Application of transoral continuous intraoperative neuromonitoring in natural orifice transluminal endoscopic surgery for thyroid disease: a preliminary study.

BACKGROUND: The novel concept of continuous intraoperative neuromonitoring (Cont-IONM) through stimulation of the vagal nerve has been used in thyroidectomies to prevent imminent injury of the recurrent laryngeal nerve (RLN). This article reports on this technology and the results of using transoral Cont-IONM in natural orifice transluminal endoscopic surgery for thyroid disease. METHODS: Cont-IONM of the RLN was achieved through automatic cyclical stimulation of the vagal nerve using a C2 monitor and delta stimulating electrode. During the operation, three vestibular incisions were made, and

Chronic spatial working memory deficit associated with the superior longitudinal fasciculus: a study using voxel-based lesion-symptom mapping and intraoperative direct stimulation in right prefrontal glioma surgery.

OBJECTIVE Although the right prefrontal region is regarded as a silent area, chronic deficits of the executive function, including working memory (WM), could occur after resection of a right prefrontal glioma. This may be overlooked by postoperative standard examinations, and the disabilities could affect the patient's professional life. The right prefrontal region is a part of the frontoparietal network and is subserved by the superior longitudinal fasciculus (SLF); however, the role of the SLF in spatial WM is unclear. This study investigated a persistent spatial WM deficit in patients who u

Location of motor fibers within branches of the recurrent laryngeal nerve with extralaryngeal terminal bifurcation; Functional identification by intraoperative neuromonitoring.

BACKGROUND: Extralaryngeal terminal bifurcation (ETB) of the recurrent laryngeal nerve (RLN) is an anatomic variation that threatens the safety of thyroid operation. Therefore, it is important to identify motor function in nerve branches to preserve appropriate motor activity. Intraoperative neuromonitoring (IONM) is an accepted procedure to identify motor function of the RLN. METHODS: We established the operative anatomy of RLNs with ETB in 47 patients. The main trunk, bifurcation point, and the branches were identified and exposed completely during thyroid operation. The location of motor f

Preoperative donor nerve electromyography as a predictor of nerve transfer outcomes.

PURPOSE: We hypothesized that health of the donor nerve and corresponding muscle, as assessed by electromyography (EMG), could predict the outcome of nerve transfer surgery. METHODS: A retrospective review was performed to investigate outcomes of nerve transfers for elbow flexion and shoulder abduction. Motor strength was graded preoperatively and after a minimum 1-year follow-up. Preoperative EMG results were classified as functionally normal or affected based on motor unit recruitment pattern and correlated with follow-up motor strength and range of motion. RESULTS: Forty nerve transfers w

Fat tissue alters quadriceps response to femoral nerve magnetic stimulation.

OBJECTIVE: We investigate the influence that inguinal fat has on maximal quadriceps response to femoral nerve magnetic stimulation. METHODS: Vastus lateralis M-wave amplitude and quadriceps twitch peak torque were measured (i) in lean and overweight subjects and (ii) when slices of pig fat ranging from 2 to 18 mm in thickness were placed between the coil and the nerve. RESULTS: In overweight group, the maximal response could not be elicited when intensity was ≤90% and ≤85% of maximal power for twitch torque and M-wave amplitude, respectively. The maximal response was obtained at 80% of maxim

Predicting optimal electrical stimulation for repetitive human muscle activation.

Functional electrical stimulation is the use of electrical currents to activate paralyzed muscles to produce functional movements. Muscle force output must meet or exceed the external load to maintain a posture or produce movements. A mathematical force-fatigue modeling system that predicts muscle force responses during repetitive electrical stimulation has been developed in our laboratory to help identify stimulation patterns that optimize force output for individual subjects. This study tests how well this model predicts the number of contractions that can be maintained above a required forc

Chronic measurement of the stimulation selectivity of the flat interface nerve electrode.

