.2% and 17.7 +/- 3.

After 10 years of screening for breast cancer by mammography in Nijmegen, the predictive value of positive screening results (PV+) was evaluated. In a family study, sacroiliitis was found in 15 (25%) out of 61 HLA-B27 positive first-degree relatives (FDR) of 20 randomly chosen HLA-B27 positive ankylosing spondylitis (AS) patients and never in 41 HLA-B27 negative FDR. Eight (53%) of the 15 FDR with sacroiliitis fulfilled the New York criteria for AS. In the population study the prevalence of HLA-B27 positive AS was found to be 0.1%. AS will develop in 1.3% of all HLA-B27 positive individuals belonging to the general population.