The flat interface nerve electrode (FINE) is an attempt to improve the stimulation selectivity of extraneural electrodes. By reshaping peripheral nerves into elliptical cylinders, central fibers are moved closer to the nerve-electrode interface, and additional surface area is created for contact placement. The goals of this study were to test the hypothesis that greater nerve reshaping leads to improved selectivity and to examine the chronic recruitment properties of the FINE. Three FINEs were developed to reshape peripheral nerves to different degrees. Four electrodes of each type were implan

The effect of lengthening of the femur on the extensors of the knee. An electromyographic study.

Surface electromyography of the quadriceps femoris muscle was performed in seven patients in order to determine the effect of extensive lengthening of the femur on the muscles that extend the knee. Electromyograms were made during sustained (thirty-second) isometric extension of the knee, at an angle of flexion of the knee of 30 degrees, while a load equal to 15 per cent of body weight was applied to the leg just proximal to the ankle. The average circumference of the thigh was smaller, the motor-unit recruitment of the muscles was slower, and the fatigability was greater on the involved side

Anatomical and Temporal Blind Spots Explain the High False-Negative Rate of Motor Evoked Potential Monitoring in Glioma Surgery: A Single-Center Analysis of 106 Consecutive Patients.

BACKGROUND: Intraoperative motor evoked potential (MEP) monitoring is standard during glioma surgery, yet its diagnostic performance in cohorts enriched for deep-seated tumors remains poorly characterized. METHODS: We retrospectively analyzed 106 consecutive patients undergoing MEP-monitored glioma resection. False-negative cases were systematically reviewed to classify monitoring failure as anatomical blind spots (basal ganglia injury, supplementary motor area (SMA)/parietal involvement, thalamic injury) or temporal blind spots (delayed hemorrhage). Preoperative MRI variables were compared b

Acute Modulation of Lumbar Motor Unit Behavior During Experimentally Induced Low Back Pain: A Motor Unit Decomposition Analysis.

BACKGROUND AND PURPOSE: Acute low back pain is a prevalent musculoskeletal disorder that can disrupt motor control and compromise functional stability. Evidence suggests that pain alters motor unit discharge characteristics and the temporal organization of muscle activation; however, the specific neuromuscular adaptations associated with acute low back pain remain insufficiently understood. This study aimed to investigate the effects of experimentally induced acute low back pain on force steadiness and motor unit discharge characteristics (including recruitment threshold, motor unit firing rat

The Double-Deck Cervical Paraspinal Interfascial Block: Combined Cervical Cervicis Plane and Lower Cervical Erector Spinae Plane Blocks for C1-C2 Fusion Under SSEP/MEP Monitoring-A Case Report.

Analgesia for craniovertebral junction (CVJ) and high cervical posterior surgery is challenging because surgical exposure disrupts multilevel posterior cervical musculature and irritates the upper cervical dorsal rami, particularly C2-C3. Procedures performed with somatosensory evoked potential (SSEP) and motor evoked potential (MEP) monitoring also favor posterior, motor-sparing regional techniques. We describe a "double-deck" strategy combining an upper cervical cervicis plane (CCeP) block with a lower cervical erector spinae plane (ESP) block for posterior C1-C2 fusion performed under multi

Intraoperative use of lateral spread response measurement in the upper orbicularis oculi and mandibular muscles in patients with hemifacial spasm after botulinum toxin treatment.

BACKGROUND: Following repeated botulinum toxin (BTX) treatments, patients with hemifacial spasm (HFS) are recommended to undergo microvascular decompression (MVD) for a permanent cure. Intraoperative lateral spread response (LSR) monitoring is important to improve surgical outcomes. However, LSR monitoring during MVD surgery in HFS patients who have previously received BTX is challenging, since the muscles typically used to assess the LSR are paralyzed. Here, we describe our use of the upper part of the orbicularis oculi and mentalis muscles for intraoperative monitoring of the LSR during MVD
